Job Description

Role: Substantive Consultant in Obstetrics & Gynaecology


Banding: Consultant
Division: Women’s Care Group
Responsible to: Clinical Director
Accountable to: Medical Director

Job Purpose:
This is a substantive consultant post to join our busy department of Obstetrics and
Gynaecology. The successful candidate will provide Consultant-led and delivered care in
different aspects of Obstetrics and Gynaecology including antenatal and intrapartum care and
benign gynaecological services including out-patients and theatres. The candidate must be an
experienced in Minimally Invasive Gynaecological Surgery and must hold the appropriate
qualification to support this role.The candidate is expected to have experience in working
independently in all general aspects of Obstetrics and Gynaecology. The post will attract 10
programmed activities according to the new contract including on call commitments.

Medway NHS Foundation Trust:


The Medway NHS Foundation Trust provides care to the largest urban population in Kent,
serving Gillingham, Chatham, Rainham and the City of Rochester, collectively known as the
Medway Towns. The population is about 350,000 and the Trust covers an area of 162 square
miles and incorporates Swale sector, comprising of Sittingbourne and Sheerness with a
population of 104,000. The department provides services at Medway Maritime Hospital, with
out patient clinics at Sittingbourne Memorial and Sheppey Community Hospitals. Although
predominantly an urban area, the A2, M2, M25 and M26 provide easy access to London and
the countryside of Kent and the South East Kent coastal resorts. Close proximity to the M20
and Dartford Tunnel allows easy access to the Eastern counties. The Channel Tunnel is 45
minutes drive from the hospital. There is a good selection of private, public, selective (including
grammar) and comprehensive schools and a high standard of secondary education in the area.

The Women’s Care Group:


The Women’s Care Goup is led by the Clinical Director for Women’s who is accountable to the
Care Group Medical Director.

Gynaecology
A full range of routine and emergency work is undertaken. Gynaecology Out patients Clinics are
held at Medway Maritime Hospital (Gillingham), Sheppey and Sittingbourne Community
Hospitals.

There are special clinics for colposcopy, hysteroscopy, urodynamics and infertility. The concept
of a “one stop visit” is well advanced in Cancer assessment, Colposcopy and Menstrual
Disorder Clinics. An increasing number of out patient procedures are being undertaken, e.g.
hysteroscopy, cystoscopy, transvaginal scanning, and terhapuetic procedures etc. Whenever
possible minor procedures are performed in the out patient setting, providing an ambulatory
service. Rapid Access Clinics for women suspected of having a gynaecological malignancy are
run weekly with provision of initial diagnostic services at the first visit.

All emergency gynaecology referrals from General Practitioners are assessed in an Emergency
Gynaecology Assessment Unit which is situated within the Gynaecological ward. This service is
Consultant led together with a Nurse Practitioner. Women with early pregnancy complications
are referred by their GP/Midwife to an Early Pregancy Assessment Unit situated in the out
patient clinic area during he week and on the ward at the weekend. Routine transvaginal
ultrasound scanning facilities are delivered principally by a Nurse Practitioner. We hope to
further develop this aspect of our service.

The Sunderland Day Surgery Unit


This unit has four fully equipped and staffed theatres. Last year 1,500 women underwent day
case surgery for gynaecological conditions.

Elective and Emergency Workload


The development of the Early Pregnancy Assessment clinic has reduced the number of
emergencies treated out of hours and dedicated emergency lists are provided for the treatment
of gynaecological emergencies.

Nurse Specialists
Gynaecology nurses have already been trained to take cervical smears, to insert hormone
implants and to carry out urodynamic tests. This role has been extended further to include
colposcopy where 2 nurse specialists are fully accredited. Nurses also run the early pregnancy
assessment clinic .

Medway Maritime Hospital is a unit delivering gynaecological cancer care. A close liaison has
been well established with the Cancer Centre and multi-disciplinary meetings are run at
Medway on a weekly basis with the clinical oncologist via video-link. The unit at Medway has
been commended during the recent peer review for harmonious collaboration with the Cancer
Centre at Mid Kent Oncology.

There is a dedicated full time Cancer Nurse Specialist for gynaecology working closely with the
Lead for cancer. The Cancer Nurse Specialist works from a dedicated unit here at Medway
(The Richard Watts Unit) which provides a base for all cancer nurse specialists (colorectal,
breast and urology) and the hospital palliative care team, which is consultant led.

Outpatient Maternity
This is a busy maternity unit with approximately 5,000 deliveries per year. The maternity unit
includes the Medway Fetal and Maternal Medicine Unit (MFMC), Antenatal clinics and Maternity
Care Unit (MCU) which are all situated together in the same outpatient area on the ground
floor.Consultant antenatal clinics are held Monday to Friday. The Maternity Care Unit (MCU) is
midwifery led supported by middle grade doctors with agreed protocols for the management of
common obstetric problems.

Medway Fetal and Maternal Medicine Centre (MFMC):


The MFMC is accredited by NHS England for provision of tertiary-level Fetal Medicine Services
in Kent and Medway Local Maternity System. The Fetal Medicine Unit has a unique set up
which includes all routine obstetric ultrasound scans carried out at 12, 20 and 36 weeks along
with Fetal Medicine high-risk clinics for fetal abnormalities and procedures, which are run
alongside the routine scanning lists in the same clinical setting.

The MFMC offers all specialist Fetal Medicine procedures including invasive prenatal tests such
as amniocentesis and CVS, fetocides, fetal shunting procedures and fetal blood transfusions
except endoscopic laser procedures. Women are reviewed in a specialist multidisciplinary
setting with support from specialist fetal medicine midwives. There is a specialist

multidisciplinary set up which includes clinical input from Fetal cardiologists, Clinical geneticists,
Fetal MRI specialists, paediatric surgeons and Consltant neonatologists. All cases are
discussed a weekly regional MDT meeting which includes all above specialists from different
hospitals in Kent and Medway.

Inpatient Maternity
The Delivery Suite has 16 single delivery rooms including one with a birthing pool and a four
bedded area for women in very early labour or where labour is being induced. There is fully
equipped dual obstetric theatre suite with anaesthetic and a recovery area equipped for High
Dependency monitoring. The delivery suite has 24-hour anaesthetic cover for epidurals and for
emergency obstetric surgery. There is also Consultant led anaesthetic cover for elective
Caesarean Section lists which are held 5 days per week.

There is a dedicated team of Delivery Suite Midwives with significant experience in high and low
risk intrapartum care. The senior midwife co-ordinator on duty works closely with the on call
obstetric team at all times. An assessment area and bereavement room has recently been
created within the delivery suite. There is continuous support from the neonatal department
situated immediately adjacent to the delivery suite theatre.

Community Midwifery Services


These services are well developed and integrated into the whole maternity service. Community
midwives work closely with the hospital based midwives undertaking regular shifts in Delivery
Suite. Trained paramedics of Kent Ambulance Service – including Kent Air Ambulance – attend
maternity emergency calls if required.

Neonatal Intensive and Special Care Baby Unit (Level III)


At present there are 12 designated NICU costs, appropriate equipped and staffed. There are
five Consultant Neonatologists and supporting junior staff and neonatal nurses. A joint weekly
educational meeting between Obstetric, Midwifery and Neonatal teams takes place. A monthly
perinatal mortality review meeting is held.

The unit is a recognised tertiary referral centre for in-utero transfers both from surrounding
hospitals in West Kent but also from further afield. Many of these come because of the
excellent neonatal unit at Medway. Many of those mothers also require high dependency care
from the obstetric and midwifery team for delivery and post natal care.

Key Responsibilities:
The successful candidate will provide support to the busy Obstetric and Gynaecology unit. The
on-call commitments will include Gynaecology hot week emergency cover as well as supporting
the on-call rota for delivery suite. During on-call, all other clinical committmkents are cancelled.
Private practice should cease during on-call. Extra support is available from senior consultsnrts
in complex cases. Your weekly obstetric session and gynaecology theatre sessions should
serve to maintain your skills in emergency obstetrics and emergency gynaecology.

A formal job plan will be agreed between the appointee and the Clinical lead for Gynaecology,
within 3 months commencement of the date of appointment. This will be signed off by the Chief
Executive officer and will be effective from commencement of the appointment.

For a whole time contract


Direct Clinical Care (DCC): 8.5 programmed activities (on average) per week. (includes clinical
activity, clinically related activity, predictable and unpredictable emergency work)

Supporting Professional Activities (SPA): 1.5 programmed activities (on average) per week.
(includes continuing professional development, audit, teaching and research). Consultants will
be expected to show evidence of Supporting Professional Activities at their follow up job plan
meetings.
The on-call frequency is as follows:
Obstetrics
1:20 weekdays obstetric on-call for 24 hours, gynae included from 17:30. On-site 08:30-22:00
1:20 weekend on-call for O&G resident 08:30-22:00
Gynaecology
Gynae hot week 1:17 on site 08:30-17:30
Gynae on -call weekends 1:52 on-site 08;30-12:00

Education
All Consultants are required to provide a positive learning environment for Junior Doctors both
formally and informally. All Consultants should arrange their own Continuing Medical
Education/Continuing Professional Development according to the RCOG guidelines.

Administration/Managerial Responsibilities
It is necessary to maintain waiting lists, waiting times and admissions in line with the Medway
NHS Policy. All Consultants should participate in the monthly departmental management
meetings and be responsible for Junior Doctors fulfilling their timetable commitments to ensure
a high quality service is provided.

Quality Assurance & Audit and Clinical Governance


The Trust has adopted a comprehensive Quality Assurance and Consumer Relations Policy
and all staff members are expected to play their part. The Medical Audit Advisory Committee
leads the audit programme through a department of one lead and three assistant staff. The
Trust is closely involved with National Confidential Enquiries. Audit in Surgical Specialties is
advancing rapidly with the in house development of a networked computerised database, which
provides waiting list management and summary clinical information in addition to reporting
facilities for Audit. Attendance at monthly audit meetings is required.

Annual Leave and Study Leave


Annual Leave and Study Leave is available according to national agreements. Leave must be
co-ordinated with other Consultants. Ideally no more than four senior members of staff may
take planned leave at any time. Except in unusual circumstances, 6 weeks notice of annual or
study leave must be notified to the Medical Staffing Manager. Until the existing on call rota is
reviewed Consultants may not take annual leave during their on call week unless effective
cover arrangements have been made. This will involve cancelling routine clinical work and can
only be contemplated with good notice and extensive liaison.

Private study leave is only approved in certain circumstances; full details are given in the Trust’s
Study Leave Guidelines.

A maximum of 10 days may be approved over a period of 1 years for Consultants.

Registered Health Professional

All staff who are members of a professional body must comply with standards of professional
practice/conduct. It is the post holder's responsibility to ensure they are both familiar with and
adhere to these requirements. All registered professionals must maintain registered in line with
the Trust Professional Registration with Regulatory Bodies Policy.

The Learning Resource Centre


All Trust employees have access to a comprehensive library and audio visual aids service. This
supports any training and educational programmes, self-directed learning or simply allows staff
to keep abreast of the latest research developments in any specialty. The centre not only
provides access to information in a variety of formats (including books, journals, video or other
electronic media) but also allows access to computers for database searches, access to the
Internet and the medical school web-sites. The centre’s staff can also assist and advise in the
preparation of electronic presentations slide production and the use of scanners and audio
equipment. Equipment available for use outside the Learning Resource Centre includes a digital
camera, OHP’s, multimedia projectors, laptop computers and screens.

The library is part of a Regional network and therefore has access to resources at other centres
in the South East including the BMA and the Royal College of Surgeons.

Undergraduate Teaching
The Medway Maritime Hospital is recognised by London University as an Associated University
Hospital. There is a sub-dean at the Medway Maritime Hospital, and a Medway Academic
Committee and representation through these on the Academic Board of the Medical School,
along with Guy’s Thomas’, King’s and Lewisham hospitals.

The first medical students from the new Kent and Medway Medical School will be taught on the
wards at Medway from the autumn of 2021.

Mandatory Training

All staff must complete on-going mandatory and role-specific training pertinent to their post, and
this should be confirmed with their line manager.

All post holders have a personal obligation to act to reduce healthcare associated infections
(HCAIs). They must attend mandatory training in Infection Control and be compliant with all
measures required by the Trust to reduce HCAIs. All post holders must comply with Trust
infection screening and immunisation policies as well as be familiar with the Trust’s Infection
Control Policies, including those that apply to their duties, such as Hand Hygiene Policy and Trust
procedures for use of personal protective Equipment, safe procedures for using aseptic
techniques and safe disposal of sharps.

maintain certification in ALS, ATLS and APLS/EPALS, or recognised equivalents to these courses
and provide the trust copies of these certificates.

Medical Doctors in Acute Medicine/General Internal Medicine:

He/She will be expected to follow the various guidelines and protocols issued by the trust. In the
case of cardiac arrests and critically ill patients the post holder will be expected to act as part of
the cardiac arrest/medical emergency team, following appropriate life support guidelines. Post
holders would be expected to gain within 6 months and maintain certification in ALS or recognised
equivalents to these courses and provide the trust copies of these certificates.

Anaesthetic Doctors:

He/She will be expected to follow the various guidelines and protocols issued by the trust. In the
case of cardiac arrests and critically ill patients the post holder will be expected to act as part of
the cardiac arrest/medical emergency team, following appropriate life support guidelines. Post
holders would be expected to gain within 6 months and maintain certification in ALS and if
covering on calls APLS/EPALS or recognised equivalents to these courses and provide the trust
copies of these certificates.

Paediatric Doctors:

He/She will be expected to follow the various guidelines and protocols issued by the trust. In the
case of cardiac arrests and critically ill patients the post holder will be expected to act as part of
the cardiac arrest/medical emergency team, following appropriate life support guidelines. Post
holders would be expected to gain within 6 months and maintain certification in APLS/EPALS and
where the post includes cover on the Neonatal Unit NLS/ARNI or recognised equivalents to these
courses and provide the trust copies of these certificates.

Neonatal Doctors:

He/She will be expected to follow the various guidelines and protocols issued by the trust. In the
case of cardiac arrests and critically ill patients the post holder will be expected to act as part of
the cardiac arrest/medical emergency team, following appropriate life support guidelines. Post
holders would be expected to gain within 6 months and maintain certification in NLS/ARNI or
recognised equivalents to these courses and provide the trust copies of these certificates.

Information Governance
Staff is required to keep all patient and staff information confidential unless disclosure is expressly
authorised by your employer. Misuse of or a failure to properly safeguard any data considered to
be confidential may be regarded as misconduct/gross misconduct and a disciplinary offence.

Patient Experience
Staff should ensure that they help to create a positive patient experience at all stages of a patient’s
interaction with the hospital and help to improve the patient experience within the hospital
environment.
