Depuis le premier cas rapporté, peu d'études ont fait le point sur ce sujet. Méthode : Une étude rétrospective, a intéressé 17 cas décédés par intoxication à la paraphénylène-diamine, au service de réanimation des urgences chirurgicales de l'hôpital Ibn Sina de Rabat sur une période de trois ans (1999-2001). Résultats : Dans la majorité des cas, ce sont des femmes célibataires, de bas niveau socio-économique, ayant ingéré le produit dans un but d'autolyse (suicide). La symptomatologie clinique a été stéréotypée, dominée dans un premier temps par un oedème cervical responsable d'une détresse respiratoire aigue. Il n'existe à l'heure actuelle aucune certitude quant aux théories visant à expliquer cette pathologie notamment l'origine auto-immune; aussi doit-on la considérer comme d'étiologie inconnue. Adoptant le plus souvent une sémiologie radio-clinique très évocatrice d'un cancer, cette pathologie illustre la nécessité d'établir la preuve histologique de bénignité. Le diagnostic de la mastite granulomateuse idiopathique repose sur une approche multidisciplinaire. C'est un diagnostic d'exclusion qui, lorsqu'il est posé, doit amener à proposer un traitement adéquat et un suivi au long cours. En rapportant un nouveau cas de mastite granulomateuse idiopathique, nous essayons à travers une revue de la littérature, de présenter les aspects épidémiologiques et cliniques, discuter les moyens diagnostiques ainsi que les difficultés thérapeutiques.

In most cases they present as a slowly growing painful mass situated in those regions where this type of fat persists in the adult. We report a case of hibernoma of the forearm, which is an unusual localization, and we review the diagnostic and therapeutic aspects of this of tumour whose main differential diagnosis is liposarcoma.

The development of cancer on the ileal graft after augmentation ileocystoplasty benign bladder di... more The development of cancer on the ileal graft after augmentation ileocystoplasty benign bladder disease is a little known complication. The authors report a case of squamous cell carcinoma in the ileal bladder occurring 31 years after augmentation ileocystoplasty for tuberculous bladder, in a 60-year-old patient. The ileal bladder was resected and a new augmentation ileocystoplasty was performed. The postoperative course was uneventful. Histological examination of the operative specimen showed infiltration of all of the intestinal wall. The patient died one year after, in a context of peritoneal carcinomatosis.

They have a poor prognosis. The authors report a case of primary leiomyosarcoma of the kidney in a 44-year old patient, presenting in the form of low back pain and haematuria. Treatment consisted of radical nephrectomy. Two months later, the patient presented with hepatic metastases. The patient is currently receiving chemotherapy.

Cette tumeur est extrêmement rare chez l’adulte et son pronostic est sombre. Seuls quelques cas ont été rapportés chez l’adulte. Ainsi, nous présentons dans ce travail le cas d’un hépatoblastome diagnostiqué chez une jeune femme de 22 ans. Observation Patiente de 22 ans sans antécédents, accusant depuis quelques mois des douleurs de l’hypochondre droit. L’examen clinique ainsi que le bilan hépatique étaient sans particularité. Ni la biologie ni les examens morphologiques n’étaient en faveur d’un foie de cirrhose. L’échographie, le scanner et l’IRMhépatiquesmettaient en évidence une énorme masse hépatique de 11 cm de diamètre. Les résultats histologiques des biopsies écho-guidées sur foie tumoral et foie restant étaient en faveur d’un hépatoblastome de type foetal associé à une fibrose extensive. L’exploration chirurgicale détectait une large masse occupant tout le foie droit avec un foie restant cirrhotique. Cette patiente bénéficiait d’une résection hépatique qui consistait en une trisegmentectomie VI, VII, VIII avec des marges de résection saines. Les suites opératoires étaient simples avec un recul sans récidive de 22 mois. Conclusion L’hépatoblastome survient de façon extrêmement rare chez l’adulte chez qui la présentation est assez agressive. Le pronostic chez l’adulte est plus sombre par rapport à l’enfant mais meilleur que celui du carcinome hépatocellulaire. Le traitement actuel de cette tumeur inclus la chimiothérapie et la chirurgie mais cette dernière demeure l’essentiel du traitement chez l’adulte.

The majority of cases are found with distant metastases at the time of diagnosis. We present a case of a 53-year-old male, who was diagnosed with this rare malignancy. Despite aggressive treatment consisting of radio-chemotherapy, the disease progressed rapidly with metastases occurring 12 months following the initial diagnosis. In summary, final diagnosis requires careful histological and immunohistochemical examination to determine the therapeutic strategy to be followed. Chemotherapy remains the mainstay of treatment for small-cell carcinoma of the anus. Radiotherapy exerts additional activity and remains a prime choice to gain local control. In this report we aim to describe the clinical, pathological features of this rare entity, and to discuss therapeutic options. Résumé Le carcinome à petites cellules du canal anal est une tumeur extrêmement rare et agressive à la fois, souvent diagnostiquée au stade métastatique. On rapporte l’observation d’un patient âgé de 53 ans, chez qui le diagnostic de carcinome à petites cellules du canal anal a été confirmé. Malgré une thérapeutique agressive, faite de radiochimiothérapie, l’évolution de la maladie a été fatale, avec métastases et décès du patient 12 mois plus tard. En conclusion, une étude histopathologique minutieuse associée au complément immunohistochimique sont indispensables pour le diagnostic final. En effet, la chimiothérapie constitue le principal traitement. En revanche, la radiothérapie est le traitement de choix pour le contrôle local de cette tumeur.

Extragastrointestinal stromal tumors (EGISTs), neoplasms with immunohistological features overlapping those of GISTs, are found in the abdomen outside of the gastrointestinal tract with no connection to the gastric or intestinal wall. The present report presents the clinical, macroscopic and immunohistological features of an EGIST arising in the greater omentum of a 63-year-old woman, and discusses the clinical behavior and prognostic factors of such lesions in comparison to their gastrointestinal counterparts.

Women are slightly more affected than men. We report the case a 27 year old woman presenting with a twelve months history of painful and progressively growing thoracic mass in the right anterior chest wall. Physical examination found out a fixed thoracic mass in the right retro-mammary area measuring 8 x 6 centimeters. The overlying skin was normal. Chest roentgenogram demonstrated a large ill defined mass continuing the anterior arc of the fourth right rib. Computed tomography evidenced a well defined pathologic process originating from the fourth right rib without expansion of the surrounding soft tissue. Pulmonary functional tests were normal and other complementary investigations evidenced no abnormalities. Our patient first had a fine needle cytological biopsy that brought strong suspicion of Giant-cell tumor of the rib. She then underwent an "en bloc" resection of the tumor who...
Thyroid sarcomatoid carcinoma is a rare and aggressive neoplasm composed of a follicular carcinom... more Thyroid sarcomatoid carcinoma is a rare and aggressive neoplasm composed of a follicular carcinoma which is contiguous or admixed with a pleomorphic spindle cell component. We report the case of a thyroid tumor reputed to have a poor outcome, in a 62-year-old woman. The radical thyroidectomy specimen was totally invaded. Results of immunoperoxidase staining for thyroglobulin and epithelial markers were positive in the areas of follicular carcinoma and negative in the sarcomatous component. The patient died a few days later due to septic shock. The epithelial and mesenchymal components of thyroid carcinosarcoma were both part of the neoplastic parenchyma and evolved from a single common stem cell, in agreement with the hypothesis that the tumors are of monoclonal origin. The definition of this tumor as its histogenesis and prognostic are discussed.

Primary sarcomas of the great vessels, that is, the aorta, pulmonary artery, and inferior vena ca... more Primary sarcomas of the great vessels, that is, the aorta, pulmonary artery, and inferior vena cava, are rare. They can be classified according to the location of the sarcoma in the vessel wall and by their gross appearance. Most often they are leiomyosarcomas or fibrosarcomas. We report here a case of an intimal sarcoma of the inferior vena cava. Histological and immunohistochemical findings confirmed the diagnosis for this 17-year-old girl and distinguished it from leiomyosarcoma and angiosarcoma, both of which have better prognoses. Intimal sarcoma of the inferior vena cava is rare and difficult to diagnose before surgery or biopsy. Histologically, it is a poorly differentiated tumor with the worst prognosis among the primary vascular sarcomas. Pathologic findings and immunohistochemical staining are useful for a positive diagnosis.
