JUDICIAL AFFIDAVIT
OF AIDA MARIA H. PEREZ, Ph.D, RPsy.
This Judicial Affidavit is being offered to prove that:
1. The witness is an EXPERT in the field of Psychology;
2. She conducted the psychological evaluation of the petitioner and
the respondent in this instant case;
3. The respondent manifests signs and symptoms of Antisocial
Personality Disorder combined with Dependent Features; and the
petitioner manifests signs and symptoms of a Compulsive Personality
Disorder;
4. The flawed personality structures of both petitioner and respondent
were shaped and rooted in their childhood experiences and in the
quality of their family environment and developed through the years
and manifested clearly during their short eight (8) years of marriage to
each other; and
5. Both petitioner and respondent are both psychologically
incapacitated to carry out their essential marital obligations and were
not aware that their flawed personality structures would eventually
cause their marriage to fail.
PRELIMINARY STATEMENT
I am AIDA MARIA H. PEREZ, Ph. D, RPsy., of legal age, Filipino citizen,
married, a resident of and with postal address at 7 A Mahinhin St. U.P.
Village, Diliman, Quezon City Philippines after having been duly sworn in
accordance with law, depose and say that:
I am a Registered Psychologist (License No. 0000236) and presently the
Executive Director of Psych Profile Center for Psychological
Assessment Services with office address at Unit 305, # 28 Margarita Bldg.,
Matalino corner Masikap Sts., Brgy. Central, Diliman, Quezon City.

(Attached herewith is witness’ Curriculum Vitae for the perusal and
appreciation of the Honorable Court marked as ANNEX “M” & series and
hereby made integral parts hereof).
I execute this Judicial Affidavit pursuant to the Judicial Affidavit Rule in
connection with the PETITION FOR DECLARATION OF NULLITY OF
MARRIAGE filed by petitioner, JAMIE RHIA R. GONZALES-MAPILE
against her husband, DION RALPH G. MAPILE with the Family Court of
the Regional Trial Court of Pasig City, Metro-Manila and the lawyer who
conducted or supervised my examination as witness was ATTY. REYNALDO
M. MOSUELA, and he conducted the examination at his office at Mosuela,
Gonzales and Associates Law Offices, Unit 1 San Juan LGU Multi-
Purpose Center, Sta. Rosa de Lima Street, Ili Sur, San Juan, La Union,
Philippines. He propounded the questions in English and Pilipino, of which I
fully understood and I answered the questions asked of me in the same
language/s fully conscious that I did so under oath and that I may face criminal
liability for false testimony or perjury.
The testimony of the witness through this Judicial Affidavit is
offered in evidence to form part of her evidence in chief including her
testimony at the witness stand to support the material allegations of
the Petitioner in her PETITION FOR DECLARATION OF NULLITY OF
MARRIAGE pursuant to Article 36 of the Family Code which she filed
against her respondent husband; to identify relevant documents on the
matter; and to testify on the same and on other related matters.

Q1 – You mentioned that you are a Registered Psychologist, where did
you finish your degree/s in Psychology?
A – I completed my bachelor’s degree, masteral degree and doctorate degree
in Psychology at the University of the Philippines in Diliman, Quezon City.
(Copies of witness’ bachelor (M-10), masteral (M-11), and doctorate (M-12)
diplomas are part of the Curriculum vitae attached, adopted, and marked as ANNEX
“M” & series, and hereby made integral parts hereof).
Q2- Do you have proof that you are a registered and licensed
Psychologist?
A - Yes, Sir.
Q3- And when were you licensed as a Psychologist?
A – Under the grandfather’s clause of Republic Act 10029 or the Psychology
Act of 2009 providing exemption of examination, I was given a license without
taking the board exam. I was registered as a licensed psychologist on October
7, 2013.
(Copies of witness’ professional license (M-8) and certificate (M-9) are parts of
the Curriculum Vitae attached herein and referred to as ANNEX “M” & series in Q&A-
1 of this Judicial Affidavit).
Q4- What was (were) your employment after you passed the Board
Examinations?
A –After getting my professional license, I set up my private clinic to offer
psychological services, while at the same time, I taught Psychology courses in
the former Kalayaan College, and was also the guidance counselor of the
same.

Q5- And where do you practice your profession?
A – My practice is based in Quezon City, but I also served and appeared as
expert witness in various courts in Pasig City, Pasay City, Makati City,
Antipolo City, Naga City, Pangasinan, Malolos and Valenzuela,
Bulacan.
Q6- And who are you clients?
A – My clients are both children and adults who need psychological evaluation
for various reasons such as school readmission, employment screening for
government employees, legal cases such as annulment, child custody, VAWC,
child abuse, and rape cases.
Q7- Are you a member of any organization past or present relative to
your profession? And what are these, if any?
A – I am a regular member of our professional organization, the Psychological
Association of the Philippines.
Q8- Do you have any other trainings to prepare you or update you in
the practice of your profession? And what are these, if any?
A - I attended webinars during the pandemic. And I attend the annual
conferences conducted by the Psychological Association of the Philippines. I
have listed in my resume the seminars, webinars and conferences I have
attended to update my knowledge in my field.

the seminars, webinars and conferences (M-2, M-3, & M-4) she attended to update
her knowledge in the field of Psychology).
Q9 – And what is the practice of Psychology all about?
A – Psychology, being the study of human behavior, has wide applications in
various fields such as education, organizations, research, legal and clinical
settings. In clinical psychology, the focus is to determine what causes a
disorder to occur, identify the problem and the maladaptive behaviors that
affect the individual’s personal, social and occupational functioning.
Q10 - Among those which you have enumerated, which activity have
you encountered the most in your practice?
A – The major activities that I encountered in my practice are the legal cases,
clinical assessment and employment screening of government personnel.
Q11- And so far, would you recall how many of that kind of activity
(Q11) have you been engaged in the course of your practice?
A – I could no longer count the number of cases I have handled since the start
of my practice in 2013, but my estimate is hundreds of cases already and more
specifically, I participated and served as an Expert Witness in 70 annulment
cases since 2012.
Q12- In relation to the practice of your profession, do you recall having
been engaged by one, JAIMIE RHIA R. GONZALES-MAPILE? And when
was that?

A – Yes. In April 2023, Mrs. Jaimie Rhia R. Gonzales-Mapile sought my services
to conduct a psychological assessment.
Q13- I will be referring to her in this examination as JAIMIE. And what
kind of service did she ask you to do for her?
A - Jaimie asked me to do a psychological assessment to determine if she or
respondent, or both of them are psychologically incapacitated to carry out
their marital obligations.
Q14- Would you know why she asked you to do that for her?
A – She sought my services to do this assessment in connection with her
petition for the nullity of her marriage.
Q15 – Can you tell us what does psychological evaluation would provide
regarding the personality of a person?
A - The psychological evaluation through the tools employed by the
psychologist will reveal certain possible dysfunctional personalities that can
affect one’s personal functioning, and the quality of his or her relationships
with others.
Q16 – How did you go about the psychological evaluation of the
respondent and the petitioner?
A - I conducted two (2) clinical interviews with the Petitioner, and conducted a
series of psychological tests. I also asked Petitioner to send me informants
who know both of them in order to validate some statements in the narrative
of Petitioner, and to provide further information as to the character of both
Petitioner and Respondent.
Q17 – Did both petitioner and respondent participate in their
psychological evaluation which you conducted?
A - _Only the Petitioner participated in the psychological evaluation I invited
the Respondent to participate in the evaluation process by sending him a
formal letter inviting him to come and undergo the same procedure as what
Petitioner went through. But there was no response from his side.
Q18- So, if only the petitioner participated in the actual psychological
evaluation, how could you then make a conclusive psychological
evaluation of the respondent? How did you go about his evaluation?
A - The narratives provided by the Petitioner are vital information because he
lived with Respondent for eight years. More information about the Respondent
was also gathered to validate the Petitioner’s description of Respondent’s
character.
Q19 - Is that allowed in the practice of your profession?
A- Gathering data in research has its limitations like unavailability of the
source of information, or refusal to participate in the research process. Thus,
conclusion can be done based on the available information gathered. There are
also guidelines stipulated in the Diagnostic and Statistical Manual of Mental
Disorder 5th edition (DSM5) that will determine if the behaviors manifested by
the person match the criteria of a personality disorder set in DSM 5.

Q20– Based on your examinations of the witnesses provided by the
petitioner, were you able to extract sufficient data on the personality
structure of the respondent?
A – Yes. The data gathered came from witnesses who knew the respondent
well and had personal encounters with him. They have observed how he
behaved in family gatherings and other special occasions with common friends
or family members.
Q21- And how would you be sure that what those witnesses have
actually described reflected respondent’s real personality structure? Do
you have any basis on this regard?
A - What they observed were all consistent and common to all the interviewed
witnesses, and as basis, their observations align consistently with how
Petitioner described Respondent’s character.
Q22– Were you able to complete the work which the petitioner
commissioned you to do? Did you make a report on your findings?
A – Yes sir. I reduced all the data I have gathered and made my conclusion in a
psychological assessment report.

adopted/marked as ANNEX “L” & series and hereby made integral parts hereof).
Q23- You mentioned that you invited the respondent to participate in
his psychological evaluation. Do you have proof of your invitation?
A - I attached a copy of the letter in my Psychological Evaluation Report
together with the original copy of the LBC receipt.

the subject LBC receipt (L-19) as part of the Psychological Evaluation Report in Q&A-
23 she made relative to her commissioned work for the petitioner).
Q24 – Briefly, what did you find out about the psychological structure
of the respondent?
A –The Respondent is found to be suffering from a psychological condition
called Antisocial Personality Disorder combined with Dependent
Personality.
Q25 – How about the petitioner, could you briefly summarize what you
found out about her psychological structure?
A – The Petitioner is found to have a Compulsive Personality Disorder. She
is expressively disciplined which manifests in her highly structured and strictly
organized life. She takes her duties and moral obligations seriously. She
constructs the world in terms of rules, regulations, time schedules and social
hierarchies.
Q26 – Can you be more specific as to the kind of personality issues or
problems that a person (like the respondent) with Antisocial
Personality Disorder with Dependent Features would manifests?
A - Signs and symptoms of an antisocial personality were seen in his being
deceitful when he was repeatedly lying to Petitioner. His flirting with women
and his cheating did not make him feel guilty or remorseful. He failed to plan
ahead when he was supposed to be partnering with Petitioner for their future

together. And he did not show any remorse with his chronic flirting and
cheating. His dependent features were seen in his being withdrawn from
adult responsibilities, being passive and displaying few functional
competencies, seeking nurturance from others in a childlike manner,
submitting himself to a stronger, nurturing figure, and needy of excessive
advice and reassurance because he sees himself as inept and inadequate.
Q27 – Can you be more specific as to the kind of personality issues or
problems that a person (like the petitioner) with Compulsive
Personality Disorder would manifests?
A - A person with compulsive personality shows a pervasive pattern of
preoccupation with orderliness, perfectionism, mental and interpersonal
control, at the expense of flexibility, openness and efficiency. In the case of
Petitioner, she is preoccupied with details, rules, lists, order, organization and
schedule as she sees herself to be disciplined, efficient and meticulous. She
shows rigid perfectionism because of overly strict standards. She has an
excessive devotion to work and productivity to the exclusion of leisure
activities since she does not want to be viewed as irresponsible. She is also
over-conscientious, scrupulous and inflexible about matters of morality, ethics,
or values. Lastly, rigidity and stubbornness as she will not back down when it
comes to moral principles and values.
Q28- From your evaluation of the respondent, do you have any
explanation why he had developed such Antisocial Personality disorder
with dependent features?
A – Respondent’s siblings have been successful in their individual careers,
while Respondent felt to be not at par with them. In my opinion, Respondent
tried to prove to his family that he is worthy of attention and admiration from
others. And he used flirting with women to get that much- needed attention
and admiration. Thus, he resorted to lying, and lack the sense of guilt or
remorse for his actions.
Q29 – How about on the part of the petitioner, do you have any
explanation why she had developed such Compulsive Personality
disorder?
A – Petitioner’s parents separated when she was age 2. Growing up, she tried
to organize her life by following rules and regulations, sticking to schedules,
and adhering to good moral values. She was well-loved by both sides of her
family, and she was endeared to them because of her thoughtfulness and
kindness. These values and beliefs she imbibed and grew up with until
adulthood. She does not want to be viewed as irresponsible. But she may have
overvalued discipline, and perfectionism.
Q30 – Are these flawed personality structures of both petitioner and
respondent serious enough to cause their marriage to fail? Why do you
say so?
A – Yes, these personality flaws are serious enough to cause marital failure
because individuals with personality disorders are inflexible and lacks control
of their impulses. It causes significant distress to both the individuals
concerned and they have difficulty carrying their relationships with others.
Q31 – So based on your findings on both petitioner’s and respondent’s
personality structure, what is your conclusion and how does it impact
on their marriage?

A – Petitioner’s and Respondent’s personality structures are incompatible and
irreconcilable since both of them showed stubborn and rigid traits. Both of
them experienced distress and cannot come up with a solution to their marital
problems.
Q32 – Do you see any chance of reconciliation between the petitioner
and the respondent? Why do you say so?
A – No Sir, their marital relationship is irreconcilable due to their personality
structures, and they have lost their love and respect for each other. Despite
Petitioner’s attempt to save their marriage by adjusting to Petitioner’s
personality, and willingness to forgive him, nothing improved in their
relationship. Respondent did not show any sense of guilt and was not
remorseful.
Q33 - Do you have other things to say?
A – No more, sir.
Q34 - Do you understand everything you stated in this Judicial
Affidavit?
A – Yes, sir.
Q35 – Do you affirm and attest as to the veracity and truthfulness of all
your statements in this Judicial Affidavit?
A – Yes, sir.
