Directions for the Assessor:


1. Rate each NCOI according to the submitted portfolio annotations and responses during
the Behavioral Events Interview.
2. Each NCOI is assessed on an individual basis, regardless of its relationship to other
indicators.
3. Indicate the corresponding rating for each NCOI based on the rubric in the appropriate
column, with 5 as the highest and 1 as the lowest.
4. Ratings from this form will serve as basis in the deliberation of the final rating of the
Teacher II applicant.
