What to include with your application

Below is a list of checks you need to have completed within this form and tick boxes you will
need to complete prior to submission.

Please note we need one sentence that will describe your project. This sentence needs to be a

If any aspect of your project involves undertaking physical improvements in an indoor or outdoor
space, we will need a Project Consent Form to be completed – this is available to download on
Page 3 at Section 16 of the application form - Page 2

In this section of the application form we’re asking you to read some important information about

FROM YOU IN THE CONTACTS SECTION ON PAGE 2 (ABOUT YOUR

In order for us to complete our due diligence, please provide the following information to us, If
your organisation is NOT a registered charity or a registered company, OR if you are NOT a
public sector organisation. We will require the following from you:

1) Home Addresses for both the Main and Alternate contacts. Please also note, you must provide

information from you with this application form.

If your organisation is a registered charity or a registered company, and/or if you are a public sector
organisation, we DO NOT require this information from you. You may provide home addresses if you
wish to do so but it is not a requirement and you may provide your organisation office address for both
contacts.

FORMS OF IDENTIFICATION GUIDANCE

You cannot use one form of identification for both name and address. For example, if you provide your
driving licence as proof of your name you must provide another form of identification for your address,
such as a utility bill.

Proof of Identity - please upload a document, such as your Current signed passport; or a Current UK or
EEA photocard driving licence.

Proof of Address - please upload a document, such as a Utility bill (gas, electric - issued within the last
three months, UK water bill; Local authority council tax bill for the current council tax year; Tenancy
agreement from a housing association or a council; Current UK driving licence (but only if not used for
the name evidence).

You can take a picture of your document with your mobile phone or you can scan your document
and then upload to the attachment fields.

The Avenues is starting its very own dance squad and we need your support. Vote for us to help fund the
troupe, providing expert choreography for a fun way to stay fit and active.

We are looking for funding to help establish a girl's dance troupe within the sports programme of our
Youth Club to be run onsite in our sports hall. The Avenues Youth Project is a best-in-class Youth Club
that provides fun, meaningful and inspiring after-school, weekend, and holiday activities for 8-18yr-olds.

With currently no girls-only sports activities, this programme will deliver something that we know there is
a need for, enhancing the sense of ownership that our girls feel within their Avenues experience helping
increase their self-esteem. With a growing membership of now over 800 young people (48.7% female),
and with clear benefits not only to young people's physical but also their mental health, we see additional
sports and movement capacity as an important part of our progression as a youth provider.
Having an increased sports programme will mean much to our female members. The Avenues is located
in Queen’s Park ward in Westminster, one of the UK’s 10-20% most deprived neighbourhoods. In
addition, 24% of year 6 children in Queen’s Park ward are obese, which although matching the national
average, is still a figure far too high, indicating the real need for more access to fun and engaging
physical activity for our young people. And with all our services free to access, we see ourselves as a
vital local resource, embedded at the heart of our community.

We have a well-established Sporty Saturday programme running from midday to 4 pm that includes
Judo, multi-sport and boys’ football. However, something just for the girls is conspicuous in its absence.
Following consultation with our female members, dance was the preferred addition. Existing at that
intersection of art and sport, dance tests the body in unique ways requiring stamina, strength, flexibility,
and perseverance and taps into our members' love of music.

We have approximately 400 female members, many of whom love dance. Dance was the preferred
choice for girls-only sports activity. Our mixed dance sessions of Friday evening for our seniors (13-
18yrs) regularly have 10-15 participants. We would open this dance troupe to all our female members (8-
18yrs) and would expect to easily double existing dance attendance. Through outreach and via our
existing contacts in surrounding schools and through social media we will make our community aware of
this new activity and expect it to be well attended with certain members attending every week and a
natural churn of other members attendance. We also are located in a ward which has the highest number
of teenagers in the borough with 25% of the population under 18, so we expect demand to be high.

Who we are
Groundwork UK is the data controller and contract holder (ICO registration number Z6601182)
responsible for personal data about Tesco Community Grants applicants and approved grantees. We do
not trade personal data for commercial purposes and will only process your data if required by law,
necessary to administer your grant, or with your consent. Groundwork UK uses GIFTS grant
management system to store your personal data in order for us to administer your grant. GIFTS data is
hosted on Microsoft Azure servers within the EU.

How we will use your personal data


Personal data is information which relates to, or could be used to identify you or another person. Data
protection legislation does not relate to non-personal information – for example, about an organisation or
your project. We will process the personal data you provide for the purpose of administering your grant
application (assessing your application, grant due diligence, processing grant payments grant variations,
grant monitoring, end of grant reporting) on the basis of a contract (the Grant Agreement) between
yourself and Groundwork UK.

The personal information we will hold will be your name, contact details and payment information. If you
are using an accountable body to process your grant funds, we will hold the organisation information and
organisation payment information.

Where you choose to receive email marketing about Groundwork UK’s other activities, we will process
your contact information out on the basis of your consent for us to do so.

We may share the personal data that you provide with Enablers who work for Groundwork Trusts in order
for them to provide support with this or any future application to Tesco Community Grants.

To protect our legitimate interests, we may also conduct checks against records held by other
government departments, and other agencies for the purposes of assessing your eligibility for a grant,
preventing fraud, or for the purposes of the prevention or detection of crime.

All information will be shared with Tesco (the funding body).

We need to keep the details of financial transactions for 7 years, in the event of a tax or banking enquiry.

Identity Verification Checks - Data that is sent to us for ID verification checks for proof of identity and
proof of address will be kept securely in our grant management system. If your application is successful
and you receive funding from Tesco Community Grants, we will keep this data until we receive a Project
Completion Report from you. Upon receiving a completed report, we will delete your data from our
system. If your application is unsuccessful, we will delete your data soon after informing you on your
application decision. We may at any time get in touch with the main contact to request additional
verification information if required.
