Checklist:

The previous attempt at fixing the bug only partially fixed it - instead of a potentially unlimited number of duplicate moderation records being created, a maximum of 2 were created (1 by api.freecodecamp.org and 1 by exam-services).

Now, with the attempt moderationId existing, exam-services will not create a moderation record.
