Beschreibung description
The invention relates to a hip joint shell that can be cemented in, consisting of a hemispherical shell made of polyethylene and provided with recesses on its outer surface in order to achieve anchoring in a bone bed in the acetabulum prepared by spherical milling by means of hardening bone cement.
Hip joint cups that can be cemented are shown in the patent specifications FR 7 234 894 and EP 0 105 098. Hip cups that are not pre-anchored in the bone tissue, after the bone cement has been introduced into a prepared bone cavity, are inserted into this bone cavity in order to displace the bone cement and to force it into the spaces between the hip joint shell and the bone cavity. The pasteus mass of the bone cement, when displaced, opposes the penetrating hip joint shell, against which the intended end position of the shell must be reached, while the intended end position must be held until the bone cement has hardened.
The object of the invention is to make it easier for the surgeon to fix the hip joint shell in the intended end position. According to the invention, the object is achieved in that the cavity resulting from the milling is formed from a hemisphere portion and an adjoining cylinder portion, that a sealing gap of less than 1 mm per side is provided between the shell edge and the cavity, and that the gap between the outer surface of the hip joint shell (1) and the cylinder portion of the cavity away from the edge of the shell in the direction of the pole increases continuously.
The advantages of the invention can be seen in the fact that the hip joint shell is mechanically precentered, that the resistance to insertion increases until the intended end position is reached, and that the displaced bone cement increases evenly due to the increasing flow resistance towards the edge of the shell and is less dependent on gravity distributed and compressed between the hip joint shell and the cavity. The dependent claims 2 and 3 relate to advantageous developments of the invention.
Es zeigt: The invention is described below using an exemplary embodiment. It shows:
Fig. 1 schematisch einen Schnitt längs der Polachse durch eine implantierte Hüftgelenkschale; 1 schematically shows a section along the polar axis through an implanted hip joint shell;
Fig. 2 schematisch die Draufsicht auf eine Hüftgelenkschale gemäss Fig. 1 und Fig. 2 shows schematically the top view of a hip joint shell according to Fig. 1 and
Fig. 3 schematisch die Seitenansicht einer Hüftgelenkschale gemäss Fig. 1. 3 schematically shows the side view of a hip joint shell according to FIG. 1.
In the figures, a hip joint shell is shown which consists of a hemispherical shell made of polyethylene and which is provided with recesses on its outer surface in order to anchor it in a bone bed in the acetabulum prepared by spherical milling by means of hardening bone cement. The cavity resulting from the milling is formed from a hemisphere portion and a cylinder portion adjoining it. In the end position there is a sealing gap of less than 1 mm per side between the edge of the shell and the cavity. The gap between the hip joint shell and the cylinder portion of the cavity increases steadily away from the edge of the shell towards the pole.
1 shows a hip joint shell 1 made of polyethylene with a hemispherical inner surface 18 of inner radius 2 and with an outer surface 3 interrupted by transverse grooves 4 and longitudinal grooves 5, which serves for anchoring in the bone cement 7. The shell edge 6 protrudes a distance 12 beyond the equator of the inner surface 18. The cavity 9 in the bone tissue 8 consists of a hemisphere portion 10 of radius 21 and a cylinder portion 11 adjoining it at the equator 20 with a height in the direction of the polar axis 17 which corresponds at least to the distance 12. In the intended end position, the hemisphere portion 10 and the hemispherical inner surface 18 have a common center point. In the intended end position, the sealing gap 19 between the cylinder portion 11 and the shell edge 6 is less than 1 mm per side, while the gap 22 in the area of the cylinder portion 11 to the outer surface 3 increases with the distance from the shell edge 6 in the direction of pole 16 to a depth the distance 12 is continuously increased, in that the outer surface 3 follows a cone with half the cone angle 15 in this area. The outer surface 3 is interrupted by a circumferential notch 13, in which a metal wire 14 is caulked for later position checks by means of X-rays.
Before the hip joint shell 1 is inserted, the cavity 9, which essentially opens upwards, is filled with a quantity of still compressible bone cement 7, which in addition to the volume that is formed in the end position between the hip joint shell 1 and the cavity 9, also the volume of the Contains cylinder portion 11. When the hip joint shell 1 is retracted, the bone cement 7 is displaced and moves outwards between the hip joint shell 1 and the cavity 9. As soon as the area of the hip joint shell conical from the shell edge 6 to a depth of the distance 12 is immersed in the cylinder portion 11 of the cavity 9, the hip joint shell 1 has a constantly narrowing throttle point between the edge of the cylinder portion 11 and the conical region of the hip joint shell. The pressure required for pressing out in front of the throttle point increases and forces the bone cement into all the recesses 4, 5 and into the microstructure of the surfaces of the polyethylene and into the naturally existing cancellous bone openings, where it compresses it. Due to the strong increase in the throttling effect, the immersion speed is reduced for a given press-in force and enables precise starting and stopping in the end position. In addition, corrections to the angular position of the polar axis 17 by one-sided
