Abstract
本发明公开一种颈椎跨界椎间高度维持器,属于医疗器械领域。该发明包括上终板、下终板、销轴,所述上终板与下终板上下重叠,上终板与下终板一端通过销轴连接,所述上终板和下终板内设置通道,上终板的通道连通上终板的上表面和侧面,下终板的通道连通下终板的下表面和侧面。本发明创新性提出跨界设计思路,使得颈椎跨界椎间高度维持器实现了融合、置换两个理念一体化设计,所设计颈椎跨界椎间高度维持器集人工颈椎间盘的运动保留、即刻稳定性优良,椎间融合器的高度维持、远期稳定性好、能改善并维持颈椎曲度的优点于一身,又采用了椎间零切迹的设计元素,较大程度避免了置换、融合的不足,更显著地扩大了适应症患者群,具有极高的实用性。
Description
技术领域
本发明涉及一种医疗器械,特别涉及一种颈椎跨界椎间高度维持器。
背景技术
人工颈椎椎间盘置换术是20世纪后期出现的新技术,是在前路椎间盘切除后通过在椎间隙置入一个可以活动的装置,代替原来的椎间盘并行使其功能,实现保留运动节段、减少相邻节段出现继发性退变的目的。在此之前,传统的颈椎病手术治疗方法是:将病变的椎间盘切除,然后在上、下椎体之间植骨,填补椎间盘切除后留下的空间、同时促使上、下椎体完全融合在一起,来达到治疗的目的。但通过临床效果的研究发现:前路融合后,与融合节段相邻的节段会产生继发性退变和不稳定,造成原有症状复发或加重,成为影响前路手术治疗颈椎病远期疗效的一个重要因素。由于融合术后节段获得了一种非生理状态的稳定,失去了相应的运动节段,造成颈椎总活动度减少,相邻节段退变加快,有的甚至需要再接受相邻节段的融合术。颈椎间隙融合手术(ACDF)是颈椎前路手术中经典的术式,早期的ACDF手术使用椎间植入髂骨块/人工材料植骨,上下椎体前方用钢板螺钉的方式固定。近年来,ACDF内固定器械发展到了椎间高分子材料+植骨+螺钉的内固定模式,即零切迹的内固定系统。能有效减少椎前软组织的不适感。但ACDF手术本身存在不足,主要是融合后颈椎节段活动度的丧失和临近椎间盘退变加速两房面。
现颈椎椎间盘系统是一种可以模拟正常人类颈椎椎间盘的特殊装置。当人们患有颈椎病或急性颈椎椎间盘突出症等原因需要手术治疗时,病变的椎间盘将被切除,然后将人工椎间盘植入原来的椎间盘的位置来代替原有椎间盘并行使其功能,可实现保留运动节段、减少出现相邻节段继发性退变的目的。目前全球已用于临床的人工颈椎间盘系统的设计原理均能实现保留置换节段的活动,并不同程度维持稳定的目标。但患者颈椎退变程度不一、以及术者操作判断标准各异等原因,使人工颈椎间盘置换术后会存在颈椎出现节段性反弓、节段活动受限或节段活动过度等并发症,影响患者术后疗效,严重者可能出现节段高度明显丢失,出现新的临床症状,同时甚至可能加重临近节段颈椎退变。
发明内容
本发明创新性提出跨界设计思路,使得人工椎间高度维持器实现了融合、置换两个理念一体化设计,所设计椎间高度维持器集人工颈椎间盘的运动保留、即刻稳定性优良,椎间融合器的高度维持、远期稳定性好、能改善并维持颈椎曲度的优点于一身,又采用了椎间零切迹的设计元素,较大程度避免了置换、融合的不足,更显著地扩大了适应症患者群,具有极高的实用性。
为达到上述目的,本发明采用的技术方案如下:
本发明公开的一种颈椎跨界椎间高度维持器,包括上终板、下终板、销轴,所述上终板与下终板上下重叠,上终板与下终板一端通过销轴连接,所述上终板和下终板内设置通道,上终板的通道贯穿上终板的上表面和侧面,下终板的通道贯穿下终板的下表面和侧面。
优选的,所述上终板的上表面和下终板的下表面均为向外凸出的弧形,且均设有尖嵴。
优选的,所述上终板和下终板的尖嵴各有六个,六个尖嵴均匀分布。
进一步的,所述上终板的通道在上表面的开口有五个,五个开口均匀分布,所述下终板的通道在下表面的开口有五个,五个开口均匀分布。
进一步的,所述上终板的通道在侧面的开口位于远离销轴的一端,所述下终板的通道在侧面的开口位于远离销轴的一端。
优选的,所述通道直径为2mm。
优选的,所述销轴为钛合金销轴。
进一步的,所述上终板和下终板与销轴连接处至最近边缘设置限制上终板与下终板张开到最大11°夹角的斜面。
优选的,所述上终板的上表面和下终板的下表面均设有羟基磷灰石涂层。
本发明具有以下有益效果:
1.本发明能够维持椎间高度;
2.弧形和尖嵴的设计使本发明置入后即刻稳定,尖嵴设计还促成了零切迹的效果;
3.通道的设计,充分引流终板间出血,同时也可用于加入非固态材料,促进骨板界面愈合;
4.本设计假体不会存在上下板前方闭合、后方张开的姿态,故不存在手术节段反弓的可能;
5.钛合金销轴与斜面的设计充分允许后伸角度;
6.羟基磷灰石涂层,促进骨-板界面骨融合。

为了使本发明的目的、技术方案及优点更加清楚明白,以下结合附图,对本发明进行进一步详细说明。
如图1-3所示,本发明包括上终板1、下终板2、销轴5,上终板1与下终板2上下重叠,上终板1与下终板2一端通过销轴5连接,上终板1和下终板2内设置通道3,上终板1的通道3贯穿上终板1的上表面和侧面,下终板2的通道3贯穿下终板2的下表面和侧面,上终板1的上表面和下终板2的下表面均为向外凸出的弧形,两个弧形表面各设有六个尖嵴4,六个尖嵴4均匀分布于其所在表面,上终板1的通道3在上表面的开口有五个,五个开口均匀分布,下终板2的通道3在下表面的开口有五个,五个开口均匀分布,上终板1的通道3在侧面的开口位于远离销轴5的一端,下终板2的通道3在侧面的开口位于远离销轴5的一端,通道3直径为2mm,销轴5为钛合金销轴,上终板1和下终板2与销轴5连接处至最近边缘设置限制上终板1与下终板2张开到最大11°夹角的斜面6,上终板1的上表面和下终板2的下表面均设有羟基磷灰石涂层。
当然,本发明还可有其它多种实施例,在不背离本发明精神及其实质的情况下,熟悉本领域的技术人员可根据本发明作出各种相应的改变和变形,但这些相应的改变和变形都应属于本发明所附的权利要求的保护范围。
Claims (9)
1.颈椎跨界椎间高度维持器,其特征在于:包括上终板、下终板、销轴,所述上终板与下终板上下重叠,上终板与下终板一端通过销轴连接,所述上终板和下终板内设置通道,上终板的通道贯穿上终板的上表面和侧面,下终板的通道贯穿下终板的下表面和侧面。
2.根据权利要求1所述的颈椎跨界椎间高度维持器,其特征在于:所述上终板的上表面和下终板的下表面均为向外凸出的弧形,且均设有尖嵴。
3.根据权利要求2所述的颈椎跨界椎间高度维持器,其特征在于:所述上终板和下终板的尖嵴各有六个,六个尖嵴均匀分布。
4.根据权利要求1所述的颈椎跨界椎间高度维持器,其特征在于:所述上终板的通道在上表面的开口有五个,五个开口均匀分布,所述下终板的通道在下表面的开口有五个,五个开口均匀分布。
5.根据权利要求4所述的颈椎跨界椎间高度维持器,其特征在于:所述上终板的通道在侧面的开口位于远离销轴的一端,所述下终板的通道在侧面的开口位于远离销轴的一端。
6.根据权利要求1-5任意一项所述的颈椎跨界椎间高度维持器,其特征在于:所述通道直径为2mm。
7.根据权利要求1所述的颈椎跨界椎间高度维持器,其特征在于:所述销轴为钛合金销轴。
8.根据权利要求1或7所述的颈椎跨界椎间高度维持器,其特征在于:所述上终板和下终板与销轴连接处至最近边缘设置限制上终板与下终板张开到最大11°夹角的斜面。
9.根据权利要求1所述的颈椎跨界椎间高度维持器,其特征在于:所述上终板的上表面和下终板的下表面均设有羟基磷灰石涂层。
