Because of the loss of mobility, scarring of the median nerve in the carpal tunnel can lead to chronic pain syndrome of the wrist joint, with reduced sensation, muscular dystrophy and severe limitation of the use of the hand. This syndrome most often appears following open carpal tunnel release. Nine patients with scarring of the median nerve in the carpal tunnel were treated with a hypothenar fat-pad flap. Eight of them showed a significant reduction in pain, with improved sensation, trophism and strength. The procedure is suitable as a salvage procedure for restoring a sliding pathway and for cushioning the median nerve in the presence of recurrent lesions in the carpal tunnel.Entraînant une perte de mobilité, la fibrose cicatricielle du nerf médian au canal carpien peut conduire à un syndrome douloureux chronique qui comporte une réduction de la sensibilité, des troubles musculaires et une limitation fonctionnelle de la main. Ce type de syndrome apparaît le plus souvent après chirurgie du canal carpien à ciel ouvert. Neuf patients présentant des adhérences cicatricielles du nerf médian au poignet ont été traités par un lambeau graisseux hypothénarien. Huit d'entre eux ont manifesté une réduction significative de la douleur avec amélioration portant sur la sensibilité, le tropisme et la force. Cette intervention constitue un procédé de sauvetage pour restaurer un tissu de glissement et matelasser le nerf médian dans les cas de troubles récidivants au canal carpien.La fibrosis cicatrizal del nervio mediano en el túnel carpiano origina una pérdida del movimiento que puede conducir a un síndrome doloroso crónico que incluye una reducción de la sensibilidad, problemas musculares y una limitación funcional de la mano. Este tipo de síndrome aparece más frecuentemente después de la cirugía del túnel carpiano a cielo abierto. Nueve pacientes quienes presentaban adherencias cicatrizales del nervio mediano en la muñeca se trataron mediante un colgajo graso hipotenar. Ocho de entre ellos manifestaron una reducción significativa del dolor con mejoría sobre la sensibilidad, el trofismo y la fuerza. Esta intervención constituye un procedimiento de salvamento para restaurar los tejidos de deslizamiento y cubrir el nervio mediano en los casos de problemas recidivantes en el túnel carpiano.

Carpal tunnel release (CTR) is regarded as a common and successful operative procedure in hand surgery. However, an increasing number of patients with complications have been referred to our hospital. This retrospective investigation was undertaken to clarify the reasons for persisting or recurrent symptoms in 200 patients who underwent secondary exploration during a 26 month period at a single institution. In 108 cases, the flexor retinaculum was found to have been released incompletely. In 12 patients, a nerve laceration had occurred during the primary intervention. In 46 patients, symptoms were due to the nerve being tethered in scar tissue. The re-exploration revealed circumferential fibrosis around and within the median nerve in 17 patients and a tumour in the carpal tunnel in four patients. In 13 patients, no specific reason was found for recurrence of symptoms. We conclude that CTR seems to be a widely underestimated procedure and revision surgery could be largely avoided by reducing technical errors during the primary operation.

Goal of Surgery Elimination or reduction of pain of the wrist while maintaining the residual motion. Indications Nonunion of scaphoid. Scapholunate dissociation with advanced collapse (SLAC). Osteoarthritis of radioscaphoid and midcarpal joints accompanied by pain and swelling of wrist. Contraindications Concomitant osteoarthritis of radioulnar joint, i. e. in instances of chronic rheumatoid arthritis or chondromalacia of lunate. Positioning and Anaesthesia Supine, tourniquet. General anaesthesia (bone graft harvesting from iliac crest). Surgical Technique Dorsal approach. Mobilisation of retinaculum and opening of 2nd and 4th tendon compartment. Resection of dorsal interosseous nerve. Removal of scaphoid and resection of articular cartilage between capitate, lunate, hamate and triquetrum. Correction of intercarpal malposition (DISI) and interposition of cancellous bone grafts. Internal fixation with Kirschner wires. Postoperative Management Elevation of arm. After removal of drain and regression of swelling: forearm cast. Radiographic control after 6 weeks, and active and passive exercises as soon as fusion is solid. Removal of wires 2 to 3 months postoperatively. Possible Complications Injury of nerves or vessels. Irritation of tendons due to Kirschner wires exceeding bony surface. Nonunion, infection, SRD, osteoarthritis of radioulnar joint. Results 45 patients, 41 men and 4 women, mean age 42 years (21 to 70 years). Mean follow-up time 41 months (8 to 54 months). Loss of combined flexion—extension 21% in comparison to preoperative state. Loss of ad- and abduction of 12% but increase of strength by 38% (Table 2). Decrease of pain from 75 points preoperatively to 19 points postoperatively (Table 3). 36 patients were satisfied, 34 returned to previous occupation which consisted of heavy manual labor in 28. No improvement in 7 patients. Wrist fusion became necessary twice, a revision on account of nonunion in 4 patients and an ulnar styloidectomy for impingement in 3 patients. Up to now no rapidly progressing radiolunar osteoarthritis or ulnar drift of carpus.

Indikationen Schmerzen und Funktionsstörungen bei fehlverheilten distalen Radiusfrakturen vom Typ A 2.2 und A 3.2 nach der AO-Klassifikation. Kontraindikationen Fehlverheilte Radiusfrakturen im Kindes- und Jugendalter. Korrekturbedürftige intraartikuläre Fehlstellung. Deutliche Radiokarpalarthrose. Operationstechnik Korrekturosteotomie des Radius unter Interposition eines autogenen Knochenblockes und Osteosynthese mit einer Spezialplatte, wobei die Radiusbasis über einen Y-förmigen, radiopalmaren Zugang freigelegt wird. Ergebnisse Bis August 1996 wurden insgesamt 166 Radiuskorrekturosteotomien durchgeführt, 124 davon bei fehlverheilten Extensionsfrakturen. Von diesen wurden 115 nach der dargestellten Technik versorgt; 91 Patienten hiervon konnten nach durchschnittlich 4,5 Jahren nachuntersucht werden. Durch den Eingriff konnte die Beweglichkeit des Handgelenkes in allen Ebenen verbessert werden; die Kraft nahm gegenüber präoperativ zu, erreichte jedoch nicht den Wert der unverletzten Seite. Mit einer durchschnittlichen Palmarneigung von 7° konnte eine fast physiologische Gelenkstellung erzielt werden. Der Ulnavorschub wurde vollständig ausgeglichen. Häufigste Komplikation war eine leichte Irritation des Ramus superficialis des Nervus radialis.

Fourteen patients with an amputation injury of four fingers are reported. Fifty-one of these 56 fingers were replanted with a survival rate of 88 %. To achieve optimal results replantation of all fingers should be attempted. Dividing the operative procedure into a macrosurgical and a microsurgical part helps to economize both tourniquet time and total operative time. In case of arterial spasm or other difficulties to reestablish blood flow, a check list can be of great value. To achieve good results in such severe and difficult procedures a special organisation of replantation centers is needed.Quatorze cas d'amputation de quatre doigts sont rapportés. Cinquante-et-un des 56 doigts ont été réimplantés avec un taux de survie de 88 %. Pour obtenir les meilleurs résultats possible, il faut tenter de réimplanter tous les doigts. La séparation de l'intervention en un temps macrochirurgical et un temps microchirurgical permet de réduire le temps de pose du garrot et la durée totale de l'opération. En cas de spasme artériel ou d'un autre obstacle au rétablissement de la circulation sanguine, une liste de points à vérifier peut être très utile. Pour obtenir de bons résultats dans ces interventions graves et difficiles, les centres de réimplantation doivent adopter une organisation appropriée.Se describen 41 pacientes quienes presentaron una amputación de los cuatro dedos. Cincuenta y uno de estos 56 dedos fueron reimplantados con un porcentaje de exito de 88 %. Para obtener un resultado óptimo, se debe intentar la reimplantación de todos los dedos. La división del procedimiento quirurgico : en un tiempo microquirúrgico y un tiempo macroquirúrgico ayuda a economizar tiempo de torniquete y tiempo de cirugía total. En presencia de un espasmo arterial o de otras dificultades para restablecer el flujo sanguíneo, una serie de medidas a seguir paso a paso puede resultar de gran valor. Para obtener unos buenos resultados en estos casos severos, que requieren procedimientos de tratamiento difíciles, se requirere una organización especial de centros de reimplantación.

Operationsziel Anatomische Rekonstruktion der Gelenkfläche eines Fingerendgliedes und Wiederherstellung der Streckensehnenfunktion. Indikationen Frakturen der dorsalen Endgliedbasis mit knöchernem Fragment, größer als 30% der Gelenkfläche. Kontraindikationen Entzündliche Veränderungen im Operationsgebiet. Operationstechnik Offene Reposition von dorsal und Retention mit Zuggurtungsosteosynthese. Ergebnisse Von 1992 bis 1997 wurden 40 Patienten nach der Methode operiert. Achtunddreißig Patienten konnten nach durchschnittlich 25 Monaten nachkontrolliert werden. Darunter waren sechs Patienten nur einer telefonischen Befragung zugänglich. Von den verbliebenen 32 Patienten wiesen 19 eine uneingeschränkte Streckung auf; bei den übrigen 13 war ein Streckdefizit von 10° auszumachen. Achtmal fand sich kein Beugedefizit; bei 18 Patienten bestand ein Beugedefizit von 10°, bei fünf von 20° und bei einem Patienten von 30°. Subjektives Ergebnis bei 84% der 38 Patienten sehr gut und gut. Komplikationen: einmal Nagelwachstumsstörung, einmal Wundinfekt. Objectives Anatomic reduction of the articular surfaces of the distal phalanx and restoration of extensor tendon function. Indications Avulsion fractures of the distal phalanx with a fragment exceeding 30% of the articular surface. Contraindications Inflammatory changes at the surgical site. Surgical Technique Open reduction through a dorsal approach and fixation with tension band wiring. Results From 1992 to 1997, this technique was employed in 40 patients, 38 could be followed up for an average of 25 months. Of these 38 patients, 6 could only be reached by telephone. Nineteen of the 32 patients had full extension, the remaining 13 had an extension lag of 10°. No limitation of flexion was seen 8 times, 18 patients had a lag of 10°. 5 of 20°, and 1 of 30°. Eighty-four percent of the 38 patients felt that the result was excellent or good. The following complications were noted: once an abnormal growth of the nail, and once an infection.

Operationsziel Schmerzreduktion und Funktionserhalt des Fingermittelgelenks durch prothetischen Ersatz. Indikationen Symptomatische Arthrose des Fingermittelgelenks mit erhaltenen Kollateralbändern, ausreichender knöcherner Abstützung und intaktem oder zumindest rekonstruierbarem Strecksehnenapparat. Kontraindikationen Mangelhafte Stabilität, z. B. infolge rheumatoider Arthritis oder unfallbedingter Zerstörung des Bandapparats. Nicht rekonstruierbarer Strecksehnenapparat. Florider oder chronischer Infekt. Mangelhafte Mitarbeit des Patienten. Operationstechnik Dorsaler Zugang zum Fingermittelgelenk. Es wird ein dreieckförmiger, distal am Ansatz des Tractus intermedius gestielter Sehnenlappen gebildet. Die Seitenzügel bleiben erhalten. Resektion der Gelenkflächen unter Erhalt der palmaren Platte und der Kollateralbänder. Einpassen der Probeprothese, Lagekontrolle und Einbringen der endgültigen ungekoppelten Prothesenkomponenten in Press-fit-Technik. Naht der Streckaponeurose. Ergebnisse 20 Patienten wurden wegen einer posttraumatischen oder idiopathischen Arthrose mit 24 Pyrocarbon-PIP-Prothesen versorgt und im Median nach 15 Monaten (6–30 Monate) nachuntersucht. Drei Prothesen wurden in Arthrodesen konvertiert. Bei den verbliebenen Prothesen konnte ein Bewegungsumfang der Fingermittelgelenke von durchschnittlich 50° erreicht werden. Auf der visuellen Analogskala (VAS; 0: kein Schmerz, 10: stärkster Schmerz) waren die Patienten beschwerdearm (VAS: 0–3). 80% der Patienten waren subjektiv mit dem Erfolg der Operation zufrieden. In drei Fällen (eine Infektion, zwei Luxationen) mussten die Prothesen entfernt und eine Arthrodese durchgeführt werden. Eine Migration konnte in fünf Fällen der distalen und in vier Fällen der proximalen Komponente im Röntgenbild beobachtet werden, ohne indass sich dies auf die funktionellen Parameter auswirkte. Eine schmerzlose Geräuschentwicklung („Quietschen‘ war bei neun von 21 Prothesen zu verzeichnen. Ebenso wie bei der Migration der Prothesen ergaben sich hieraus jedoch keine funktionellen Nachteile. Objective Prosthetic joint replacement to reduce pain and maintain function of the proximal interphalangeal joint. Indications Symptomatic arthritis of the proximal interphalangeal joint with preservation of the collateral ligaments, sufficient bone support, and intact or at least reconstructable extensor tendons. Contraindications Lack of stability, e. g., as a result of rheumatoid arthritis or destruction of the ligaments caused by an accident. Nonreconstructable extensor tendons. Florid or chronic infection. Lack of patient compliance. Surgical Technique Dorsal approach to the proximal interphalangeal joint. A triangular tendinous flap with pedicle, based distally on the insertion of the medial band, is lifted up, leaving the lateral bands intact. The joint surfaces are resected while maintaining the palmar plate and the collateral ligaments. The trial prosthesis is fitted, its position is checked, and the final unconstrained prosthetic components are inserted using a press-fit technique. The dorsal aponeurosis is reapproximated. Results 20 patients were treated for posttraumatic or idiopathic arthritis with 24 pyrolytic carbon PIP prostheses, and a follow-up examination was carried out after an average of 15 months (6–30 months). Surgical management was changed from arthroplasty to arthrodesis in three cases. For the remaining prostheses, an average range of motion of 50° was achieved for the proximal interphalangeal joint. On the visual analog scale (VAS; 0: no pain, 10: incapacitating pain), the patients suffered few symptoms (VAS: 0–3). 80% of patients said they were satisfied with the outcome of the operation. In three cases (one infection, two dislocations) the prostheses had to be removed and arthrodesis performed. Migration of the distal components was observed on the radiographs in five cases, and of the proximal components in four cases, although this did not have any effect on the functional parameters. The development of a painless noise (“squeaking” was noticed in nine out of 21 prostheses. However, as with prosthetic migration, this did not cause any functional deficits.

If one muscle group is lost and the other is also damaged, today reconstruction can be obtained by free muscle transplantation with microvascular and microneural connections. The authors report on six cases of transplantation of the gracilis muscle to the forearm to replace finger flexors or extensors. All muscles except one (in which the 6-month observation time is still too short) recovered useful function. Trotz entscheidender Erkenntnisse über Biomechanik und Funktion des Handgelenks der letzten Jahre sieht sich der traumatologisch tätige Arzt nach wie vor mit einer Vielzahl posttraumatischer Spätschäden an diesem Gelenk konfrontiert. Neben der früher führenden Rettungsoperation, der Totalarthrodese des Handgelenks, stehen heute differenziertere Behandlungsverfahren zur Verfügung. Teilarthrodesen basieren auf dem Prinzip, den Karpus zu stabilisieren, zerstörte Gelenkflächen auszuschalten und in den noch erhaltenen Gelenkabschnitten eine Restbeweglichkeit aufrecht zu erhalten. Sie können unter Beachtung der Pathobiomechanik am Handgelenk für eine zunehmende Zahl von Indikationen mit Erfolg eingesetzt werden. Für den Patienten resultiert ein besseres funktionelles Ergebnis als bei Panarthrodesen und eine höhere Stabilität und längere Beschwerdefreiheit als bei Endoprothesen oder Arthroplastiken. Dennoch sind Untersuchungen zur Verbesserung der Ergebnisse und Wertigkeit einzelner Teilfusionen im Vergleich zu anderen Verfahren zu fordern und zu erwarten. Despite the important observations that have been made concerning biomechanics and function of the wrist joint in recent years, the surgeon is still confronted with a huge number of delayed posttraumatic disorders of this joint. In the past, total wrist fusion was the leading salvage procedure, whereas today several salvage procedures are available to solve these problems. Partial wrist fusions are supposed to stabilize the carpus, immobilize destroyed articulating surfaces while allowing motion in the unharmed parts of the wrist. Acknowledging pathobiomechanical laws, they have been used for a growing number of indications with a large benefit. The patients profit of better functional results than can be observed in patients with total arthrodesis of the wrist. Furthermore, partial wrist fusion provides a more stable situation and the absence of pain for a longer period of time. However, further studies evaluating the clinical outcome of these salvage procedures are necessary and to be expected in the near future.
