DOCUMENTARY REQUIREMENTS
 Application Letter and/or Recommendation Letter with Conforme (sample form available at jbc.judiciary.gov.ph/forms/)
 Duly Notarized JBC O.R.A.S.-generated Personal Data Sheet
 Transcript of Law School Records*
 If applicable, Transcript of Post-Graduate Degree*
 If applicable, Certifications of Awards and Distinctions in college, law school, or post-graduate degree,
and Civic Awards*
 PSA or NSO-issued Birth Certificate*
 Certificate of Admission to the Bar with Bar Rating issued by the Office of the Bar Confidant
(OBC)*
 IBP National Office Certificate of Payment of Membership Dues or of Lifetime Membership**,
issued not earlier than 14 February 2025.
 Certificate of Employment (COE) issued by current employer not earlier than 14 February 2025,
providing the following: present position, employment status, annual salary, salary grade, and
judicial rank, if any (Judges and solo practitioners are not required to submit)
 Service Record (if, at any time, the applicant has been in government service; not required of
incumbent Judges)
 Certificate of Assumption of Office or Oath of Office (only applies to lawyers from non-judiciary
government offices)
 Certificate of Compliance with, or Exemption from, the Mandatory Continuing Legal Education
Program for the eighth (8th) Compliance Period*
 Income Tax Return or Certificate if Compensation Payment/Tax Withheld for years 2020, 2021,
2022, 2023, 2024***
 Sworn Statement of Assets, Liabilities and Net Worth (SALN) *** (sample form available at jbc.judiciary.gov.ph/forms/)
• All SALNs for the past five (5) years (i.e., for years 2020-2024, including entry and exit SALNs of those
who joined the government or were separated from the service at any time within the past five [5] years)
the applicant has been in government service. If in government service for less than five (5) years, only
SALNs for the years while in government service need to be submitted:

• Applicant who has not served the government within the past five (5) years:
As of December 31, 2024
 Certification issued by banks or other financial institutions of the applicant to all types of deposits
and/or investments, including insurance, within the application period
 Performance Rating for Years 2022, 2023, 2024 (only applies to applicants who served the
government at any time between 2022 and 2024 except judges)***
 Two (2) sample Decisions or Resolutions (required of Judges and quasi-judicial officers only),
Pleadings, Treatises, written Articles, Opinions, edited or published ****
 Copies of Complaints, Comments, or other Records of pending civil, criminal, or administrative cases
against the applicant, or if decided, copies of the Decisions/Resolutions ***
 Results of the following Medical Examinations conducted not earlier than 14 February 2025:
o Complete Blood Count
o Routine Urinalysis
o Blood Chemistry (FBS, BUN, Creatinine, Uric Acid, Cholesterol, and SGPT)
o Electrocardiogram (12-lead) result
 Result of Routine Chest X-ray (conducted not earlier than 14 August 2024)
 Medical Certificate issued by a government doctor not earlier than 14 February 2025 with findings or
impression on the results of the medical examinations, certifying that the applicant is medically fit to
assume the position applying for
 Affidavit of Undertaking re: Contraction of any Serious Illnesses (sample form available at jbc.judiciary.gov.ph/forms/)

 Clearance/Certification of pending and decided cases issued not earlier than 14 February 2025 by
current employer (Note: not required of Judges)
 Judicial Integrity Board (JIB) Clearance Certificate of past and pending case (if available) issued by
the JIB not earlier than 14 February 2025 (required only of previous and current employees of the
Judiciary)
 OBC’s Certificate of Good Standing issued not earlier than 14 February 2025. (Note: not required of
Judges)
 OBC’s Clearance Certificate of Past/No Past Case issued not earlier than 14 February 2025. (Note:
not required of Judges)
 Clearances/Certification issued not earlier than 14 February 2025:
o Clearance/Certification of No Pending Case issued by the IBP National Office
o Ombudsman Clearance
o National Police Clearance
 NBI Clearance (issued not earlier than 14 August 2024)
 PHILJA Letter to the applicant indicating the Pre-Judicature Program (PJP) Numerical Grade and
Descriptive Rating (52nd to 79th PJP for Trial Courts) [Note: May not be required of those subject of
valid exemption rules issued by PHILJA]

*Previous applicants who have submitted an original or certified true copy thereof no longer have to submit.
**Previous applicants who submitted an original or certified true copy of lifetime membership no longer have to submit.
***Previous applicants who submitted an original or certified true copy need only submit photocopies thereof.
****Previous applicants who have submitted these no longer have to submit.
