AFFIDAVIT OF LOSS

I, JIMMY CINCO REYES, Filipino, of legal age, married and with postal
address at Brgy. Ilang-Ilang, Guiguinto, Bulacan, Philippines, after being duly
sworn to in accordance with law, depose and state:

1. That I am a holder of the following Identification Cards, to wit:

a. Gaffer’s License Card duly issued Games and Amusements


Board (GAB);
b. Driver’s License Card duly issued by Land Transportation
Office;
c. SSS ID duly issued by Social Security System;

2. That on December 5, 2021 at around 5:00 o’clock in the afternoon


my wallet more or less fell off from my pocket without me noticing
it;

3. That inside my wallet are the above mention Gaffer’s License,


Driver’s License and SSS ID card ;

4. that despite diligent search and effort to locate the said wallet and
the Gaffer’s License, Driver’s License and SSS ID card inside
therein, it could no longer be found, such that I believe it is lost
beyond recovery;

5. Thus, I am executing this affidavit to attest to the truth of the


foregoing and to establish the lost of my card for my application in
lieu of the one that was lost and for whatever legal purpose it may
serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 6 th day of


December 2021 at Guiguinto, Bulacan, Philippines.
