CONTRACT OF SERVICE
KNOW ALL MEN BY THESE PRESENTS:
This CONTRACT OF SERVICE (COS) is entered and executed this August
9, 2024 by and between:
The LOCAL GOVERNMENT UNIT OF THE MUNICIPALITY OF SAN
JOSE, represented by the Municipal Mayor Hon. Yngwie Hero B. Ecleo,
herein referred to as the “FIRST PARTY”;
and
The BASKETBALL ASSOCIATION OF THE PHILIPPINES,
represented by JOSE A. DE GUZMAN a Filipino of legal age, a Provincial
Commissioner herein referred as the “SECOND PARTY”.
WITNESSETH
WHEREAS, The Local Government Unit of the Municipality of San Jose, as
mandated by the Law believes the importance of camaraderie, teamwork
and sportsmanship in the development of the youth.
WHERAS, The Local Organization of BAP (Basketball Association of the
Philippines) has the technical knowledge and skills to officiate the game
for the Municipal Sangguniang Kabataan Cup Basketball Tournament.
NOW THEREFORE, for and in consideration of the foregoing, the both
parties agreed, as they hereby agree to undertake their respective role as
provided hereunder;
A. LOCAL GOVERNMENT UNIT OF THE MUNICIPALITY OF SAN
JOSE
1. Provided the venue for the holding of the Basketball
Tournament as well as its food’s snacks, meals and also the
illumination when games are conducted during night time.
2. Appropriate and Release funds to defray the expense of the
league, as follows;
Officiating Officials Fee: ₱
30,000.00
Total ------------------------------------------------------------₱ 30,000.00
3. Provide Technical assistance in the Operation Management of
the Municipal Sangguniang Kabataan Cup Basketball
Tournament for the year 2024.
4. Monitor the Operation of the Tournament to ensure full
implementation of the Terms and Conditions of this
Agreement.
B. LOCAL ORGANIZATION OF BASKETBALL ASSOCIATION OF
THE PHILIPPINES
1. Responsible for the conduct of the Municipal Sangguniang
Kabataan Cup Basketball Tournament and ensure its smooth
Operations starting August 11, 2024 to August 17, 2024.
2. Implement the necessary rules and regulations made by the
Local Youth Development Council Sports Committee for a fair
and just tournament.
3. Submit the necessary report after every game to be
transparent to all players and members of the committee.
This Agreement shall take effect upon its signing by the Parties and
shall remain full force until all undertakings of the AGREEMENT has been
faithfully complied with.
Done this 2ND day of August, 2024 in Municipality of San Jose,
Dinagat Islands, Philippines.
IN WITNESS HEREOF THE PARTIES AFFORESAID, have affixed
their signature on the date first above written.
