技术领域
本发明属于医药技术领域,具体涉及一种治疗足跟痛的药物及其制备方法。
背景技术
足跟痛又称跟痛症,是中老年人的一种常见病。轻者影响走路,重者足跟不能着地,影响着患者的生活质量,也会造成精神压力。起病较缓,病程较长,有时长达数月至数年。造成足跟痛的常见病因,多为随年龄的增长,人体组织发生退行性改变,长期劳损,使足跟部周围组织发生炎性改变和骨内压增高。多见于足跟脂肪纤维垫炎、跖腱膜炎、跟部滑囊炎、跟腱周围炎、跟骨高压症和跟骨骨刺等。这些足跟周围不同组织的相应疾病是形成足跟病的重要因素,多发于40~60岁的中老年人,且以女性为多。
西药治疗足跟痛目前主要是口服抗风湿镇痛类药物治疗足跟痛,这类药物往往因对胃刺激性较大造成不适而中断治疗,患者忍受疼痛,治疗效果不理想。
发明内容
为了解决上述的技术问题,本发明提供了一种治疗足跟痛的药物及其制备方法。该药物克服了现有技术中西药副作用较大,而且时有反复,效果不理想的缺陷。
本发明是通过下述的技术方案来实现的:
一种治疗足跟痛的药物,由如下重量份的中药原料药组成:
熟地25-35份、何首乌25-35份、附子2-4份、干姜3-9份、肉桂3-9份、大黄3-9份、鹿茸6-12份、刺老包6-12份、狗脊6-12份、追风伞11-19份、水冬瓜根11-19份、透骨草11-19份、黑骨藤3-9份、大血藤11-19份、路路通11-19份、当归11-19份、赤芍12份、桃仁6-12份、红花6-12份、蒲黄6-12份、五灵脂6-12份、血竭2-4份、元胡6-12份、甘草6-12份。
优选的,上述的治疗足跟痛的药物,由如下重量份的中药原料药组成:
熟地28-32份、何首乌28-32份、附子2.5-3.5份、干姜5-7份、肉桂5-7份、大黄5-7份、鹿茸7-11份、刺老包7-11份、狗脊7-11份、追风伞13-17份、水冬瓜根13-17份、透骨草13-17份、黑骨藤5-7份、大血藤13-17份、路路通13-17份、当归13-17份、赤芍10-14份、桃仁7-11份、红花7-11份、蒲黄7-11份、五灵脂7-11份、血竭2.5-3.5份、元胡7-11份、甘草7-11份。
更优选的,上述的治疗足跟痛的药物,由如下重量份的中药原料药组成:
熟地30份、何首乌30份、附子3份、干姜6份、肉桂6份、大黄6份、鹿茸9份、刺老包9份、狗脊9份、追风伞15份、水冬瓜根15份、透骨草15份、黑骨藤6份、大血藤15份、路路通15份、当归15份、赤芍12份、桃仁9份、红花9份、蒲黄9份、五灵脂9份、血竭3份、元胡9份、甘草9份。
上述的治疗足跟痛的药物,该中药的剂型形式为口服制剂。
上述的治疗足跟痛的药物的制备方法,步骤如下:
(a)将鹿茸、血竭、蒲黄超微粉碎成超微粉粒度范围5-45μm的药物微粉;
(b)将其他的药材加水煎煮2次,第一次加水量为药物重量的8-10倍,煎煮1.5-2.5小时,第二次加水量为药物重量的4-6倍,煎煮1-2小时,合并煎液,滤过,滤液浓缩成相对密度为1.19-1.22的稠膏,加乙醇使含醇量为75%,静置8小时,滤过,滤液60℃减压浓缩回收乙醇,然后将药物浓缩成稠膏,60℃减压干燥粉碎后过100目筛,制备成浸膏粉;
(c)将步骤(a)得到的药粉与步骤(b)制得的药物微粉混匀,加入药学上可接受的辅料制成制剂,即得。

刺老包又名楤木白皮、鹊不踏,仙人杖、刺春头、鹊不站、破凉伞、虎阳刺、刺包头、通刺、黄龙苞、鸟不食草、老虎刺草、鸟不宿、红桐刺、白刺椿、刺杖、白刺桑柴、老虎吊、猫儿刺、百鸟不栖、红虎刺、刺桐、红刺蒙桐、虎椒刺、白塔刺头、黄狼尾巴、飞天蜈蚣、天蜈蚣、雀不站,为五加科植物楤木树皮的韧皮部。治风湿痹痛,跌打损伤。《闽东本草》记载:"补腰肾,壮筋骨,舒筋活血,散瘀止痛。"
追风伞别名惊风伞、一把伞、公接骨丹、破凉伞、背花草、灯台单,为报春花科植物狭叶落地梅LysimachiaparidiformisFranch.var.StenophyllaFranch.的全草或根。辛;性温,具有祛风通络;活血止痛的功效。主风湿痹痛;半身不遂;小儿惊风;跌打;骨折
水冬瓜根别名接骨丹根,为山茱萸科植物角叶鞘柄木ToricelliaangulataOlinv.的根或根皮。味辛;微苦;性微温。具有祛风除湿;活血接骨的功效。主风湿性关节痛;跌打瘀肿;骨折;闭经。
黑骨藤又名铁散沙、铁骨头、牛尾蕨、山筋线(《广西药植名录》),山杨柳、青蛇胆、小青蛇、柳叶夹(《昆明民间常用草药》),黑龙骨、飞仙藤,达风藤(《云南中草药选》),小黑牛、青香藤、奶浆藤、青色丹。为萝藦种植物西南杠柳PeriplocaforrestiiSchltr.的根或全株。味甘;辛性温;小毒。祛风除湿;活血消痈。主治跌打损伤;风湿痹痛;闭经;乳痈;骨折。
其它原料药材均在中国药典2010版一部中有记载。
中医认为肝肾阴血不足,引起筋脉失养和筋挛,再加上外伤和感受寒湿,容易引发组跟痛。治当补肝肾、强筋骨、祛风除湿、温经散寒、活血镇痛。本发明的药物组方中,重用熟地、何首乌、狗脊、鹿茸补肝肾强筋骨;附子、干姜、肉桂温经散寒;刺老包、追风伞、水冬瓜根、黑骨藤祛风除湿,舒筋活血,透骨草、大血藤、路路通舒筋,当归、赤芍、桃仁、红花、蒲黄、五灵脂、血竭活血散瘀定痛,元胡理气止痛,大黄清清湿热凉血祛瘀解毒,甘草调和诸药。诸药合用,共奏补肝肾、强筋骨、祛风除湿、温经散寒、活血镇痛的功效,用于足跟痛,可以达到止痛治标,调养治本,标本兼治的目的。
本发明的优点在于:
(1)本发明提供了一种新的治疗足跟痛的中药,满足了临床需要。
(2)本发明将治疗足跟痛的中药制备成颗粒剂,既保持了汤剂吸收快、显效迅速的特点,又克服了汤剂服用前临时煎煮,久置易霉败变质的缺点,并可掩盖中药的苦味。本发明中药颗粒剂中药材全部经过精制提取,体积缩小,运输、携带、服用方便。
(3)临床试验结果表明,本发明治疗足跟痛的中药治疗组89例,痊愈28例(32.58%),显效36例(39.33%),有效21例(24.71%),无效4例,总有效率96.62%;对照组42例,痊愈9例(21.43%),显效15例(35.71%),有效11例(26.19%),无效7例,总有效率82.5%。治疗组疗效优于对照组(P<0.05)。

实施例1
处方:
熟地300g、何首乌300g、附子30g、干姜60g、肉桂60g、大黄60g、鹿茸90g、刺老包90g、狗脊90g、追风伞150g、水冬瓜根150g、透骨草150g、黑骨藤60g、大血藤150g、路路通150g、当归150g、赤芍120g、桃仁90g、红花90g、蒲黄90g、五灵脂90g、血竭30g、元胡90g、甘草90g。
制备方法:
(a)将鹿茸、血竭、蒲黄超微粉碎成超微粉粒度范围5-45μm的药物微粉;
(b)将其他的药材加水煎煮2次,第一次加水量为药物重量的9倍,煎煮2小时,第二次加水量为药物重量的5倍,煎煮1.5小时,合并煎液,滤过,滤液浓缩成相对密度为1.19-1.22的稠膏,加乙醇使含醇量为75%,静置8小时,滤过,滤液60℃减压浓缩回收乙醇,然后将药物浓缩成稠膏,60℃减压干燥粉碎后过100目筛,制备成浸膏粉;
(c)将步骤(a)得到的药粉与步骤(b)制得的药物微粉混匀,加入蔗糖,混匀,以75%乙醇作润湿剂制粒,干燥,整粒,制成颗粒剂1000g,即得。
共制备颗粒剂100袋,每袋重10g。
用法用量:每次1袋,每日3次。
实施例2
处方:
熟地280g、何首乌320g、附子2.50g、干姜70g、肉桂50g、大黄70g、鹿茸70g、刺老包110g、狗脊70g、追风伞170g、水冬瓜根130g、透骨草170g、黑骨藤50g、大血藤130g、路路通170g、当归130g、赤芍100g、桃仁70g、红花110g、蒲黄70g、五灵脂110g、血竭3.50g、元胡70g、甘草110g。
制备方法:
(a)将鹿茸、血竭、蒲黄超微粉碎成超微粉粒度范围5-45μm的药物微粉;
(b)将其他的药材加水煎煮2次,第一次加水量为药物重量的8倍,煎煮1.5小时,第二次加水量为药物重量的6倍,煎煮2小时,合并煎液,滤过,滤液浓缩成相对密度为1.19-1.22的稠膏,加乙醇使含醇量为75%,静置8小时,滤过,滤液60℃减压浓缩回收乙醇,然后将药物浓缩成稠膏,60℃减压干燥粉碎后过100目筛,制备成浸膏粉;

制备方法:
(a)将鹿茸、血竭、蒲黄超微粉碎成超微粉粒度范围5-45μm的药物微粉;
(b)将其他的药材加水煎煮2次,第一次加水量为药物重量的10倍,煎煮2.5小时,第二次加水量为药物重量的4倍,煎煮1小时,合并煎液,滤过,滤液浓缩成相对密度为1.19-1.22的稠膏,加乙醇使含醇量为75%,静置8小时,滤过,滤液60℃减压浓缩回收乙醇,然后将药物浓缩成稠膏,60℃减压干燥粉碎后过100目筛,制备成浸膏粉;

注意事项:孕妇禁用。
对本发明的实施例l的制剂进行了临床药效学试验,结果如下:
一般资料
131例足跟痛患者随机为两组。治疗组89,男性38例,女性51例;年龄29-54岁,平均56岁;病程10d-12年,平均5.4年;单足发病72例,双足发病17例,共106足;65例接受过封闭、理疗、中西药多方治疗,效果皆不理想;X线摄片示跟骨骨质增生或有骨刺者84例。对照组42例,男性19例,女性23例;年龄28-76岁,平均56.6岁;病程7d-15年,平均5.8年;单足发病34例,双足发病8例,共50足;28例接受过封闭、理疗、中西药多方治疗,效果皆不理想;X线摄片示跟骨骨质增生或有骨刺者31例。两组一般资料具有可比性(P>0.05)
治疗方法
治疗组给予本发明实施例1制备的治疗足跟痛中药组合物颗粒剂,每次1袋,每日3次。
对照组给予25%强的松龙注射液或复方丹参注射液1ml作患处封闭,每周1次,可连续治疗1-3次;口服消炎痛50mg,地巴唑片10mg,均每日5次,餐后服。若服上药后胃胀满者予吗丁啉片20mg同服。14d为1疗程。治疗期间注意休息,避免负重、过度劳累。
疗效标准
依照文献(张传礼,中医常见病证诊疗常规[M]郑州:河南医科大学出版社,1998,47)拟订。痊愈:症状消失,活动后无不适,1年内无复发。显效:症状基本消失,久站或劳累时有轻度不适,但无疼痛,休息后可缓解或消失。有效:症状基本消失,但久站或劳累时有轻度疼痛,休息后消失。无效:治疗前后无明显变化或症状加重者。
治疗结果结果见表1。

由表1可知,治疗组89例,痊愈28例(32.58%),显效36例(39.33%),有效21例(24.71%),无效4例,总有效率96.62%;对照组42例,痊愈9例(21.43%),显效15例(35.71%),有效11例(26.19%),无效7例,总有效率82.5%。治疗组疗效优于对照组(P<0.05)。
不良反应:治疗组未见不良反应。治疗组5例出现胃胀。两组随访3年,治疗组无复发,对照组复发9人。
Claims (3)
1.一种治疗足跟痛的药物,其特征在于,由如下重量份的中药原料药组成:
熟地30份、何首乌30份、附子3份、干姜6份、肉桂6份、大黄6份、鹿茸9份、刺老包9份、狗脊9份、追风伞15份、水冬瓜根15份、透骨草15份、黑骨藤6份、大血藤15份、路路通15份、当归15份、赤芍12份、桃仁9份、红花9份、蒲黄9份、五灵脂9份、血竭3份、元胡9份、甘草9份。
2.根据权利要求1所述的治疗足跟痛的药物,其特征在于,该中药的剂型形式为口服制剂。
3.权利要求1所述的治疗足跟痛的药物的制备方法,步骤如下:
(a)将鹿茸、血竭、蒲黄超微粉碎成超微粉粒度范围5-45μm的药物微粉;
(b)将其他的药材加水煎煮2次,第一次加水量为药物重量的8-10倍,煎煮1.5-2.5小时,第二次加水量为药物重量的4-6倍,煎煮1-2小时,合并煎液,滤过,滤液浓缩成相对密度为1.19-1.22的稠膏,加乙醇使含醇量为75%,静置8小时,滤过,滤液60℃减压浓缩回收乙醇,然后将药物浓缩成稠膏,60℃减压干燥粉碎后过100目筛,制备成浸膏粉;
(c)将步骤(a)得到的药物微粉与步骤(b)制得的浸膏粉混匀,加入药学上可接受的辅料制成制剂,即得。
