Neonatal Resuscitation

This document discusses neonatal resuscitation, including definitions of key terms, signs of a healthy newborn, important equipment, and resuscitation procedures and algorithms. It defines terms like neonate, resuscitation, ventilation, and Ambu bag. It outlines signs of a healthy baby and important preparation steps like having supplies ready. The document provides guidance on when to provide ventilation and chest compressions based on heart rate, with a compression to breath ratio of 3:1 at 120 events per minute.

Neonatal

Resuscitation
Dr. Jessley
24.2.23
Definitions?
• Neonate- child less than 28 days
• Resuscitation- process of reviving someone from unconsciousness or apparent
death
• Ventilation- The exchange of air between the lungs and the atmosphere so
that oxygen can be exchanged for carbon dioxide in the alveoli (the tiny air sacs
in the lungs).
• Ambu bag/BMV airway- A bag valve mask (BVM), sometimes referred to as an
Ambu bag, is a handheld tool that is used to deliver positive pressure
ventilation to any subject with insufficient or ineffective breaths. It consists of a
self-inflating bag, one-way valve, mask, and an oxygen reservoir
• Algorithm- is a set of well-defined instructions to solve a particular problem.
Healthy newborn baby:
• Breathes regularly/cries soon after birth
• Has a pulse rate of 120-160 beats per minute
• Has a respiratory rate of 40-60 breaths per minute
• Breaths comfortably without grunting, nasal flaring or in-drawing of the lower chest wall
• Curls up with bent/flexed arms, legs and hands
• Weighs over 2.5kg
• Has pink lips and tongue by 5 minutes
• Sucks at the breast: feeds about 12 times a day
• Passes urine at least 8 times a day
• Passes meconium within 24 hours
• Has normal anatomy; no malformation
Key points:
• Always have the things you need ready before every delivery
• Always try to have at least 2 health workers present for delivery and
10-15mins after delivery
• Know your patient!
 2 dry blankets/nappies
 A baby hat
 A sucker/bulb syringe to
suck airway if needed
 Self-inflating neonatal bag-
valve-mask resuscitator
(BMV or Ambu bag) with
neonatal masks
When to suction?
Only when VERY THICK meconium
blocks the airway

Compression to breaths ratio?
3:1 and a rate of 120 events per minute to achieve
approximately 90 compressions and 30 breaths per minute.
