Abstract Three-dimensional (3D) printing represents a relatively novel technology of this century with a wide field of applications in industry, and especially the manufacturing sector. The increasing role of technology in medicine and surgery has led to an ongoing effort to identify the possible applications of 3D printing in these fields, as well as the challenges involved. This book, with an intended audience that goes well beyond the field of medicine, aims to provide an overview of the relevant questions and answers. Ultimately, and through the collaboration of cross-disciplinary experts, we hope to provide an understanding of the actual potential of 3D printing in surgery and how this will impact our patients.

Major liver resection may compromise gut-barrier function, increasing the risk of postoperative infectious complications. The aim of the present experimental study was to compare the effect of the laparoscopic versus the open technique for major liver resection on integrity as well as inflammatory and immune responses of the gut barrier. Wistar rats were subjected to open 70% hepatectomy (group H), laparoscopic 70% hepatectomy (group LH), sham operation (group S) or no intervention (group C). At various timepoints (1 hour-1 week) after operation, ileal tissue was excised for oxidative state assessment (TBARS levels), histopathologic examination, histomorphometric analysis, immunohistochemical assessment of the mitotic and apoptotic activity, and tissue expression of inflammatory (interleukin-6, tumor necrosis factor-α, nuclear factor-κB and vascular cell adhesion molecule-1) and immune response biomarkers (CD4+ and CD8+ T-lymphocytes) of the intestinal mucosa. No changes were noted in oxidative state. The histopathologic profile was less deteriorated in group LH compared to group H. Intestinal mucosa atrophy was less intense in group LH compared to group H and was related to an equally compromised crypt cell mitotic activity. Tissue overexpression of interleukin-6, tumor necrosis factor-α, nuclear factor-κΒ, vascular cell adhesion molecule-1, CD4+, and CD8+ T-lymphocytes was less pronounced in group LH compared to group H. The employment of the laparoscopic technique for major liver resection in the rat attenuated disruption of the gut barrier compared to the open procedure. This was related to less pronounced inflammatory and immune responses of the intestinal mucosa.

Introduction: Talus dislocation without a concomitant fracture of the malleoli is a rare injury. Case Report: We present a case of a closed anteromedial talus dislocation in a 25-year-old male motorcyclist due to a high-energy trauma, resulting from a fall following a road traffic accident. The talus could not be reduced by closed means making open reduction performed through a dorsal approach necessary. At 6-month follow-up, the patient has little pain and the range of ankle motion was satisfactory with some signs of bone contusion. Conclusions: Talus dislocation is a rare injury that occurs after high-energy trauma. Open reduction can be beneficial, particularly if the initial attempt at closed reduction fails.

Abstract The rapid prototyping has dramatically expanded over the last 20 yrs, with the three-dimensional (3D) printing to be in the forefront and one of the most promising technologies among others. 3D printing is an additive manufacturing (AM) process based on sequential addition of material layers, offering the opportunity to print either two-dimensional parts (thin or thick films as self-standing films or coatings) or bulk 3D parts and components made of different materials with variable mechanical and physical properties. The global sales in 3D printing (products and services) rose by 21% from 2017, reaching 7 B$ in 2018 ( https://www.forbes.com ). There are various techniques for printing 3D solid materials, including electron-beam freeform fabrication, direct metal laser sintering, and fused deposition modeling (FDM), among others. 3D FDM holds strong potential for the formation of a new class of multifunctional thermoplastic polymer nanocomposites. With the ability to print complex 3D objects layer by layer even by low-cost 3D printers appear to the market nowadays, AM with nanomaterials could be leveraged in new ways toward greater control over material properties across part dimensions. Multifunctionality through embedding of nanomaterials and nanotechnology-enabled final objects can further extend capabilities of nanocomposites to properties such as by-design and patient-specific biomedical equipment, gradients in thermal and electrical conductivity, photonic emissions tunable for wavelength, increased strength, and reduced weight. 3D FDM printing method could allow direct printing of nanomaterial-modified thermoplastic filaments, to bring a new paradigm for nanocomposite functionality in bulk 3D objects.

Gastrointestinal stromal tumors (GISTs) are considered as rare gastrointestinal tumors, and their location in the anal track is exceptionally unusual. We describe the case of a 28-year-old man with anal GIST, and a review of the cases that have been reported so far in the literature. The patient was referred for treatment of a gradually enlarging perianal mass. Clinical examination and imaging including orthosigmoidoscopy, transanal ultrasound, and magnetic resonance imaging (MRI) revealed a mass sized 7.5 cm in greatest diameter, in relation with the sphincters, which was excised under general anesthesia. His postoperative course was uneventful and he was discharged on the fourth postoperative day. Pathologic examination revealed characteristics of anal GISTs and further treatment with tyrosine kinase inhibitors was planned. BACKGROUND Cutaneous melanoma is known for its aggressive tendency for metastasis, most commonly to lymph nodes, lung, liver, and brain. CASE REPORT We present the case of an 80-year-old male, with a history of cutaneous melanoma, found to have biopsy-proven metastatic melanoma deposits in the sigmoid colon. This rare case was initially thought to be a walled-off perforation secondary to diverticulitis. Although, computed tomography, showed features more strongly suggestive of malignancy. We propose early consideration of bowel metastasis in patients with a history of cutaneous melanoma presenting with lower gastrointestinal symptoms. CONCLUSION Once malignancy is included in the differential for any inflammatory type bowel lesion, arranging endoscopy and biopsy is essential to confirm the diagnosis and guide management. Introduction: Gastrointestinal Stromal Tumors (GISTs) are unique mesenchymal tumours arising from the interstitial cells of Cajal and occur anywhere in the digestive system, from esophagus to anorectum. Such tumors are found, most often, in the wall of the stomach thus representing 1-3% of all gastrointestinal malignancies. Their behavior is driven, 90-95% of the times, by mutations in the Kit gene. Prognosis depends on tumor's size and the number of mitosis, with a 5-year survival range from 50-80%. As far as HIV positive patients are concerned, there has been observed no greater incidence of GISTs. Case report: A HIV-positive 49-year-old male patient presented to the surgical outpatient clinic citing melena. Endoscopy displayed a sizeable tumor on the lesser curvature, macroscopically resembling to a GIST neoplasm. An abdominal CT was performed , pre-operatively, so as to clarify the stage of the tumor. In laparotomy, a gastrotomy was performed followed by enuclea-tion of the tumor to healthy tissue margins. Post-operatively course was uneventful and the patient was discharged on the 7 th post-operative day. Pathology examination showed the existence of moderate differentiated spindle cells and the immunohisto-chemical study came out positive for the classic GIST cell markers CD34 and CD117, confirming the stromal origin of the tumor. Conclusion: In HIV positive patients, the differential diagnosis between GISTs and GI Kaposi tumors is yet challenging. In such patients , GISTs' diagnosis requires thorough imaging examination, accompanied by a complete immunohistochemical analysis. Thus, the help of an experienced pathologist is crucial in order to set accurate diagnosis and to provide the right treatment.

3D printing is a technology which allows the sequential addition of material layers and offers us the opportunity to print parts and components from different materials with different mechanical and physical properties. This method was coined in 1986, by Hull,1 who patented it as a method. 3D printing for in vitro studies of aorta began to be applying before a few years ago.2 Nowadays, these technologies are very useful in maxillofacial or surgery, reconstructive surgery, neurosurgery, pediatric cardiac surgery and in many other areas of medicine.3

Adrenal pseudocysts are rare entities and occurred in the 5th and the 6th decades of life. They are discovered accidentally, while appearing with nonspecific clinical and imaging findings. We report a case of a 28-year-old woman presented in our Emergency Department complaining about upper abdomen pain. Computed tomography revealed a hypodense cystic lesion containing hyperdense material. The size of a mass was 11. 7 × 9.3 × 6.6 cm in diameter close to the pancreas, but the origin was from the left adrenal gland. The mass was excised with surgical laparotomy. Giant adrenal pseudocysts are rare entities. Final diagnosis usually confirmed with the pathology examination. Management of such adrenal lesions depends on the unique characteristics, the surgeon’s experience, and local resources.

Lipomas of the ligamentum teres hepatis are extremely uncommon. There have been only a few cases reported in the literature, including lipomas of the falciform ligament of the liver. Here we report a case of torsion and infarction of a lipoma of the ligamentum teres hepatis in a 43-year-old female patient, who presented with acute epigastric pain, nausea and vomiting. Diagnosis was based on computed tomography and magnetic resonance imaging. Patient underwent exploratory laparoscopy followed by laparoscopic excision of the infracted lipoma. Finally, we also provide a mini-review of the literature in order to highlight that although rare, this pathology should be included in the differential diagnosis of acute abdomen.

A versatile method is reported for the manufacturing of antimicrobial (AM) surgery equipment utilising fused deposition modelling (FDM), three-dimensional (3D) printing and sonochemistry thin-film deposition technology. A surgical retractor was replicated from a commercial polylactic acid (PLA) thermoplastic filament, while a thin layer of silver (Ag) nanoparticles (NPs) was developed via a simple and scalable sonochemical deposition method. The PLA retractor covered with Ag NPs (PLA@Ag) exhibited vigorous AM properties examined by a reduction in Staphylococcus aureus (S. aureus), Pseudomonas aeruginosa (P. aeruginosa) and Escherichia coli (E. coli) bacteria viability (%) experiments at 30, 60 and 120 min duration of contact (p < 0.05). Most colorectal cancer patients in the early stages of the disease do not display any alarming symptoms. A total percentage of 9-27% of colorectal cancer patients present with acute abdomen, bowel obstruction, perforation, or bleeding. Perforation as the first presentation of the disease is seen in no more than 2.6-10% of patients. Intestinal perforation may be found on either the site of the tumor or on a more proximal site, caused by distention of the bowel due to peripheral obstruction. This is a case of a 75-year-old female patient who presents in the emergency department with retroperitoneal cecal perforation due to an obstructing tumor of the ascending colon. She underwent an emergency right hemicolectomy and washout of the retroperitoneal space. OBJECTIVE A colon microarray data is a repository of thousands of gene expressions with different strengths for each cancer cell. It is necessary to detect which genes are responsible for cancer growth. This study presents an exhaustive comparative study of different machine learning (ML) systems which serves two major purposes: (a) identification of high risk differential genes using statistical tests and (b) development of a ML strategy for predicting cancer genes. METHODS Four statistical tests namely: Wilcoxon sign rank sum (WCSRS), t test, Kruskal-Wallis (KW), and F-test were adapted for cancerous gene identification using their p-values. The extracted gene set was used to classify cancer patients using ten classifiers namely: linear discriminant analysis (LDA), quadratic discriminant analysis (QDA), naïve Bayes (NB), Gaussian process classification (GPC), support vector machine (SVM), artificial neural network (ANN), logistic regression (LR), decision tree (DT), Adaboost (AB), and random forest (RF). Performance was then evaluated using cross-validation protocols and standardized metrics viz. accuracy (ACC) and area under the curve (AUC). RESULTS The colon cancer dataset consists of 2000 genes from 62 patients (40 cancer vs. 22 control). The overall mean ACC of our ML system using all four statistical tests and all ten classifiers was 90.50%. The ML system showed an ACC of 99.81% using a combination WCSRS test and RF-based classifier. This is an improvement of 8% over previously published values in literature. CONCLUSIONS RF-based model with statistical tests for detection of high risk genes showed the best performance for accurate cancer classification in multi-center clinical trials.

Collateral circulation is an alternative path occurring in case of venous or artery obstruction. This path may usually develop after primary recanalization. In our case, a 62-year-old woman presented to our Emergency Department complaining about a suprapubic swelling with a cyanotic discoloration of the overlying skin for the past 10 days for which she had been previously prescribed antibiotics. Investigation with ultrasound and contrast-enhanced computed tomography was performed. An imaging study revealed thrombosed pubic varicose collateral veins due to deep vein obstruction and occlusion of the left external iliac vein. The patient was treated with low-molecular-weight heparin, and swelling subsided gradually. Collateral veins of the abdominal wall and over the pubic tubercle are highly predictive of deep venous obstructive disease proximal to the groin level. Introduction.Hydatid cyst disease caused byEchinococcus granulosusis rarely presented in the kidneys, whereas isolated renal occurrence is estimated to be about as low as 2–4% of all cases. We present a case of a female patient suffering from this condition that was treated successfully in our department.Case Presentation.A 44-year-old woman was incidentally diagnosed with a 14 cm left renal cystic mass through ultrasound imaging performed during upper abdominal pain investigation. Laboratory examinations were normal and CT imaging set the diagnosis of an isolated left renal hydatid cyst. The cyst was excised and the postoperative period was uneventful.Discussion.Isolated renal hydatid cyst is a very rare condition and could possibly be misdiagnosed with other renal masses. The clinical history, laboratory tests, and thorough radiologic imaging are crucial for the accurate preoperative diagnosis.

Solitary fibrous tumor (SFT) is an unusual mesenchymal neoplasm that most often arises in the pleura; however, it has recently been described in a number of extrapleural sites. This report describes an extremely rare case of a benign SFT arising in the pancreas. A 41-year-old woman presented in the clinic with right upper abdominal pain. Subsequent ultrasonographic studies revealed a 1.5 × 1.5 × 1.4 cm hypoechoic mass within the pancreatic body, which was later confirmed on both helical computerized tomography and magnetic resonance imaging studies. An endocrine tumor was clinically suspected. Laparoscopic enucleation of the mass was performed. Microscopically, the tumor was composed of bland uniform spindle cells arranged between collagen bundles. On immunohistochemical studies, these spindle cells were positive for CD34 and bcl-2 but negative for cytokeratin (AE1+AE3 and Cam5.2), smooth muscle actin, desmin, S-100, and c-kit. Stoma prolapse represents one of the most common late complications, occurring in 1-16%. Final rate depends on systematic follow up of the patient and the primary technique. A 49-year-old male patient presented in the Emergency Department, complaining about stoma prolapse, pain and stoma care difficulties. On admission, his colostomy protruded ~20 cm from the skin. The symptoms were local pain and psychological stress. The prolapse was repaired successfully with a simple revision procedure under local anesthesia, by resecting the prolapsed part of the bowel and reconstruction of stoma. Prolapsed part of the colon is removed and the remaining end of the colon is fixated to the abdominal wall. Colorectal surgeons must familiarize with management of stoma complications. Stoma revision under local anesthesia is an alternative and safe method.

Gallbladder adenomyomatosis is an uncommon and benign condition that can exist with or without lithiasis and does not, usually , present any symptoms. We report a case of symptomatic gallbladder adenomyomatosis in which adenomyomatosis had altered the shape of the gallbladder to an " hourglass "-shaped form, as shown during the preoperative imaging control. The patient was treated with laparoscopic cholecystectomy and no further treatment was needed. This morphological type of the gallbladder adenomyomatosis is rare and it can alter the shape of the cyst, as shown in our case. The accurate diagnosis is set only after pathologic examination and the last it is important in order to exclude probable gallbladder malignancies.

Conclusion: Pancreatic paraganglioma is a rare entity with limited cases reported. The diagnosis pre-operatively is speculative and malignant tumor is a feasible differential diagnosis. Surgical resection is necessary for histological assessment. However, the risk of malignant transformation justifies aggressive management.

Introduction: The presence of foreign bodies in the stomach may be after accidental or voluntary swallowing and it is a relatively common cause of admission in emergency department. Treatment option still remains controversial considering the waiting policy or promptly intervention either endoscopic or surgical. This case series presents our experience after surgical removal of sizeable gastric objects in psychiatric patients in the attempt of self-harm action. Cases presentation: We present two cases of swallowed peculiar sizeable foreign objects, such as metallic spoon and key, where endoscopy wasn't successful in removing them. Both cases were managed with gastrotomy, through limited midline laparotomy, and removal of the objects. Postoperative period was uneventful. Conclusion: Surgical intervention is crucial for gastrointestinal foreign body removal, either when endoscopy fails, especially in foreign objects of peculiar size and shape, or in case of gastrointestinal complications.

3D printing provides the sequential addition of material layers and, thus, the opportunity to print parts and components made of different materials with variable mechanical and physical properties. It helps us create 3D anatomical models for the better planning of surgical procedures when needed, since it can reveal any complex anatomical feature. Images of abdominal aortic aneurysms received by computed tomographic angiography were converted into 3D images using a Google SketchUp free software and saved in stereolithography format. Using a 3D printer (Makerbot), a model made of polylactic acid material (thermoplastic filament) was printed. A 3D model of an abdominal aorta aneurysm was created in 138 min, while the model was a precise copy of the aorta visualized in the computed tomographic images. underwent laparoscopic cholecystectomy. Upon postoperative
specimen dissection, multiple intraluminal, as
well as intramural gallstones, were discovered. Moreover,
histopathologic examination demonstrated the presence of
gallbladder adenomyosis along with chronic inflammatory
alterations.
The second case refers to a 58-year-old female patient,
scheduled for elective laparoscopic cholecystectomy due to
symptomatic cholelithiasis. Upon postoperative specimen
dissection, multiple intramural gallstones, but no intraluminal
gallstones, were discovered. Histopathologic examination
demonstrated signs of chronic inflammation,
without adenomyosis or other pathology.
Conclusion: Intramural gallbladder lithiasis is a rare condition
whose pathogenetic background is not clarified yet.
Further studies need to be conducted, in order to establish
its etiology, as well as its possible correlation to other
complications of gallstone disease, including malignancy
