CARD OR CERTIFICATE ALONG WITH THE REVALUATION / RETOTALING REPORT ISSUED BY THE DEPT.

7. PLEASE ATTACH A SELF – ADDRESSED COVER WITH STAMP AFFIXED.

8. APPLICATION’S WILL NOT BE RECEIVED / ACCEPTED FOR REVALUATION / RETOTALING AFTER 30 DAYS OF
