Discussion / CBL # 01
TOPIC: AXILLA AND BRACHIAL PLEXUS
LEARNING OBJECTIVES: Students should be able to:
Describe the shape, location and boundaries of axilla and its anatomical significance.
Identify the contents of axilla and their important relations.
Outline the extension and division of axillary artery
Enumerate different branches of axillary artery and its important relations
Define axillary vein and its tributaries.
Describe the various groups of axillary lymph nodes and their drainage.
Describe the formation, parts and location of brachial plexus
Discuss the course, distribution and injuries of some major branches of brachial plexus (i-e Long
thoracic, Ulnar, Median, Radial and Axillary nerves).
Explain various mechanisms of injuries to the upper and lower roots/trunks of brachial plexus and the
resulting deformities in upper limb.
Define Terms
Axillary Sheath
Suspensory ligament of axilla
Examination of axillary lymph nodes
Prefixed and post fixed brachial plexus
Erb’s palsy
Klumpke’s palsy
Ape Hand
Claw hand
Waiter’s tip deformity
Wrist drop
Problem # 1
A 32 year old man purchased a house that was badly in need of renovations and painting. As he was
painting the ceiling in a room, he felt pain and tingling in his right shoulder and in the proximal part of
his right arm.
On Examination: Radial pulse at his right wrist was decreased when physician raised the patient’s
limb above the shoulder.
Patient felt tingling and pain around his shoulder after the limb had been elevated for several
minutes.
The physician also noted cyanosis, edema and distention of superficial limb veins.
Task:
Compression of what structures in the axilla could produce the sign and symptoms described
above?
What bony structures do you think could compress these axillary structures?
What is the clinical designation for this type of compression syndrome?
To be conducted by: To be supervised by:
Dr. Khalid Hussain Shaikh
Dr. Moazam Safiullah Channa
Department of Anatomy Prof. Dr. Shahid Hussain Soomro
To be discussed on according to weekly schedule
Discussion / CBL # 02
TOPIC: Joints of the upper limb
LEARNING OBJECTIVES: Students should be able to:
Enumerate joints of the upper limb.
Describe the shoulder joint. Also highlight the movements possible and muscles causing those
movements.
Explain why upper limb is freely mobile and less stable while lower limb movement is restricted and
limb is more stable.
Define scapula humeral mechanism.
Describe the elbow joint. Mention its type, variety, articular surfaces, ligaments and movements
possible with special reference of the nerves related with the joint.
Outline the features of superior and inferior radio ulnar joints.
Describe the wrist joint. Mention its type, variety, articular surfaces, ligaments and movements
possible.
Describe the flexor and extensor retinacula and structures passing underneath them.
Mention the facial spaces of palm.
Define Terms
Carpal tunnel syndrome
Dupuytren’s Contracture
Palmar aponeurosis
Carrying angle
Bursitis of elbow
Fibrous digital sheaths
Problem 01
A 52 year old carpenter visited his physician concerning severe “pins and needles”
sensations in his right hand, which also involved the palmar surface of his thumb and
lateral two and a half fingers. He told the physician that the pain was becoming
progressively worse. He said that he has been experiencing difficulty using his hand
tools and buttoning his shirt. During physical examination, the physician did not detect
objective impairment of sensation in the hand, but she noted slight weakness of the
thenar muscles. She told the carpenter that he has a nerve entrapment syndrome.
Task:
Using your anatomical knowledge of the wrist, what nerve was compressed?
What is the name of the entrapment syndrome described by the patient?
What conditions can cause this type of nerve entrapment?
What movements would aggravate the symptoms?
If this condition was not treated, what further signs would you expect to develop in a few
months?
To be conducted by: Dr. Moazam Saifullah Channa Dr. Uzma Lakho
To be supervised by:
Department of Anatomy Prof. Dr.Shahid Hussain Soomro
To be discussed on according to weekly schedule
