Abstract
本发明公开了一种可撑开颈椎融合器,包括弹性的融合器本体和螺钉,融合体本体包括上终板支撑部和下终板支撑部,上终板支撑部和下终板支撑部的前端通过连接部相连,上终板支撑部的底面和下终板支撑部的顶面设有正对设置的螺纹槽,螺纹槽内攻内螺纹,螺钉具有圆锥面,并在圆锥面上攻外螺纹,螺钉可旋入螺纹槽内并将上终板支撑部和下终板支撑部向上下两侧撑开。融合器本体采用可变形结构设计,上终板支撑部和下终板支撑部在植入初始上下面平行,方便操作,植入到位后,可通过螺钉将其撑开,从而维持需要的颈椎弧度。本发明融合器体积小巧,发挥支撑功能,采用先植入融合器本体,然后在周围填充植骨材料的方法实现支撑与融合功能的结合。
Description
技术领域
本发明用于医疗用品技术领域,特别是涉及一种可撑开颈椎融合器。
背景技术
颈椎前路减压手术常常需要行颈椎间盘摘除及椎间植骨融合和钢板固定。传统的手术方式是取块状的髂骨,修整为适当大小后植入病变椎间隙,再用钢板固定。由于该手术方式需要在病人的髂部另取切口,增加了手术创伤,且取骨部位的并发症较多。所以,这一手术方式已经逐渐被椎间融合器所取代。所谓椎间融合器是一个用钛合金或其他高分子材料制作的中空长方体结构。其中空部位可以填充手术中取下的碎骨或异体骨,人工骨等。融合器可以替代髂骨块的作用,支和维持椎间隙高度,并发挥诱导骨融合的功能。目前用于临床的产品中,融合器一般设计为固定形状的圆柱体或方形柱体,其上下面与相邻颈椎的软骨终板接触,发挥支撑功能。融合器材内部中空,填入自体松质骨或人工骨,发挥骨诱导功能,促使相邻脊椎终板面的骨长入融合器内部或在融合器外部空间填充植骨材料,诱导相邻的脊椎形成骨性融合。
传统的颈椎融合器多为固定形状设计,对于大多数病例是适合的。手术一般通过试模测试手术节段的椎间隙大小,然后选择合适大小的融合器植入。但由于颈椎生理前突弧度的存在,正常人的颈椎椎间隙不是平行的,而是存在一定张角,普通融合器大多呈上下终板接触面呈平行状态,这不利于回复颈椎正常的前凸弧度。当实施单个节段的颈椎间盘手术时,这种影响并不明显,而实施多个节段的颈椎融合手术时,这种影响不容忽略,对多个颈椎间隙应用平行的固定融合器实施手术,医生会发现,患者颈椎生理弧度消失了,颈椎变成直的了,这对患者术后的颈椎功能恢复是不利的。
发明内容
为解决上述问题,本发明提供一种颈椎弧度可以调节的可撑开颈椎融合器,融合器在植入初始状态下是上下面平行的,方便操作,植入到位后,可以通过撑开装置将其张开,从而维持需要的颈椎弧度。
本发明解决其技术问题所采用的技术方案是:一种可撑开颈椎融合器,包括弹性的融合器本体和螺钉,所述融合体本体包括上终板支撑部和下终板支撑部,所述上终板支撑部和下终板支撑部的前端通过连接部相连,所述上终板支撑部的底面和下终板支撑部的顶面设有正对设置的螺纹槽,螺纹槽内攻内螺纹,所述螺钉具有圆锥面,并在所述圆锥面上攻外螺纹,所述螺钉可旋入螺纹槽内并将上终板支撑部和下终板支撑部向上下两侧撑开。
进一步作为本发明技术方案的改进,所述上终板支撑部的顶面和下终板支撑部的底面均设有增阻齿。
进一步作为本发明技术方案的改进,所述融合器本体的表面涂有HA涂层。
进一步作为本发明技术方案的改进,所述螺钉的前端面具有圆顶,后端面设有内六角沉孔。
进一步作为本发明技术方案的改进,在所述螺钉未旋入螺纹槽时,所述上终板支撑部的顶面和下终板支撑部的底面平行。
进一步作为本发明技术方案的改进,所述融合器本体为钛合金制件或聚醚醚酮制件,所述螺钉为钛合金制件。
进一步作为本发明技术方案的改进,所述融合器本体呈U形,所述上终板支撑部和下终板支撑部的厚度均大于连接部的厚度。
进一步作为本发明技术方案的改进,所述螺纹槽位于上终板支撑部底面和下终板支撑部顶面的中部。
本发明的有益效果:本可撑开颈椎融合器,融合器本体采用可变形结构设计,上终板支撑部和下终板支撑部在植入初始状态下是上下面平行的,方便操作,植入到位后,可通过螺钉旋入螺纹槽将其撑开,从而维持需要的颈椎弧度。本发明融合器体积小巧,发挥支撑功能,采用先植入融合器本体,然后在周围填充植骨材料的方法实现支撑与融合功能的结合。

具体实施方式
参照图1至图3,其显示出了本发明之较佳实施例的具体结构。以下将详细说明本发明各元件的结构特点,而如果有描述到方向( 上、下、左、右、前及后) 时,是以图3所示的结构为参考描述,但本发明的实际使用方向并不局限如此。
本发明提供了一种可撑开颈椎融合器,包括弹性的融合器本体1和螺钉2,所述融合体本体1包括上终板支撑部11和下终板支撑部12,所述上终板支撑部11和下终板支撑部12的前端通过连接部13相连,所述上终板支撑部11的底面和下终板支撑部12的顶面设有正对设置的螺纹槽14,螺纹槽14内攻内螺纹,所述螺钉2的前端面具有圆顶21,后端面设有内六角沉孔22。所述螺钉2具有圆锥面,并在所述圆锥面上攻外螺纹,所述螺钉2可旋入螺纹槽14内并将上终板支撑部11和下终板支撑部12向上下两侧撑开。所述融合器本体1呈U形,所述上终板支撑部11和下终板支撑部12的厚度均大于连接部13的厚度,保证螺钉2旋入螺纹槽14内时,连接部13完成变形要求。所述螺纹槽14位于上终板支撑部11底面和下终板支撑部12顶面的中部。所述上终板支撑部11的顶面和下终板支撑部12的底面均设有增阻齿15。所述融合器本体1的表面涂有HA涂层,HA涂层即羟基磷灰石,促进界面的骨长入与融合。
在所述螺钉2未旋入螺纹槽14时,所述上终板支撑部11的顶面和下终板支撑部12的底面平行,方便置入操作。融合器本体1采用医用钛合金或PEEK(聚醚醚酮)或其他可植入材料制作,螺钉2使用钛合金制造。
手术采用气管插管,全身麻醉。患者取仰卧位,背部垫高,尽量将头及下颌骨后仰,避免下颌骨对高位颈椎手术的遮挡。如果通过调整体外,预计下颌骨仍将对上位颈椎的垂直植钉造成困难时,便可选用该固定装置。
手术先切除病变椎间盘,对病变间隙做彻底减压。然后用试模测试椎间隙高度,选择合适尺寸的融合器本体1打入椎间隙,直至钢板与下位椎体紧密贴合。然后在融合器本体1周围的空间植入松质骨粒或人工骨诱导材料。选择一枚合适大小的螺钉2,顺融合器本体1的螺纹槽14缓慢拧入,将融合器本体1的上终板支撑部11和下终板支撑部12逐步撑开,完成固定。最后安装颈椎前路钢板,完成手术。
当然,本发明创造并不局限于上述实施方式,熟悉本领域的技术人员在不违背本发明精神的前提下还可作出等同变形或替换,这些等同的变型或替换均包含在本申请权利要求所限定的范围内。

1.一种可撑开颈椎融合器,其特征在于:包括弹性的融合器本体和螺钉,所述融合体本体包括上终板支撑部和下终板支撑部,所述上终板支撑部和下终板支撑部的前端通过连接部相连,所述上终板支撑部的底面和下终板支撑部的顶面设有正对设置的螺纹槽,螺纹槽内攻内螺纹,所述螺钉具有圆锥面,并在所述圆锥面上攻外螺纹,所述螺钉可旋入螺纹槽内并将上终板支撑部和下终板支撑部向上下两侧撑开。
2.根据权利要求1所述的可撑开颈椎融合器,其特征在于:所述上终板支撑部的顶面和下终板支撑部的底面均设有增阻齿。
3.根据权利要求1或2所述的可撑开颈椎融合器,其特征在于:所述融合器本体的表面涂有HA涂层。
4.根据权利要求1或2所述的可撑开颈椎融合器,其特征在于:所述螺钉的前端面具有圆顶,后端面设有内六角沉孔。
5.根据权利要求1或2所述的可撑开颈椎融合器,其特征在于:在所述螺钉未旋入螺纹槽时,所述上终板支撑部的顶面和下终板支撑部的底面平行。
6.根据权利要求1或2所述的可撑开颈椎融合器,其特征在于:所述融合器本体为钛合金制件或聚醚醚酮制件,所述螺钉为钛合金制件。
7.根据权利要求6所述的可撑开颈椎融合器,其特征在于:所述融合器本体呈U形,所述上终板支撑部和下终板支撑部的厚度均大于连接部的厚度。
8.根据权利要求1或2所述的可撑开颈椎融合器,其特征在于:所述螺纹槽位于上终板支撑部底面和下终板支撑部顶面的中部。
