AFFIDAVIT

I, Mohammed Rafi, aged 20 years, son of Abdul Rahman, residing at House No. 12, ABC Street,
XYZ Nagar, Kochi, Kerala - 682001, do hereby solemnly affirm and declare as under:

1. That I have been appointed / I am in the process of being onboarded as a Retailer / Agent /
Franchisee for Reliance Jio Infocomm Limited through the JioPOS Lite / JioPOS Plus platform.

2. That I am aware and understand the legal and regulatory compliance requirements under various
statutory authorities, including but not limited to the Department of Telecommunications (DoT),
Telecom Regulatory Authority of India (TRAI), and Goods and Services Tax (GST) laws.

3. That currently, I do not possess certain statutory documents such as:


- Permanent Account Number (PAN)
- Goods and Services Tax (GST) registration
- Business Registration License (if applicable)
- Any other local business permits (if applicable)

4. That I undertake and confirm that I will apply for and submit all such required documents as and
when they are available or as requested by Reliance Jio Infocomm Ltd.

5. That I hereby indemnify and keep indemnified Reliance Jio Infocomm Limited and its officers from
any penalties, legal consequences, or losses arising due to the non-availability of the
above-mentioned documents at the time of onboarding.

6. That this affidavit is being made voluntarily and without any pressure, for the purpose of
compliance with the onboarding requirements of Reliance Jio Infocomm Limited.

VERIFICATION

I, the deponent above-named, do hereby verify that the contents of this affidavit are true and correct
to the best of my knowledge and belief. No part of it is false and nothing material has been
concealed therefrom.
