This software (originaly written in 2004) has been verified to still perform as intended, 
however, it isn't likely the fastest method for accomplishing its task.
People have already forked this code years ago, but this is the last release
I had ever made public.

Below is the original README file from 2004 with some slight modifications re: Requirements.

I) Requirements
---------------

Tested on: 
MacOSX 10.3 (panther) 
MySQL MySQL 4.0.20-standard (mac binary from mysql.com)
PHP 4.3.2 (mac default binary and mac default apache module)

Ubuntu 12.04 LTS (2013 github prep)
MySQL 5.5 Standard Package
PHP 5.3 Standard Package

II) Installation
----------------
quick and dirty...

Shell
-----
1. "mysqladmin -u root -p create phpMyAudit" 
2. "mysql -u root -p phpMyAudit < database.sql"  
3. edit includes/config.inc.php
3. verify the php location (default is /usr/bin/php, this may vary).  Change the #! where necessary (for all script files ending in .php, minus the index.php).
4. "chmod +x *.php"  
5. "./pmaImport.php dictionary_filename"

Web (add-on)
---
1. setup the Shell
2. "cp -r web/ apache_html_documents/phpMyAudit"
3. edit includes/config.inc.php in the new phpMyAudit web directory.

III) Usage 

Shell
-----
1. "./phpMyAudit.php hash"
2. "./pmaFile.php hashfile" (for a list of hashes, format is "user:hash")

Web
---
1. enter a single hash, hit "Find"
2. enter a list of hashes, hit "Find".  format is "user:hash".
