These tables are used in case of no reliable drug index is available.
These tables are mostly not for neonates (revise their doses according to age and weight).
These tables are based on Medscape drug reference, BNF and drugs’ own pamphlets.
Most doses are intended for < 12 years or < 30 kg, if higher age or weight, it is preferable to re-check the dose as may be higher than mentioned here.
Some concentrations are left out intentionally as not commonly used in pediatrics.
Please always re-check the concentration of given drugs to make sure it matches the one you used for calculation.
