RELEASE AND DISCHARGE

Nationality, with passport # __________________ hereby undertake, declare and agree that
I accept from Qatar Airways Company QCSC the sum of EUR600 (amount & Currency in
numbers) SIX HUNDRED EUROS (Amount & Currency in words) as full and final settlement
of all claims that I may have, howsoever manifest and of whatsoever nature, arising out of or
in connection with Case #CAS-27656-J6N9L3 related to QR088 which occurred on
24/12/2024.

And in consideration of the above payment I fully, irrevocably and forever release and
discharge Qatar Airways Company QCSC, its Directors, Officers, Employees and Agents
from and against any and all claims, actions, demands, expenses and costs (including legal
expenses and costs) now or in the near future in connection with any of the matters related
above and acknowledge that this payment constitutes a full and final settlement of all claims
as listed above, and, that me or any third party may have now or in the future and I am
bound by these terms of settlement.

Thus, I authorize Qatar Airways Company QCSC to make payment of the above to the bank
account details submitted in the web form.
