Abstract

本发明提供了一种治疗桡骨远端骨折的桃红四物汤加味剂及其制备方法，属于中药技术领域。其桃红四物汤加味剂包括混合物A和混合物B，混合物A由以下原料制成：桃红四物汤原料、丹参、黑豆、薏苡仁、栀子、茯苓、香附、白术、大腹皮、蓬莪术、三棱、延胡索、大黄、泽兰、防己、佩兰、姜黄4‑6份、乳香；混合物B由以下原料制成：桃红四物汤原料、冰片、没药、生地、五灵脂、王不留行、蒲黄、白茅根、大小蓟、续断、接骨木、伸筋草、楤木、透骨草。混合物A口服，混合物B外敷。骨折复位采用口服和外敷桃红四物汤加味剂，促进活血化瘀，新骨增生，缓解肿痛，抗炎抗凝，加速骨折愈合，并减少骨折后并发症的发生。

技术领域
本发明涉及中药技术领域，具体是一种治疗桡骨远端骨折的桃红四物汤加味剂及其制备方法。
背景技术
桡骨远端骨折是一种频发的骨折类型之一，发生在桡骨下端2-3cm范围内的骨质松质部位，因此处松质、密质骨交界，发病率高且易累及关节面，骨折造成患者腕关节功能丧失，对其生活、工作造成极大不便，加重患者心里负担。随着老龄化趋势的发展，此病的发病率呈明显的上升趋势，掐中老年女性的发病率较高，且多为粉碎性骨折。
目前治疗桡骨远端骨折主要分为保守、手术两大类，保守治疗以手法复位为主，手术治疗切开复位为主，但不论是保守治疗还是手术治疗，复位后的恢复起着至关重要的作用，若复位后恢复不好，严重影响患者以后的生活，甚至可能导致患者残疾，老年人本身骨折愈合速度慢，如复位后不积极治疗，极易导致骨折后并发症的发生。中国专利文件CN105616052A公布了一种促进桡骨远端骨折愈合治疗方法，骨折复位固定后，口服壮筋补骨丸进行恢复，该发明采用壮筋补骨丸可以补气活血，续筋接骨，但骨折愈合速度慢，消肿慢；中国专利文件CN104524066A公布了用于治疗桡骨远端骨折的接骨膏，将药膏摊贴在夹板上，加速骨折愈合，但只外敷，不内活血化瘀，愈合速度还是慢，且疗效一般。本发明人在多年临床实践中，桡骨远端骨折复位后，利用中药口服和外敷治疗，取得了很好的疗效，且无毒副作用。
发明内容
本发明目的是提供一种治疗桡骨远端骨折的桃红四物汤加味剂及其制备方法。
为了达到上述目的，本发明采用以下技术方案。
本发明的桃红四物汤加味剂包括混合物A和混合物B，混合物A由以下重量份数的原料制成：桃红四物汤原料15-18份、丹参2-3份、黑豆5-6份、薏苡仁3-4份、栀子2-3份、茯苓0.8-1.2份、香附0.6-0.8份、白术1.1-1.5份、大腹皮0.6-1.1份、蓬莪术1.5-3.0份、三棱3-4份、延胡索5-8份、大黄6-8份、泽兰2-3份、防己3-4份、佩兰1-2份、姜黄4-6份、乳香2-3份；混合物B由以下重量份数的原料制成：桃红四物汤原料10-12份、冰片1-2份、没药2-3份、生地0.8-1.5份、五灵脂1.8-2.5份、王不留行1-2份、蒲黄1-2份、白茅根0.8-1.2份、大小蓟1-2份、续断5-6份、接骨木6-9份、伸筋草5-7份、楤木4-6份、透骨草3-5份。
上述的桃红四物汤具有养血活血、扩张血管、抗炎、抗疲劳、抗休克、调节免疫功能、降脂、补充微量元素、抗过敏等作用；丹参味苦，微寒，无毒，具有活血祛瘀，通经止痛，清心除烦，凉血消痈之功效；黑豆富含对人体有益的氨基酸、不饱和脂肪酸及钙、磷等多种微量元素，有排脓拔毒、消肿止痛之功效；薏苡仁味甘淡微寒，具有利水消肿、健脾去湿、舒筋除痹、清热排脓等功效；栀子味苦、寒、无毒，具有泻火除烦、清热利湿、凉血解毒、疮疡肿毒、治疗扭伤肿痛的功效；茯苓性味甘淡平，具有增强免疫力、抗肿瘤、抗菌以及治疗水肿胀满的作用；香附，气辛，味微苦，具有疏肝解郁，理气宽中，调经止痛的功效；白术味苦，甘，性温，具有健脾益气，燥湿利水，止汗，去水肿的功效；大腹皮味辛，性微温，具有行气宽中，行水消肿的功效；蓬莪术苦辛，温，具有行气，破血，消积，止痛，治心腹胀痛，百瘕，积聚，跌打损伤作痛的功效；三棱味辛、苦，平，具有破血行气，消积止痛的功效；延胡索性味辛，苦，温，无毒，用于行气止痛、活血散瘀、跌打损伤等；大黄性味苦，寒，具有攻积滞、清湿热、泻火、凉血、祛瘀、解毒等功效；泽兰味苦、辛，有活血通经、利水消肿的功效；防己味辛、苦，性寒，具有祛风止痛，利水消肿的功效；佩兰具有解热清暑、化湿健胃、止呕的作用；姜黄性味辛，苦，温，具有破血，行气，通经，止痛的功效；乳香性味辛，苦，温，具有活血行气止痛，消肿生肌的功效；冰片性味辛，苦，凉，具有开窍醒神，清热散毒，明目退翳的功效；没药性味辛，苦，平，主治癥瘕腹痛，风湿痹痛，跌打损伤，痈肿疮疡等病症；生地性味甘，寒，具有清热凉血，养阴，生津的功效；五灵脂性味甘，温，具有活血散瘀，炒炭止血，外用治跌打损伤的功效；王不留行性味苦，平，具有活血通经，下乳消肿，利尿通淋的功效；蒲黄性味甘，平，用于治疗吐血，崩漏，外伤出血，经闭通经，胸腹刺痛，跌扑肿痛，血淋涩痛；白茅根性味甘甜，寒，具有凉血止血，清热利尿的功效；大小蓟性味甘，凉温，具有破血行瘀，凉血止血，怯痰消肿，保精养血，退热补虚，坐补诸经之血的功效；续断性味苦，辛，温，具有补肝肾、强筋骨、续折伤、调血脉的功效；接骨木性味甘，苦，平，具有治疗风湿筋骨疼痛，跌扑损伤，骨折，创伤出血的功效；伸筋草性味辛，苦，温，具有舒筋活络，消肿止痛，治跌打损伤，瘀肿疼痛的功效；楤木性味辛，苦，具有祛风除湿，利水和中，活血解毒的功效；透骨草性味甘，辛，具有活血化瘀、利尿解毒、通经透骨之功效。
本发明桃红四物汤加味剂的制备方法为：将桃红四物汤原料当归、白芍、熟地黄、川芎、桃仁、红花混合后，放入专用中药锅中，用药物重量的25-30倍冷水浸泡20-30min后，用大火煎开，改文火煎制30-40min，将药液全部倒出过滤，再加入混合物A的其他所有药材，加药物重量的20倍冷水浸泡30-40min后，用大火煎开，改文火煎制60-90min，将药液全部倒出过滤，两次煎煮过滤后得到的药液即混合物A；将混合物B按比例混合，经中药材粉碎机粉碎后，过100-120目筛，将粉碎过筛的粉末用浓度为80％-90％的乙醇回流提取3-4次，提取液合并，将提取液减压浓缩成膏体，将浓缩后的膏体干燥成粉末，将乙醇提取后的粉末加水浸泡30-40min后，用大火烧开，改文火煎制4-5h后过滤，将滤液减压浓缩成膏体，将浓缩的膏体干燥成粉末，将两次干燥的粉末混合在一起，加凡士林制成药膏，即混合物B；加乙醇的量为过筛药末重量的4-6倍，加水的量为药末重量的4-6倍，凡士林的量为混合粉末的3-4倍。
本发明桃红四物汤加味剂的使用方法为病人桡骨骨折复位夹板固定后，口服混合物A，每天2次，外敷混合物B，每天敷2-3h，连续治疗2-3个月，治疗期间不服用其他药物。
本发明的有益技术效果：桡骨骨折复位夹板固定后采用口服和外敷桃红四物汤加味药剂，促进活血化瘀，新骨增生，缓解肿痛，抗炎抗凝，加速骨折愈合，并减少骨折后并发症的发生。
具体实施方式
下面结合实施例进一步对本发明的技术方案作详细的阐述，但不都成对本发明的范围性限制。
实施例1
称取桃红四物汤红原料当归3g、白芍3g、熟地黄3g、川芎3g、桃仁3g、红花3g加入专用中药锅中，加入500ml冷水浸泡30min后，用大火煎开，改文火煎制40min，将药液全部倒出过滤后，在锅中倒入丹参3g、黑豆6g、薏苡仁4g、栀子3g、茯苓1.2g、香附0.8g、白术1.5g、大腹皮1.1g、蓬莪术3g、三棱4g、延胡索8g、大黄8g、泽兰3g、防己4g、佩兰2g、姜黄6g、乳香3g，加2500ml冷水浸泡40min后，用大火煎开，改文火煎制90min，将药液全部倒出过滤，两次煎煮过滤后得到的药液即混合物A；将桃红四物汤红原料当归2g、白芍2g、熟地黄2g、川芎2g、桃仁2g、红花1g、冰片2g、没药3g、生地1.5g、五灵脂2.5g、王不留行2g、蒲黄2g、白茅根1.2g、大小蓟2g、续断6g、接骨木9g、伸筋草7g、楤木6g、透骨草5g混合在一起，经中药材粉碎机粉碎后，过120目筛，将粉碎过筛的粉末用250ml浓度为90％的乙醇回流提取4次，提取液合并，将提取液减压浓缩成膏体，将浓缩后的膏体干燥成粉末，将乙醇提取后的粉末加水3000ml浸泡40min后，用大火烧开，改文火煎制5h后过滤，将滤液减压浓缩成膏体，将浓缩的膏体干燥成粉末，将两次干燥的粉末混合在一起，加凡士林制成药膏，即混合物B；病人桡骨骨折复位夹板固定后，口服混合物A，每天2次，外敷混合物B，每天敷3h，连续治疗2个月，治疗期间不服用其他药物。
实施例2
称取桃红四物汤红原料当归2.5g、白芍2.5g、熟地黄2.5g、川芎2.5g、桃仁2.5g、红花2.5g加入专用中药锅中，加入400ml冷水浸泡20min后，用大火煎开，改文火煎制30min，将药液全部倒出过滤后，在锅中倒入丹参2g、黑豆5g、薏苡仁3g、栀子2g、茯苓0.8g、香附0.6g、白术1.1g、大腹皮0.6g、蓬莪术1.5g、三棱3g、延胡索5g、大黄6g、泽兰2g、防己3g、佩兰1g、姜黄4g、乳香2g，加2000ml冷水浸泡30min后，用大火煎开，改文火煎制60min，将药液全部倒出过滤，两次煎煮过滤后得到的药液即混合物A；将将桃红四物汤红原料当归2g、白芍2g、熟地黄2g、川芎2g、桃仁2g、红花2g、冰片1g、没药2g、生地0.8g、五灵脂1.8g、王不留行1g、蒲黄1g、白茅根0.8g、大小蓟1g、续断5g、接骨木6g、伸筋草5g、楤木4g、透骨草3g混合在一起，经中药材粉碎机粉碎后，过100目筛，将粉碎过筛的粉末用200ml浓度为80％的乙醇回流提取3次，提取液合并，将提取液减压浓缩成膏体，将浓缩后的膏体干燥成粉末，将乙醇提取后的粉末加水2500ml浸泡30min后，用大火烧开，改文火煎制4h后过滤，将滤液减压浓缩成膏体，将浓缩的膏体干燥成粉末，将两次干燥的粉末混合在一起，加凡士林制成药膏，即混合物B；病人桡骨骨折复位夹板固定后，口服混合物A，每天2次，外敷混合物B，每天敷2h，连续治疗2个月，治疗期间不服用其他药物。
实施例3
称取桃红四物汤红原料当归3g、白芍3g、熟地黄3g、川芎3g、桃仁3g、红花2g加入专用中药锅中，加入450ml冷水浸泡25min后，用大火煎开，改文火煎制35min，将药液全部倒出过滤后，在锅中倒入丹参2.5g、黑豆5.5g、薏苡仁3.5g、栀子2.5g、茯苓1g、香附0.7g、白术1.3g、大腹皮0.9g、蓬莪术2.2g、三棱3.5g、延胡索6g、大黄7g、泽兰2.5g、防己3.5g、佩兰2.2g、姜黄5g、乳香2.5g，加2500ml冷水浸泡35min后，用大火煎开，改文火煎制75min，将药液全部倒出过滤，两次煎煮过滤后得到的药液即混合物A；将将桃红四物汤红原料当归2g、白芍2g、熟地黄1.5g、川芎1.5g、桃仁1.5g、红花1.5g、冰片1.5g、没药2.5g、生地1.2g、五灵脂2.2g、王不留行1.5g、蒲黄1.5g、白茅根1g、大小蓟1.5g、续断5.5g、接骨木7g、伸筋草6g、楤木5g、透骨草4g混合在一起，经中药材粉碎机粉碎后，过120目筛，将粉碎过筛的粉末用220ml浓度为85％的乙醇回流提取3次，提取液合并，将提取液减压浓缩成膏体，将浓缩后的膏体干燥成粉末，将乙醇提取后的粉末加水2700ml浸泡35min后，用大火烧开，改文火煎制4.5h后过滤，将滤液减压浓缩成膏体，将浓缩的膏体干燥成粉末，将两次干燥的粉末混合在一起，加凡士林制成药膏，即混合物B；病人桡骨骨折复位夹板固定后，口服混合物A，每天2次，外敷混合物B，每天敷3h，连续治疗2个月，治疗期间不服用其他药物。
本发明药物的临床疗效和安全性验证
随机选取2015年8月至2016年8月桡骨远端骨折的女性患者48例，年龄40-65岁，分为两组，对照组24例采用跌打散外敷+口服骨折挫伤胶囊治疗2个月，实验组24例采用本发明实施例3的方法进行治疗2个月。
诊断标准：临床检查提示腕关节肿胀，有疼痛感，骨折位移明显可见腕部畸形，桡骨远端压痛明显，腕关节及前臂旋转活动受限，腕关节正侧位X线检查提示桡骨远端骨折，经复位夹板固定后骨折达到功能复位标准。
疗效判定标准：治疗2个月后参照腕关节的Dienst等功能标准评价其腕关节功能情况，优：无疼痛，活动不受限，功能无损伤；良：偶尔疼痛，运动轻微受限，无残疾；可：偶尔疼痛，运动有限受限，感到腕关节无力；差：疼痛，活动明显受限，残疾；安全性评价：治疗期间身体无不适，局部皮肤无破损、红疹、水泡等反应。
表1两组疗效比较
由表1可以看出，利用本发明的桃红四物汤加味剂治疗桡骨远端骨折，骨折愈合快且效果好，无术后并发症且药物安全。

Claims (3)

1.一种治疗桡骨远端骨折的桃红四物汤加味剂，其特征在于，包括混合物A和混合物B，混合物A由以下重量份数的原料制成：桃红四物汤原料15-18份、丹参2-3份、黑豆5-6份、薏苡仁3-4份、栀子2-3份、茯苓0.8-1.2份、香附0.6-0.8份、白术1.1-1.5份、大腹皮0.6-1.1份、蓬莪术1.5-3.0份、三棱3-4份、延胡索5-8份、大黄6-8份、泽兰2-3份、防己3-4份、佩兰1-2份、姜黄4-6份、乳香2-3份；混合物B由以下重量份数的原料制成：桃红四物汤原料10-12份、冰片1-2份、没药2-3份、生地0.8-1.5份、五灵脂1.8-2.5份、王不留行1-2份、蒲黄1-2份、白茅根0.8-1.2份、大小蓟1-2份、续断5-6份、接骨木6-9份、伸筋草5-7份、楤木4-6份、透骨草3-5份。
2.根据权利要求1所述的一种治疗桡骨远端骨折的桃红四物汤加味剂的制备方法，其特征在于，将桃红四物汤的原料当归、白芍、熟地黄、川芎、桃仁、红花混合后，放入专用中药锅中，用药物重量的25-30倍冷水浸泡20-30min后，用大火煎开，改文火煎制30-40min，将药液全部倒出过滤，再加入混合物A的其他所有药材，加药物重量的20倍冷水浸泡30-40min后，用大火煎开，改文火煎制60-90min，将药液全部倒出过滤，两次煎煮过滤后得到的药液即混合物A；将混合物B按比例混合，经中药材粉碎机粉碎后，过100-120目筛，将粉碎过筛的粉末用浓度为80％-90％的乙醇回流提取3-4次，提取液合并，将提取液减压浓缩成膏体，将浓缩后的膏体干燥成粉末，将乙醇提取后的粉末加水浸泡30-40min后，用大火烧开，改文火煎制4-5h后过滤，将滤液减压浓缩成膏体，将浓缩的膏体干燥成粉末，将两次干燥的粉末混合在一起，加凡士林制成药膏，即混合物B；加乙醇的量为过筛药末重量的4-6倍，加水的量为药末重量的4-6倍，凡士林的量为混合粉末的3-4倍。
3.根据权利要求1所述的一种治疗桡骨远端骨折的桃红四物汤加味剂的使用方法，其特征在于，病人桡骨骨折复位夹板固定后，口服混合物A，每天2次，外敷混合物B，每天敷2-3h，连续治疗2-3个月，治疗期间不服用其他药物。
