Unit IG2: Risk assessment

Declaration: By submitting this assessment (Parts 1 – 4) for marking I declare that it is entirely my own work. I understand that falsely claiming that
the work is my own is malpractice and can lead to NEBOSH imposing severe penalties (see the NEBOSH Malpractice Policy for further information).

Important note: You must refer to the document ‘Unit IG2: risk assessment – Guidance and information for learners and Learning Partners’ while
completing all parts of this assessment. Your Learning Partner should provide you with a copy, but it can also be downloaded from the relevant
resources section for this qualification on the NEBOSH website.

One of the first sugar mills in Pakistan, Zafarabad Sugar Mills Limited is a project of Zafarabad
Industries. It is a Public Limited Company. They allowed the company to operate crushing line-I,
which has a reported capacity of 5,500 TCD, as well as crushing line-II, which has a stated
capacity of 7,000 TCD. Purchasing the sugar cane, sweetening the juice, clarifying it with
grinding, ultrafiltration, centrifugation, crystallisation, packaging, and ultimately selling the finished
General description of the organisation product on the market were the main tasks. Reed cutters, carbonation machinery, a sugar
centrifuge, and application machinery all constituted heavy machinery. Workers involved in high-
risk activities such as operating heavy equipment, working at heights, and working in noisy
surroundings throughout the production process. The site's workforce was divided into three
shifts, each lasting eight hours, i.e., the first shift is from 8am to 4pm, the second shift is from 4pm
to 12am, and the third shift is from 12am to 8am.
Description of the area to be included in the The administrative area, refinery area, sugar pulp area, processing area, production area, storage
risk assessment area, waste management area, and parking area will all be included in the risk assessment.

managers, engineers, and operators who were working there at the time. All of the engineers and
plant operators had a great deal of experience, were highly competent, and had a solid
awareness of workplace risks, their current control mechanisms, their deficiencies, and the risk
associated with those hazards. They were a huge help to me in completing my risk assessment
project. I also used the "Task analysis approach" to review the history of all incidents, near
misses, absences, and sick leaves during the preceding six months.
After the inspection, in order to help me identify the necessary control measures, I reviewed the
notes I had taken during the workplace inspection as well as the information sources I had
already accessed from the various sites mentioned above. After carefully analysing the control
measures, I also looked at a handful of the HSE's authorised codes of practise or guidance
documents to suggest additional control measures.

Moral, general legal and financial arguments for ALL actions

Moral, general legal and financial Moral: For the moral benefit of its workers, Zafarabad Industries Ltd. guarantees to provide a safe
arguments workplace from where they may return in the same condition as when they arrived. The company's
reputation and moral position in the industry depend on it. One of the duties of the business is to
make its employees aware of any risks or hazards involved in carrying out job activities. Society
anticipates businesses to take all essential safety precautions and provide all required personal
protective equipment to their employees. Employees will suffer physical and mental injury if H&S
safety regulations are not followed, which will affect employee morale and reflect poorly on the
business with consumers.
Legal: Zafarabad Industries Ltd. must comply to the Safety and Health Convention and
Recommendation of the International Labour Organization (ILO) in order to safeguard its
employees (C155 and R164). Legal consequences for the organization include the possibility of a
prohibition, which would force it to stop all work on a particular project or until adjustments are
completed within a predetermined period of time. The organisation would likely experience severe
financial difficulty if these efforts were to cease. Employees who suffer injury as a consequence of
employer carelessness may bring legal lawsuits against the organization. These allegations could
be dragged up to years after the individual has left their position. Compensation paid in civil claims
is probably going to be a sizable amount. Legal fees would also be quite expensive, including the
cost of hiring lawyers and court charges.
Financial: If the organization doesn't follow the law, it will face direct and indirect financial
implications. If a workplace accident happens, the employer is liable. Employee medical expenses,
sick pay, family compensation, legal fees, and Claimants. It will cost the organization in lost time,
injury, and loss Working hours, expensive insurance rates, training expenses, recruiting costs.

Broken equipment, raw material loss, decreased production, rapid turnover, poor market value,
investigation and Cost of inspections, etc. Legal enforcement and its costs. Until specific dates, the
company may not be allowed to operate. If standards aren't satisfied, offenders might go to jail. The
organisation will lose money if it neglects.

Justification for action 1

Fire-resistant sheets must be used to separate the welding area. (Hazard category: fire)
Action (Taken from column 4 of risk
assessment)

Specific legal arguments There are some requirements of C167 - Safety and Health in Construction Convention, 1988
(No. 167) article 26 required that
“The employer shall take all appropriate measures to avoid the risk of fire, combat quickly and
efficiently any outbreak of fire, bring about a quick and safe evacuation of persons.” Company
should organise training program for the workers about hazards of fire arrangement so that they
can be used to of fire related equipment’s during emergency situations.”
R175 - Safety and Health in Construction Recommendation, 1988 (No. 175) also required that
“Every employer has the responsibility to take all necessary preventive measures for protecting
workers from fire. Arrangements should be established to protect workers from fire hazards so that
any major accident could not occur at workplace.”
Consideration of likelihood AND severity Because welding is done close to a storage area for flammable liquids, welding splashes may come
into contact with combustible material and start a fire, the likelihood is higher. This might result in
 types of injury or ill health
significant injuries such skin burns, organ burns, and disability from burning of a bodily part. Injury
 number of workers at risk
prevention is not successful with the existing control measures.
 how often the activity is carried
out Severity is categorised as:
 how widespread the risk is Minimal: No injury or damage happened.
Minor: Injury happened requiring the first aid treatment.
Major: Injury happened requiring the hospital treatment, requiring recovery time.
Fatal: Injury which causing disability or death, and loss of equipment, machinery or building
Because a worker's injury will require medical care, such as skin burns or organ burns, the severity
level for this hazard is placed at Major.

Learner number: Learner name: Page 14 of 18

Skin burns, organ burns, smoke inhalation suffocation, incapacity, and other types of injuries or
illnesses are all connected with this risk.
There were 10 at-risk employees there.
5 to 6 hours a week are spent on the activity.
Due to the possibility that welding splashes might come into contact with surrounding flammable
liquid material and ignite a fire, there is a general risk connected with the operation. Because there
was no physical barrier between them, it will thus inflict damage to both people and all departments
of the company, allowing the fire to spread across the whole building.
How effective the action is likely to be in
By enclosing the welding area with fire-resistant sheets, the risk of a workplace fire can be
controlling the risk. This should include:
decreased. The organisation will therefore see a big boost. There will be less likelihood of
 the intended impact of the action; significant harm since splashes from isolation welding won't be able to come into contact with
 justification for the timescale that explosive material afterwards.
you indicated in your risk
I’ve given this activity a two-week deadline since the area manager will have to approve it and the
assessment; and
finance manager will need to set aside money for it.
 whether you think the action will
fully control the risk The step will reduce the risk of fire by 85%, as separating the welding area with fire-resistant sheets
may do.

Justification for action 2

Provide a soundproof area where employees may relax. (Hazard category: noise)
Action (Taken from column 4 of risk
assessment)

Specific legal arguments C148 - Working Environment (Air Pollution, Noise and Vibration) Convention, 1977 (No. 148)
article 14 required that An employer who carries out work which is liable to expose any employees
to noise at or above a lower exposure action value shall make a suitable and sufficient assessment
of the risk from that noise to the health and safety of those employees, and the risk assessment
shall identify the measures which need to be taken to meet the requirements of these Regulations
and also required that; the working environment should, to the extent practicable, be kept free of
any hazard caused by noise
Consideration of likelihood AND severity The refinery region created significant noise levels due to the usage of refiners, machinery, and
generators, and workers nearby did not use ear protection, which raises the risk of hearing
 types of injury or ill health
problems. As a result, the likelihood of the risk is high. It's possible to experience temporary or

Learner number: Learner name: Page 15 of 18

 number of workers at risk permanent hearing loss at work. Any worker may get tinnitus, which might sound like ringing,
 how often the activity is carried whistling, buzzing, or humming in the ears. To protect workers from this risk, the present regulatory
out methods fall short.
 how widespread the risk is

Because working continually in a workplace with high noise exposure levels might cause
permanent hearing loss and because it is possible that employees' health will require medical care,
the severity of this hazard is Major (following justification 1).

There were sixteen employees there who were in risk.

Since the action is performed on a regular basis, it is a routine activity.
Because employees who were exposed to high frequency noise from machines and generators
without using earplugs experienced severe health issues, there is a significant risk linked with this
hazard. Along with the immediate area where the activity is occurring, surrounding employees who
work in other departments could also be impacted.
How effective the action is likely to be in
Acoustic insulation helps reduce noise exposure by building soundproof enclosures around
controlling the risk. This should include:
equipment and generators. As a result, noise may enter a building more difficultly. There is less of a
 the intended impact of the action; likelihood of long-term hearing loss, psychological distress, and harm to hearing capacity when
 justification for the timescale that sound monitoring equipment have been placed. There will be silence throughout work hours.
you indicated in your risk
In order to execute this action by building the sound proofs, I have given a timetable of two months.
assessment; and
The plant manager will authorise the action and the finance manager will allot the funds.
 whether you think the action will
fully control the risk The level of noise exposure can be reduced by 80% by placing an acoustic shield or soundproof
enclosure around the equipment and generators. This will lessen the hazard's intensity as well.

Justification for action 3

To lessen the risk posed by this hazard, provide the cutter with an adjustable guard. (work

assessment)

Specific legal arguments The International Labour Organization (ILO) has issued a suggestion to reduce the risk of accidents
using work equipment and machinery. Article 6 of the C119 - Convention on the Safety of
Equipment, 1963 (No. 119), states that hazardous sections of machinery must be fully protected

Learner number: Learner name: Page 16 of 18

throughout operation, and that guards must be in the proper position and appropriate for the
machinery.
R118 - Recommendation on the safekeeping of machinery, 1963 (No. 118) also requires that
plant, machinery, and equipment, including hand and electric tools, be of good design and
construction, taking into account, as far as possible, ergonomic principles, maintained in good
working order, and used only for the work for which they were designed, unless a competent
person has assessed the use outside the original design and concluded that the said use is
acceptable.
Consideration of likelihood AND severity Because the workers had no protection for the cutter blades when cutting sugar cane on a
machine, there is a high likelihood of catastrophic injury. They don't have any PPE on. Due of his
 types of injury or ill health
loose clothing and lack of a glove, this risky movement might entangle their hand or material.
 number of workers at risk
Significant consequences will result from this, including the removal of bodily parts, injuries, cuts,

out
 how widespread the risk is The employees will need adequate medical care for injuries such cuts, bruises, fractures, wounds,
loss of body parts, and possibly permanent hand handicap, making this risk of major severity
(following justification 1).
There were 15 employees there who were in risk.
Since the action is performed on a regular basis, it is a routine activity.
Due to exposed cutting machine components and blades that might cause serious injuries, this
hazard has a very high risk. However, only the employees of the specific location where the activity
is carried out are at risk from this hazard.
How effective the action is likely to be in
The risk posed by this hazard can be decreased by mounting an adjustable safety guard on the
controlling the risk. This should include:
cutting machine. If the blade were shielded, this risk of harm would be reduced. Physical barriers
 the intended impact of the action; called safety guards limit or forbid entry to the hazardous region and keep people away. As a result,
