3. This declaration of conformity is issued under the sole responsibility of the manufacturer:
PROTEKT, Starorudzka 9, 93-403 Lodz, Poland
4. Object of the declaration: Full body harness with two fall arrest attachment points (front and
back). Front fall arrest attachment point by two webbing buckles to link. Work positioning
belt with two equivalent symmetrical lateral attachment points. Tested 140 kg.
5. The object of the declaration described in point 4 is in conformity with REGULATION (EU)
2016/425.
6. The PPE conforms to the requirements of EN 361:2002 EN 358:2018.
7. The notified body Apave Exploitation France SAS (n°0082), 6 Rue du Général
Audran, 92412 COURBEVOIE cedex, France performed the EU type-examination (Module
B) and issued the EU type-examination certificate: 0082/293/160/01/21/0074
8. The PPE is subject to the conformity assessment procedure module D under surveillance
of the notified body Apave Exploitation France SAS (n°0082),6 Rue du Général Audran,
92412 COURBEVOIE cedex, France.
