技术领域
本发明涉及外科器械,以及更特别的是,涉及外科钳和制造外科钳的方法。
背景技术
钳子是一种钳状器械,其依靠钳夹之间的机械作用来抓紧、夹紧和压缩血管或组织。电外科钳利用机械夹紧作用和电能以通过加热组织和血管而凝结和/或烧灼组织以实现止血。某些外科手术需要不只是仅仅烧灼组织而要依靠夹紧压力、精确电外科能量控制和间隙距离(即紧靠组织时相对的钳夹元件之间的距离)的独特结合来“密封”组织、血管和某些脉管束。通常,一旦血管被密封,外科医生必须准确地沿新形成的组织密封切断血管。因此,已经设计了许多血管密封器械,它们包括有在形成组织密封之后有效地切断组织的刀或刀片元件。
发明内容

另外的,或替代的,绝缘器126,和/或其密封盘接合凸块126c,可由可弹性压缩材料形成,以根据钳夹元件110、120移动至接近位置以抓紧其间的组织时钳夹元件110、120之间的闭合压力来便于实现钳夹元件110、120的相应密封盘112、122之间的所需间隙距离“g”,这将在下文更加详细地描述。通常,钳夹元件110、120之间的闭合压力处于约3kg/cm2至16kg/cm2的范围内。
继续参照图3,在使用中,在钳夹元件110、120处于间隔位置的情况下,端部操纵器组件100定位成使得待抓紧、密封和/或分割的组织布置在钳夹元件110、120的相应密封盘112、122之间。此后,例如通过从起始位置压下钳10的可动手柄40至相对于固定手柄50的压下位置(参见图1),使钳夹元件110、120向接近位置移动以抓紧密封盘112和122之间的组织。
在钳夹元件110、120朝向接近位置移动以抓紧其间组织时,钳夹元件120的密封盘接合凸块126c的变形后的自由端126d与钳夹元件110的对应构件接触(或简单的接触钳夹元件110的密封盘112)以设定钳夹元件110、120的相应组织密封盘112、122之间的间隙距离“g”。在密封盘接合凸块126c为可弹性压缩的实施例中,在钳夹元件110、120接近时,密封盘接合凸块126c被压缩在钳夹元件110、120的相应密封盘112、122之间。可以意识到的是,由钳夹元件110、120施加的闭合力确定了密封盘接合凸块126c的压缩量,并因此确定了密封盘112、122之间的间隙距离“g”。
在组织被钳夹元件110、120的相应密封盘112、122抓紧的情况下,电外科能量可以供给至组织密封盘112、122中的一个(或两个)并通过组织,以实现组织密封。控制密封盘112和122之间的间隙距离“g”帮助确保实现有效的组织密封。一旦组织已经被密封,刀片182(图6A-6C)可以前行穿过密封盘122和绝缘器126的相应刀槽125、126e(和/或钳夹元件110的刀槽115(图6A-6C)),以沿此前形成的组织密封分割组织。
现在参照图5A-5C,描述了根据本发明的另一个实施例的钳夹元件120的钳夹框架124上的绝缘器126和密封盘122的构造和装配。绝缘器126和密封盘122可以以任意合适的方式固定至彼此,例如热熔(如参照图4A-4C在上面所描述),并因此在这里将不再描述。
如最佳在图5A所示以及如上所述的,绝缘器126构造成定位在钳夹框架124的顶上并包括从其延伸的一对纵向间隔的钳夹框架接合凸块126f,然而可以设置多于或少于两个钳夹框架接合凸块126f。另一方面,钳夹框架124包括多个穿过其形成的接合孔124a,它们中的每个都构造成接收钳夹框架接合凸块126f中的一个于其中以使得每个钳夹框架接合凸块126f的自由端126g从其延伸出来,如最佳在图5B中示出的。接合凸块126f最初限定了基本上均匀的圆柱形构造以便于穿过钳夹框架124的基本上圆形的接合孔124a,然而也可以提供其他互补构造。此外,钳夹框架124可以包括在其下侧表面124c上绕每个接合孔124a的周边限定的凹槽或唇缘124b,以使得如图5B所示,在穿过接合孔124a布置时,钳夹框架接合凸块126f的自由端126g定位成与接合孔124a的唇缘124b相邻。
现在参照图5B和5C,并首先参照图5B,在装配期间,绝缘器126定位在钳夹框架124的顶上以使得绝缘器126的钳夹框架接合凸块126f穿过钳夹框架124的接合孔124a布置,使钳夹框架接合凸块126f的自由端126g从其延伸出来。为了相对于钳夹框架124固定绝缘器126,使用热熔元件(类似于热熔元件400(图4A))以使绝缘器126的钳夹框架接合凸块126f的自由端126g变形,从而防止钳夹框架接合凸块126f穿过接合孔124a移回,这类似于上文关于绝缘器126和密封盘122所讨论的。更加具体而言,热熔元件围绕每个钳夹框架接合凸块126f的自由端126g定位,并被加热至充足的温度以使绝缘器126的钳夹框架接合凸块126f变形,而基本不会影响钳夹框架124。随着钳夹框架接合凸块126f的自由端126g被加热以实现可变形性的状态,钳夹框架接合凸块126f的自由端126g与绕接合孔124a布置的钳夹框架124的唇缘124b一致,如图5C所示。之后允许钳夹框架接合凸块126f冷却以使得其自由端126g保持在钳夹框架124的唇缘124b内。热熔元件(未示出)可以限定工作表面,其形状与钳夹框架124的下侧表面124c互补,以使得在变形后的构造中,如图5C所示,钳夹框架接合凸块126f的自由端126g与钳夹框架124的下侧表面124c基本平齐,即使得钳夹框架接合凸块126f的自由端126g完全布置在钳夹框架124的唇缘124b内。可以意识到的是,在该变形后的构造中,防止钳夹框架接合凸块126f的自由端126g穿过钳夹框架124的接合孔124a返回,因此将绝缘器126固定在钳夹框架124的顶上。
现在参照图6A-6C,示出与端部操纵器组件100一起使用的刀组件180。在使用中,如图6A所示并如上所述,在钳夹元件110、120布置于间隔位置的情况下,操纵端部操纵器组件100进入位置以使待抓紧、密封和/或切割的组织布置在钳夹元件110、120之间。然后,相对于固定手柄50(图1)向近侧牵拉可动手柄40(图1)以使得钳夹元件110相对于钳夹元件120从间隔位置枢转至接近位置以抓紧其间的组织(参见图6B)。此后,电外科能量可以例如经由开关90(图1)的启动而供给至组织密封盘112和/或组织密封盘122,并传导穿过组织以实现组织密封,如上所述。如图6C所示,之后可例如通过触发组件80的触发器82(图1)的启动,使刀片182从收缩位置(图6B)前行至延伸位置(图6C),并穿过钳夹元件110、120的相应刀片槽115、125,以切割在钳夹元件110、120之间抓紧的此前密封的组织(或切割未处理的组织,依据特定目的而定)。
继续参照图6A-6C,刀组件180包括刀片182,在其远端183限定远端刀刃184。刀片182在其近端185联接至刀杆186。刀杆186可例如在触发器82(图1)启动时选择性地穿过杆12并相对于端部操纵器组件100平移,以将刀片182从收缩位置(图6B)平移至延伸位置(图6C)。
现在参照图7A-7C,将描述根据本发明实施例的刀组件180的构造和装配。如最佳示于图7A,刀杆186包括布置在其远端188的接合凸块187。接合凸块187构造为插入穿过形状互补的接合孔189,该形状互补的接合孔189在刀片182近端185处穿过刀片182形成。类似于前面描述的,一旦接合凸块187已经插入穿过接合孔189,接合凸块187的自由端就可以通过热熔变形而基本不会影响刀片182,例如使用热熔元件400来实现,以将刀杆186和刀片182固定至彼此。此外,尽管示出的刀片182定位在刀杆186的顶上并通过单独的热熔凸块/孔接合而固定于其,热熔可以替代地用于以任何其他合适的相对于彼此的位置固定刀片182和刀杆186和/或可以设置多个热熔凸块/孔接合。

Claims (8)
1.一种钳,所述钳包括端部操纵器组件(100),所述端部操纵器组件包括第一和第二钳夹元件(110、120),钳夹元件(110、120)中的至少一个能相对于另外一个在间隔开位置和用于抓紧其间组织的接近位置之间运动,该端部操纵器组件(100)的特征在于钳夹元件(110、120)中的至少一个包括:
绝缘器(126),所述绝缘器包括从其延伸的至少一个接合凸块(126c);以及
组织密封盘(122),所述组织密封盘限定了组织密封表面,所述组织密封盘(122)包括至少一个穿过其延伸的接合孔(122b),接合孔构造成在将组织密封盘(122)定位在绝缘器(126)的顶上时接纳该至少一个接合凸块(126c),以使得该至少一个接合凸块(126c)的自由端(126d)延伸穿过该至少一个接合孔(122b)并从组织密封表面延伸出来,该至少一个接合凸块(126c)能从其中该至少一个接合凸块(126c)的自由端(126d)能从该至少一个接合孔(122b)中自由移除的第一构造,变形为其中永久性地防止该至少一个接合凸块(126c)的自由端(126d)穿过该至少一个接合孔(122b)的第二构造,进而将绝缘器(126)和组织密封盘(122)彼此接合,其中在第二构造中,该至少一个接合凸块(126c)的自由端(126d)从组织密封表面突出预定距离,以在钳夹元件(110、120)布置在接近位置时设定钳夹元件(110、120)之间的间隙距离。
2.根据权利要求1所述的钳,其中该间隙距离在0.0254mm至0.1524mm的范围内。
3.根据权利要求1所述的钳,其中该至少一个接合凸块(126c)至少部分地由能弹性压缩的材料形成,以根据钳夹元件(110、120)之间的闭合压力设定钳夹元件(110、120)之间的间隙距离。
4.根据前述任一权利要求所述的钳,其中该至少一个接合凸块(126c)的自由端(126d)经由热熔从第一构造变形为第二构造。
5.根据权利要求1至3中任一项所述的钳,其中该至少一个接合凸块(126c)的自由端(126d)能从允许该至少一个接合凸块(126c)自由穿过该至少一个接合孔(122b)的第一直径“d”变形为防止该至少一个接合凸块(126c)穿过该至少一个接合孔(122b)的第二直径“D”。
6.一种制造钳的钳夹元件(110、120)的方法,包括如下步骤:
提供绝缘器(126),所述绝缘器包括从其延伸的至少一个接合凸块(126c),该至少一个接合凸块(126c)具有直径“d”;
提供组织密封盘(122),所述组织密封盘具有穿过其限定的至少一个接合孔(122b),该组织密封盘(122)限定组织密封表面;
相对于绝缘器(126)定位组织密封盘(122)以使得该至少一个接合凸块(126c)插入穿过该至少一个接合孔(122b),并使得该至少一个接合凸块(126c)的自由端(126d)延伸穿过该至少一个接合孔(122b)并从组织密封盘(122)的组织密封表面延伸出来;以及
使该至少一个接合凸块(126c)的自由端(126d)变形,以使得该至少一个接合凸块(126c)的自由端(126d)从具有小于该至少一个接合孔(122b)直径的直径“d”的第一构造变形为具有大于该至少一个接合孔(122b)直径的直径“D”的第二构造,进而防止该至少一个接合凸块(126c)穿过该至少一个接合孔(122b)并使绝缘器(126)和组织密封盘(122)固定至彼此,其中,在第二构造中,该至少一个接合凸块(126c)的自由端(126d)从组织密封表面延伸出来以限定布置在组织密封盘(122)顶上的停止元件。
7.根据权利要求6所述的方法,其中停止元件从组织密封表面延伸出的距离介于0.0254mm和0.1524mm之间。
8.根据权利要求6或7所述的方法,其中使该至少一个接合凸块(126c)的自由端(126d)变形的步骤通过热熔进行。
