Statement of Commitment to Comply with OSH Requirements

I ALDRIN VILLACERAN by accomplishing this Construction Safety and Health


Program do hereby commit and bind ourselves to comply with the requirements
of Department Order No. 13, series of 1998 – Guidelines Governing Occupational
Safety and Health in the Construction Industry and the applicable provisions of
the Occupational Safety and Health Standards (OSHS).

We also acknowledge our obligation and responsibilities to provide throughout


the course of the project the appropriate Personal Protective Equipment (PPE) as
added protection for our workers and employees.

We also hereby commit to implement the necessary job safety and health
instructions and training to all our workers for the entire duration of the project as
well as the safety of the general public.

COMPANY SAFETY AND HEALTH POLICY

INTRODUCTION: POLICY STATEMENT

ALDRIN VILLACERAN is committed to the protection of its resources specifically


the employee and physical assets against human distress and financial loss resulting
from accidental occurrence.

In fulfilling this commitment, which is essential to end equally important as


production and quality objective, we will provide and maintain safe work environment
and protective public against foreseeable hazards resulting from construction
operation.
Consistent with this, the Owner will;
 Implement all safety working standards and to support any worker with
medical and financial aid in case of impairment as a result of accident in the
workplace.

 Support all efforts to make safe integral part of operations enforcing


mandatory safety standards; eliminate safety health hazards; ensure healthful
and safe working conditions in all places of works as set forth by the
Department of Labor and Employment.

 Involve employees and contractors in the improvement of Occupational


Health and Safety Performance.

 Ensure that the Company’s employees, workers and visitors are informed and
understand their obligations in respect of this policy.

 Communicate openly with workers, government and community on


Occupational Health and Safety issues.

 Comply with, and require all employees to comply with, occupational health
and safety standards and regulations under the act which are applicable to
their actions and situations.

 Comply all applicable laws, regulation and standards where adequate laws do
not exist, adopt and apply standards that’s reflect the Company’s commitment
to the safety and health.

 Contribute and support to the development of relevant Occupational Safety


and Health policy.

DUTIES AND RESPONSIBILITY OF SAFETY COMMITTEE

 CHAIRMAN / PROJECT MANAGER

a. Familiarizes him/her-self with health and safety regulations related to his/her


area of responsibility.

b. Directs and coordinates health and safety activities within the area of
responsibility

c. Ensure arrangements for prompt medical attention in case of serious injury


have been provided for each job, to include transportation, communication,
and emergency telephone numbers; and a person with valid certified first aid
training is available required.

d. Requires all employees supervised to use individual protective equipment and


safety devices.

e. Ensures that safety equipment is available, maintained, used and stored


correctly.

f. Instruct and trains all persons within area of responsibility in job health safety
requirements.

g. Conducts frequent and regular health and safety inspection of work area.
Directs correction of unsafe conditions.

h. Conducts weekly safety briefings with all supervisors and/or workers.

i. Ensures that foreman is aware of and comply with requirements for safe
practices.

j. Reviews all accident/ incidents with foreman and workers involved. Ensures
that corrective action is taken immediately to eliminate the cause of the
accident.

 SECRETARY / SAFETY OFFICER

a. Administer all aspects of the occupational health and safety program.

b. Develops programs and technical guidance to identify and remove


physical, chemical, and biological hazards from the facilities, operations,
and sites.

c. Assist management and supervisors in the health and safety training of


employees.

d. Conducts inspections to identify unhealthy or unsafe conditions or work


practices. Complete written report of inspection.

e. Recommends programs and activities that will develop and maintain


incentives for and motivation of employees in health and safety.

f. Develops and maintain accidents and incident investigation and reporting


procedures and systems. Investigates all accidents and takes action to
eliminate accident causes. Reportable incidents consist of fatalities, lost
work day cases, and without work days requiring medical treatment. Keep
management informed f findings.

g. Conducts safety toolbox meeting with all workers under his/her
supervision.

 MEMBERS

a. Be familiar with and comply with proper health and safety practices.

b. Use the required safety devices and proper personal protective safety
equipment.
Notify supervisor immediately of unsafe conditions/acts, accidents, and
injuries.

GENERAL SAFETY PROMOTION


SAFETY BULLETIN PROGRAM
 There should be adequate number of bulletin boards or signage for safety
purposes. It shall be located or visible to every employee, workers and
visitors.

 Bulletin boards shall provide at all fixed work site.

 Items to be placed in bulletin boards shall be regulating. Its poster are used,
they should be change weekly.

 Poster shall be selected to support programs aimed major accidents.


SAFETY CONSTRUCTION PROGRAM
a. TOOLBOX MEETING – 15-20mins in the job safety toolbox meeting for
doctrination of all workers shall be conducted every Monday (or as often as
needed) by Safety Officer and his/her assistant before work commences. The
merits of the safety program will be stressed to each employee concerned.
Safety Officer shall be stressed to each employee concerned. Safety officer
shall keep record of toolbox talks, topic discussed and attendance.
Disciplinary action will be given to employees that are found to be repeatedly
violating compliance. Topics to be discussed shall be in general such as;

o Site Safety Regulation

o Good work practices

o Personal Protective Equipment

o Fire Precautions

o Emergency procedures

o Topic related ongoing activities

b. Personal Protective Equipment

o Personal Protective Equipment must be worn as required for each job
in all operations where there is an exposure to hazardous conditions.
Equipment requirements will be reviewed by supervisor/foreman, etc.

o Employees are expected to utilize proper judgement in their personal


habits. When they report to work each morning they must be fit
condition to meet daily obligations.

o Goggles, face shields, helmets and other comparable equipment are


required to fit the eye and face protection needs of the employee for
each job.

o Hard hats and steel-toed safety work boots/shoes must be worn by all
employees at all times where required.

o Appropriate gloves and boots are to be used when necessary for


protection against acid and other chemical which could injure
employee’s skin.

o Some form or elements of fall protection must be provided where


employees are exposed to any fall hazard of six feet or greater
(exceptions; scaffolds – ten feet, and ladders.) Depending on the
situation, this fall protection may be guardrails, safety nets, personal
fall arrest system (harness, lanyard, lifeline), hole covers, or any other
appropriate protection.

o All PPE’s should be contractor’s own expenses.

c. Environmental Control Plan

Objectives – Project the environment and surrounding communities from


hazards and environmental concerns posed by the construction activities in
the project.

Responsibilities – The project in charge shall be responsible for the


implementation of this environmental control plan.

o Hazards/ Concern measure

 Dust – Sprinkle dry grounds with water


 Noise – Mount equipment so that vibration will reduce
 Muddy roads – Provide tire washing facilities near exit gates

d. Protection to the General Public – Safety measures shall be implemented


to protect from hazards connected with construction operation and activities.

 Standard warning sign, traffic signs and barricades shall be installed as


required by government regulation and OSHS.

 Pedestrian detours are clearly defined, and pedestrians protected from


the dangers of road traffic when using any detour.

 The passage of vehicles across footpaths is to be supervised, to
remove danger to the public.

 Excavations are fenced, and, if they are like to retain water, are
covered and securely fenced to prevent access for children. If in public
places, they should have warning signs and warning lights at night.

 If scaffolding must intrude on to footpaths, it is to be clearly marked,


and padded if there is any risk of causing injury.

 Adequate security of the site is to be provided during non-working


periods. This involves ensuring excavations and openings are covered
or fenced, materials are stacked safely, plant immobilized, ladders
removed or lowered to the ground.

e. Materials Handling – Materials handling forms a large part of the


construction work. There are many areas in which the difficult, costly, and
frequent dangerous manual works handling could be replaced with the
machines. This section outlines the minimum safety precaution to be
observed by the employees who will work with the materials handling
equipment.

o Mechanical handling equipment shall be provided when the weights to


be lifted, carried or moved are beyond the allowable limits for the
manual handling.

o Mechanical equipment shall be ensured in safe condition prior to each


use.

o The load capacity of the equipment shall be strictly followed.

o Materials handling equipment must be maintained in good working


condition and properly used due respect to the safely.

f. Storage Procedure

o In storing cylinder, never drop or permit them to strike each other.

o Avoid dragging and sliding cylinder – even in a short distance.

o Do not use cylinder as “roller” for moving materials or other equipment.

o Keep cylinder in designated storage areas when not use.

o No part of a cylinder should be subjected or expose to a temperature


more than 125°F.

o Do not set empty and full cylinder at the same time.

g. Fire prevention

o All firefighting equipment shall be conspicuously located, accessible,


inspected periodically, and maintained in operating condition. An
annual service check and monthly visual inspections are required for
fire extinguisher.

o All employees’ musk knows the location of the firefighting equipment in


the work area and has knowledge of its use and application.

o Only approved safety cans shall be used for handling or storing


flammable liquids in quantities greater than one gallon. For one or less
gallon, only the original container or safety can will be used.

o When heat producing equipment is used, the work area must be kept
clear of all fire hazards and all sources of potential fire will be
eliminated.

o A salamander or other open – flame device will not be used in confine


or enclosed structure without proper ventilation. Heaters will be vented
to the atmosphere and located an adequate distance from walls, ceiling
and floors.

o Fire extinguisher will be available at all times when utilizing heat –


producing equipment.

o Storage of LPG within buildings is prohibited.

h. Health Program and First Aid Services.

o All persons employee throughout the course of work shall be physically


qualified for performing the duties to which they are assigned. Workers
must undergo physical examination upon employment by the
accredited laboratory or hospital at no cost.

o Facilities shall be available to provide first aid attendant. He/She is


trained and deemed qualified t administer first aid treatment. Each
employee shall also familiarize themselves with the use of first aid kit
contents.

o A first aid kit shall always available in every floor o the working area.

o A daily record of all aid treatment shall be recorded and maintain at the
field.

i. Workers Welfare Facilities

 DRINKING WATER

 An adequate supply of wholesome drinking water should be
provided to all persons on the construction work in accordance with
the following conditions

 It should be readily accessible to all persons engaged in the work


and clearly labeled as drinking water.

 Portability of drinking water used on the site shall be checked


regularly. Containers for drinking water shall be identified for that
used only, tightly covered when in used and not use and not use for
any other purposes

 A common drinking container should not be used. If it is stored in a


container, the container:
— should be clean and protected from contamination and from
heat; and
— should be emptied and refilled daily from a wholesome source.

 First-aid facilities

 Every employer is required to provide adequate first-aid facilities,


appliances and requisites.

 Normally, first-aid is to prevent the condition of the injured person


from becoming worse until more skilled help becomes available.

 It is essential to the success of first-aid treatment that it should be


immediately available.

 The longer a wound, even a very small one, is left untreated the
greater the risk of infection.

j. Proposed hours of work and rest breaks.

o The work schedule will be from Monday to Saturday, 7:00am – 4:00pm,


with 15mins break in the morning and afternoon and 1hour break for
lunch.

k. Construction Waste Disposal

o All construction waste and debris are collected 3 times a week and
disposed properly at disposal area.

l. Emergency Preparedness

o Encompasses all activities that are necessary to be prepared people


and organization to respond to emergencies and disaster

 Mutual Aid

o Emergency Recovery – Embodies all activities necessary to bring the


organization back to normal or routing operations.

o Emergency Response Team – In the event of s disaster, or eminent


emergency situation, several emergency response teams (formed soon
after the contingency plan is initially implemented) will be responsible
for properly activating, administering the disaster plan in conjunction
with the ERT coordinator (person chosen at the time the contingency
plan is adopted). Some teams, and their responsibilities, deserving due
consideration, but not necessary contained in every plan include;

 Security Teams
 Alerting and warning
 Incident recording and reporting
 Law enforcement
 Movement / evacuation supervision

o EVACUATION AND TRANSPORTATION

The goal of this function is to evacuate people, and move


resources out of the threatened area. Depending on the emergency
circumstances, evacuation of the building, site or area may require
provisions for completing a number of sequential actions. All
interactions among these responses must b e identified, and thought
out in a systematic fashion so that proper sequence can be established

to ensure operations flow smoothly, and that no unnecessary risks
occur. This section of the plan should have provision for;

 Describing conditions under which an evacuation should be


ordered.

 Developing evacuation procedures for the appropriate options


for various hazards.

 Identifying people responsible for ordering an evacuation.

 Providing maps areas where different departments and


personnel should report after exiting the building.

 Provide for organized head count, and develop system for


identifying missing persons.

 Establish system that ensures that the vital records are


evacuated.

 Periodic evacuation drills to ensure that the procedure is


working smoothly.

 Effective procedures for the orderly evacuation of a facility to a


safe area are paramount. Evacuation plans should be designed
to evacuate the facility as quickly and safely as possible.

 The number and capacity of facility owned and/or contracted


transportation available for an immediate or delayed response to
an emergency call. The names of operators and contact
procedures should be included.

 Construction service utility vehicles should be available at all


time

o FIRE PREPAREDNESS

In today’s business world, there is an increasing emphasis being


place on the use of computer and electrical equipment. As industries
continue to push toward overhauling and updating their data systems,
there is greater risk for electrical file, in addition many companies
maintain a vast amount of combustible and flammable materials in the
work place. With this in mind, everyone must be aware of what to do in
the event that fire s ignited. There are two specific cases that need to
be addressed:

 DURING WORKING HOURS


Upon discovery of a fire, the following procedures should be
followed:

 Immediately report the fire to the local fire department


 Report the fire to the nearest ERT chairman or
department Manager

 The fire should be investigated by A or B, and a decision
should be made as to whether or not the building should
be evacuated.

 If the fire cannot be extinguished locally, the ERT


Coordinator, or one of the Chairmen will be responsible
for evacuating the premises.

 The manual alarms should be activated at this time. (The


employees should be aware of the locations of the alarms
– they are generally found near all stairway exits.

 All employees should vacate the building by way of the


nearest stairwell, and report to their designated areas
immediately.

 DURING NON – WORKING HOURS

 Report the fire to the local fire department.

 If the fire is small in size, attempts should be made to


extinguish it with the use of portable extinguisher.

 Notify anyone in the office or building of the fire, and


instruct them to exit the building immediately.

 The manual alarms should be activated at this time. The


employees should be aware of the location of these
alarms.

 If the fire cannot be extinguished locally, exit the building


by way of the nearest stairwell.

m. INCIDENT REPORTING AND INVESTIGATION

o PURPOSE – A thorough incident investigation is to be completed for


every incident to identify root causes and recommended appropriate
corrective actions.

o INCIDENT – A non-deliberate, unplanned event which may produce


undesirable effects and preceded by unsafe, avoidable act(s) or
condition(s).

o IMMEDIATELY REPORTABLE INCIDENT – Incidents that require


immediate notification to the Safety Department. Such;

 Injuries or illnesses requiring emergency medical treatment.

 Vehicle incidents with injuries, property damage, or when any
vehicle needed to be towed.

 Damage to the public utilities.

 Damage to private property.

 Any other serious incident or near miss that requires immediate


follow-up or corrective action
INVESTIGATION PROCEDURES;
o All incidents are to be investigated by the Safety Officer
investigation should answer at a minimum, the following
question:

 WHO – everybody that was involved in the incident.


 WHAT – exactly what happened
 WHERE - where did the incident occur
 HOW – what caused the incident
 WHEN – when did the incident occur
 WHY - why did those actions or condition occurs, and why
were those actions or condition not prevented.
 What immediate measures have been taken to prevent
recurrence?
 What permanent corrective actions are recommended?
The appropriate Incident Report needs to be completely filled out and
turned in along with any supporting documentation to the Safety
Department within twenty-four hours.
 Hazards Analysis. List all basic job tasks, the hazards associated with them,
and a list of controls. The form is pre-filled with some basic tasks, hazards,
and controls. Use them if they apply. Remove those that are not relevant to
the project. Additional tasks, hazards, and controls may need to be added.
Note: Be sure to list the Hazards rather than the Injuries, Accidents, or other
results of a hazard. For example:

Use of hand tools Broken or All tools and equipment must be inspected by
Unauthorized authorized personnel prior to use. This includes tools

started. Any tool or piece of equipment deemed

that will be placed on the ladder. This includes the
weight of anything that may be supported by the user.

Use of Fixed Ladders Falls Before use, check the status of the fixed ladder by
looking at the yellow ladder, and the white ladder
inspection tags.

If either of the yellow or white tag is missing or illegible


do not use the ladder and contact the authorized
personnel.

Keep hands free of additional material when climbing.


Use a rope to lift material and tools.

Do not overextend when working by reaching out to far


on the side of a ladder. Keep your belt buckle between
the two side rails.

Welding, Cutting, Grinding Fire All personnel engaged in welding or cutting job tasks
shall wear all industry recognized PPE to protect from
burns either to the skin or the eyes.

Sparks
Generation
A 20 lb. ABC fire extinguisher shall be readily accessible
and immediately available when any open flame work is
performed. Combustibles and flammables must be kept
clear of the open flame work area. Fire watches must be
competent in the use of fire suppression equipment. Fire
extinguishers must be checked monthly.

Oxygen and acetylene cylinders will be stored and


secured in an upright position with caps in place with a
minimum of 20 feet separation or separated by a
noncombustible barrier at least 5 feet high having a fire
resistance rating of one half hour except when in an
approved cart ready for use. Proper signage regarding
“No Smoking” or “Ignition Sources” must be posted.

Smoking is only allowed in designated smoking areas.

All torch set cylinders must have the valves closed and
the system de-pressured before going to breaks or
lunch.

All oxygen/acetylene set-ups must be equipped with


flash back arrestors

Butane (e.g., Bic) lighters are not allowed to be carried


by contractor employees engaged in welding or torch
cutting/brazing or soldering operations.

Working in Confined Spaces Asphyxiation, Personnel entering this type of space must wear
Engulfment or appropriate respiratory equipment, usually Self
Entrapment Contained Breathing Apparatus (SCBA) or Supplied Air
Respirators (SAR).

Ventilation should be used to remove or dilute the gases


and supply fresh air.

Scaffolds Falls Scaffolds must be used and inspected in accordance


with the manufacturer’s instruction for each specific
Damaged/ model and type of scaffold being used.
Broken
Equipment All wheels equipped with locking devices must be locked
prior to climbing, all bolts and nuts must be tight, and all
Improper cotter pins must be in place and secured before use.
Assembly
When working off scaffolds that are four ft. or more

handrails are required.

The scaffold competent person and the qualified person


may be the same individual.

Newly erected or modified scaffolds must be inspected

accordance with the manufacturer’s instruction for each

Elevated work platforms and must be inspected by a

inspection on the elevated work platform inspection tag.

If the elevated work platform does not pass inspection,


remove the inspection tag, and replace it with a red “Do

out of service are to be reported to the contractor’s
supervisor immediately. Contact the project
specialist/tech rep to obtain a red “Do Not Use tag.

CONSTRUCTION SAFETY PRACTICES
 EXCAVATION

o A work permit must be obtained before commencement of work.

o Before any attempts is made to excavate, check public utilities


for location of the underground lines.

o A manual excavation is being done, workers must be warned


about living picks, paving breakers, or other powered tools.

o Excavated materials should be placed 1meter away from the


edge of the excavated areas, unless the board or other effective
barricades have been installed to prevent fallback.

o When the excavation must remain open during the duration of


the construction work, barricades, fences and warning sign is
necessary.

o Every excavation over 1meter (3ft) shall be kept free of water at


all times.

o Every excavation over 1meter deep shall be provided with


means of access and escape in case of flooding or collapse of
the excavation work.

o The excavation does not affect the stability of neighboring


buildings.

o The sides of the excavation are sloped back or battered, with


the angle of batter sufficient to prevent collapse.

 DEMOLITION

o All demolition operations of the building with six (6) meters high
shall be under the supervision of a competent person.

o Posting danger signs shall be posted around the structure and


all doors and opening giving access to the structure shall be
kept barricaded or guarded.

o No person except the workers who are directly engaged in the


demolition shall enter a demolition area within the distance of 1
½ times the height of the structure being demolish.

o Fenced around the demolition area and no unauthorized person


shall allow within the fenced area.

o PPE’s likely safety shoes, hard hats, goggles and working
gloves should be use/wear by workers upon entering the
demolition area.

o Floors are not overloaded with demolition debris or plant being


loaded on to floors.

 SCAFFOLDING/FORMORKS

o Scaffolds or temporary forms should be securely fastened to the


building or structure. If independent of the building, it should be
braced to prevent it from breaking away.

o Scaffolds should be inspected regularly. Loose or missing part,


cracks, and splinter in uprights braces and steps should be
noted and repaired at once.

o All scaffolds more than 6ft above adjoining surfaced shall be


effectively guarded with guardrails. Should have 40 inches high.

o Every workers working on the scaffold must be provide with and


required to wear Safety Harness.

o There is proper access to the scaffold platform

o The scaffold is adequately braced to ensure stability.

o Guard rails, mid rails and toe boards shall be provided on both
sides and both ends of suspended scaffolds.

o Where suspended scaffolds are above thoroughfares, adequate


and prominent warning notices shall be displayed.

o The footing or anchorage for scaffolds shall be sound, rigid and


capable of carrying the maximum intended load without settling
or displacement. Unstable objects, such as barrels, boxes, loose
brick or concrete blocks may not be used to support scaffolds or
planks.

 TEMPORARY ELECTRICAL INSTALLATION

o All temporary electrical installation works must be carries out by


experience electricians and no matter what voltage is used
should be always treated seriously.

o Temporary electrical cables must be covered or elevated, so


they do not create tripping hazards.

o Before any part of newly installed electrical system or equipment
is energized, it must be tested.

o Energized junction boxes and circuit breaker panel shall be


covered at all time

o Generator sets and high voltage equipment should be enclosed


with fences with warning signs of “Danger high voltage” shall be
posted.

o Multiple connections of cable from safety switch or circuit


breaker shall not be allowed.

o PPE’S is highly recommended at all times.

 MASONRY

o For work at an elevated location, in addition to having a work


platform with guardrails, care shall be exercise to ensure that
materials to be handled should not exceed allowable scaffolding
loading. Bricks and blocks shall neatly stack. They shall be
passed by hand, not thrown when moving.

HAZARDS CONCERN/CONTROL MEASURE

 WELDING, CUTTING AND BURNING

o Proper grounding of welding machine shall be provided.


o Maintain adequate ventilation in an enclosed worked place.
o Cables and hoses must not cause tripping hazards or explosive
to damage.
o No hoses showing leaks, burns or worn places shall be used.
o A torch lighter must be used to light torch, the use of electrode is
strictly prohibited.

 PROTECTIVE MEASURES

o Welding and cutting operations shall be prohibited in room or


areas containing combustible materials or proximity to explosive
or flammable liquids.
o Operators of arc welding must always switch off the current to
the electrode holder where never it is to be set down or its not
actually needed.

 PAINTING

o When solvent cleaning, the toxic and combustion characteristics


of the solvent shall determine and appropriate protection
provided.

o The use of Safety Harnesses is mandatory for all work more


than 10ft (3m) above grade, which is not done from a fixed
platform with guardrails.

o Pressurized spray guns are frequently used for paint application.

o If spray painting is done in an enclosed area, adequate


ventilation and/or means of safety exhausting fumes must be
provide.

 ROOFING

o The use of Safety Harnesses and lanyards is mandatory for all


work more than 10ft (3m) above grade.

o The roof structure is accessible for the installation of safety nets,


anchorage points and static lines.

o Guard railing or guardrail attachments are incorporated in the


perimeter structure, especially prior to erection.

o Provide temporary walkways where any person is required to


work on the roof or use it as a means of access. (These
walkways should be at least 450 mm in width, and should
incorporate a guardrail. The walkways should be adequately
secured);

o Provide temporary roof ladders of sufficient strength where any


person is required to carry out work on or adjacent to any part of
a sloping roof sheathed in brittle material

o Provide individual fall arrest systems, including anchorages for


any person to work more than 10ft (3m) above grade.

 POWER TOOLS

o Select right tools for the job; use the tools for the task it was
intended.

o Avoid using metal ladders with an electrical tool.

o Always store electrical tools in a dry place

o Never carry power tools by its cord

o Keep work area free from anything that could catch fire from tool
spark.

o Don’t wear loose shirt, jewelry, or other items that could get in a
power tools. Tie back and cover long hair.

o Do not use a tool other than its designated or approved use.

COMPANY RULES AND REGULATION OF SAFETY AND HEALTH


ALL OF OUR SAFETY RULES MUST BE OBEYED. FAILURE TO DO SO WILL
RESULT STRICT DISCIPLINARY ACTION BEING TAKEN.
1. Head protection will be worn on job sites at all times by all trades.
2. Eye protection will be worn when there are potentials of hazards from flying
objects or particles, chemical, arcing, glare or dust.
3. Protective footwear shall be worn to protect from falling objects or stepping on
sharp objects. Athletic or canvass-type shoes shall not be worn.
4. Protective gloves or clothing shall be worn when required to protect against
hazard.
5. Harnesses and lanyards shall be utilized for fall protection as required on
Construction Safety Standards.
6. Keep your mind on your work at all times. No horseplay on the job. Injury or
removal from the job site or both can be result.
7. Precautions are necessary to prevent sunburn and to protect against burns
from hot materials.
8. If any part of your body should come in contact with acid or caustic substance,
rush to the nearest water available and flush the affected part. Secure medical
aid immediately.
9. The use of illegal drugs or alcohol or being under the influence on the same
project shall be cause for termination. Inform your supervisor if taking strong
prescription drugs that warn against driving and machinery.
10. Do not distract the attention of fellow workers. Do not engage in any act which
would endanger another employee.

11. Sanitation facilities have been or will be provided for your use. Defacing or
damaging these facilities is forbidden.
12. A good job is a clean job, and a clean job is the start of a safe job. So keep
your working area free from rubbish and debris.

13. Do not use a compressor to blow dust and dirt from your clothes, hair or
hands.

14. Never move an injured person unless it is absolutely necessary. Further injury
may result. Keep the injured as comfortable as possible and utilize job site
first-aid equipment until an ambulance arrives.
15. Know where firefighting equipment is located and be trained on how to use it.

16. Lift correctly with legs, not the back. If the load is too heavy GET HELP. Stay
fit control your weight. Do stretching exercises. Approximately twenty percent
of all construction related injuries result from lifting materials.
17. Nobody but operator shall be allowed to ride on equipment unless proper
sitting is provided.
18. Do not use power tools and equipment until you have been properly instructed
in the safe work methods and become authorized to use them.
19. Be sure that all guards are in place. Do not removed, displace, damage, or
destroy and safety device or safeguard furnished or provided for use on the
job, nor interfere with the use thereof.
20. Do not enter an area which has been barricaded.
21. If you must work around power shovels, trucks and dozers, make sure
operators can always see you. Barricades are required for cranes.
22. Never oil, lubricates, or fuel equipment while it’s running or in motion.
23. Trenches over five feet deep must be shored or sloped as required. Keep out
of trenches or cuts that have not been properly shored or sloped. Excavated
or other material shall not be stored nearer than two feet from the edge of the
excavation. Excavations less than 5ft may also require cave in protection in
some instance.
24. Use the “four and one” rule when using a ladder. One foot of base for every
four feet of height.
25. Defective ladders must be properly tagged and removed from service.
26. Keep ladder bases free of debris, hoses, wires, materials and etc.
27. Scaffolds planks shall be properly lapped cleared or otherwise secured to
prevent shifting.
28. Use only extension cords of the three-prolong type. Use ground fault circuit
interrupters at all times and when using tools in wet atmosphere (e.g.
outdoors) or with any temporary power supply. Check the electrical grounding
system daily.
29. The use of harnesses with safety lines when working from unprotected high
places is mandatory. Always keep your line as tight as possible.
30. Never throw anything “overboard”. Someone passing below may be seriously
injured.
31. Know what emergency procedures have been established for your job site.
(Location of emergency phone, first aid kit, stretcher location, fire extinguisher
locations, evacuation plans, etc.).
32. Never enter a manhole, well shaft, tunnel or other confined space which could
possibly have a non-respirable atmosphere because of lack of oxygen, or
presence of toxic or flammable gas, or has a possibility of engulfment by
solids or liquids. Make certain a qualified person tests the confined area with
an appropriate detector before entry, that the necessary safety equipment is
worn. Standby person may be required to be station at the entrance.
