Before this day, I have inspected all the tools, materials, equipment,
and facilities we will be needing in our training session. I made sure
that everything was safe, available, and accessible to use.

again, welcome and thank you for choosing JLC Training Center as
your training venue.

PURPOSE OF DATA GATHERING INSTRUMENT AND SELF


ASSESSMENT CHECKLIST
Trainer:
Before we commence our training session,
Please answer this data gathering instrument and self assessment check
This will help me know about your information, your learning style and
Characteristics.
This will guide me in making adjustments for the learning materials that best
suit your learning style.
The self assessment check will help me to determine your current
competencies and identify your training gaps.

PURPOSE OF SELF ASSESSMENT CHECKLIST

have a prior learning experience in UC1 which is Carry out minor
vehicle maintenance and servicing
To confirm this, let’s proceed to the Institutional assessment area,

We will now be testing for your underpinning knowledge and skills in UC1
Carry out minor vehicle maintenance and servicing through an oral
questioning and a practical demonstration.

Thank you. Let’s now proceed to the practical work area for your
demonstration.

Okay well done. Based on the corroboration of all the evidences I gathered, I
can confirm that you are already competent in UC1 Carry out minor vehicle
maintenance and servicing.
I’m going to put a check beside your name in the Achievement chart and
Progress chart,
I will also put your name in the Training Activity matrix for the next Unit of
Competency which is UC2 Drive light vehicle

4 Operating procedures of Light vehicle.
After Reading the Information sheets, answer Self-Check 2.2-4 and compare
your answers to the Answer key 2.2-4.

That’s great! Let’s now proceed to the Practical Work Area
So here in the Practical Work Area, we are going to perform the Operating
procedures of Light vehicle. The step by step instruction can be found on the
Job sheet 2.2-4 then after your assess your performance by comparing in the
on Criteria Checklist 2.2-4
Trainer: I will first Demonstrate to you on how to operate the light vehicle.
Please keenly observe as you perform a return demonstration after.
Please use the job sheet as your guide while performing the return
demonstration.
But before that, I would like to remind you that you must always observe
safety and follow the safety protocols during your return demonstration.

Trainer: that is the steps on how to demonstrate properly and accurately in


operating light vehicle.

Trainer: Congratulations, now you are done with Activity Required, I will
check it on the achievement chart.
I have noticed that you have done all the tasks in the UC2 which is Drive
light vehicle.
Can you confirm if you are ready to undergo the Institutional assessment for
your UC2 Drive light vehicle?
Trainee: “Sir, I am ready to undergo the Institutional Assessment on UC2
Drive light vehicle.”
Trainer: Well done, let’s go to Institutional assessment area
Our assessment consists of 3 methods.
We have the written test good for 1 hour,
The Actual demonstration test which is good for 1 hour
And the oral questioning for 30 minutes
The purpose of these tests is to measure your underpinning
knowledge and skills in the critical aspects of UC2 Drive light
vehicle.
You are to answer this written test consisting of 30 items with multiple
choice, true or false and fill in the blanks questions for 1 hour. The passing
score is 70% which is 21/30 items. Any questions before we begin?

Let us now proceed to the Practical work are for the actual demonstration
Trainer: So here are the specific instructions, please follow the procedures as
I explain to you.

Given the necessary materials, tools and equipment perform the


following job with in 1 hour (operate light vehicle)

● You have the SPECIFIC INSTRUCTION serve as your guide and I have
here the performance criteria checklist to assess your performance.

● Trainer: I will observe and record your performance so your timer


starts now, (Observe and monitor)

Trainer: Maam I’m done

● Trainer: Now let us return to the institutional assessment area for the
oral questioning and feedbacking.

Congratulations on your written test result. You got 25 correct answers.
MS. ____________________ I notice that you are able to answer all the questions
in the oral questioning and you were able to perform well during your
demonstration on drive light vehicle. I have to point out that you did not use
your signal lights when turning, always remember to use them when turning
since it informs other where you will turn. To improve on this I advice to take
a pause before you turn so your will remember to use your signal lights.
Nevertheless, you are able to perform very well on all the tests methods.
And based on the corroboration of all the evidence I have gathered, I can
confidently say that you are Competent in UC2 Drive light vehicle.

Trainer: Thus, I will give you this certificate of Achievement as indication that
you’ve passed the levels in this unit of competency. Good job!

Trainer: This time, let me check your progress in the Progress chart.

Trainer: Again, congratulations Ms. __________, for completing 218 hours of


training in Driving NC II.
As of now, you are already competent in all Units of Competencies in Driving
NC II. You are now qualified for the National assessment.
To apply for the National assessment, please go to the nearest assessment
center and apply for the national competency assessment in Driving NC II
Please bring 2pcs. Passport Size (White Background with Collar), please fill-
up the Application Form and also prepare payment for the Assessment Fees.
For Driving NC II bring 1,034 pesos.
Once again congratulations, trainee!
Before you go I have here the Evaluation Form, please fill up the evaluation
form based on your honest evaluation. The purpose of this evaluation form is
for me to improve my training and facilities, resources for areas of
improvement in the future trainings.
