AFFIDAVIT OF LOSS
I, Antonio Jason Fabe, of legal age, Filipino and resident of
Santol Road, Brgy. JP Rizal, Butuan City, Philippines, after
having been duly sworn to in accordance with law, do hereby
depose and say that:
1. That I am a Government Employee of DSWD Caraga Region
assigned under Regional Program Management Office of
Pantawid Pamilya Pilipino Program, and I was issued one (1)
government-owned laptop for official use in connection with
my duties;
2. That the issuance and temporary offsite use of the said
laptop was authorized in view of my engagement in time-
sensitive official work, particularly in the preparation and
layout of government documents, which required rendering
authorized overtime beyond regular office hours;
3. That on or about 3:45 am based on the CCTV I saw regarding
the incident, while the said laptop was securely kept inside
my apartment located at Santol Road, Brgy. JP Rizal, Butuan
City was unlawfully taken by an unknown individual;
4. That prior to the incident, I had rendered overtime work
related to the preparation and layout of official documents,
as duly authorized by my immediate supervisor or office
management, and had stored the laptop inside its bag in a
concealed, non-visible location within the locked premises of
my apartment;
5. That the unknown suspect gained access by climbing over
the perimeter wall lined with barbed wire, proceeded only to
the porch area, and forcibly destroyed the front jalousie
window with screen;
6. That the laptop was not visible from outside and was
retrieved by the suspect only after blindly probing and
tapping around the floor area using a stick or similar object
through the broken window;
7. That no other items were taken and no part of the apartment
interior was accessed, indicating a deliberate and targeted
act of theft;
8. That I exercised due diligence and reasonable care in
securing the laptop and that the incident occurred despite all
precautions taken on my part;
9. That this affidavit is being executed to attest to the truth of
the foregoing facts, and to comply with documentary
requirements for any administrative and egal purpose it may
serve.
10. That incident was immediately reported to the barangay for
blotter documentation, and to the local police authorities for
investigation;
