Symptom or Diagnosis
Acute Cholecystitis Acute Otitis Media
Epidemiology  A syndrome of right upper quadrant pain, fever,  Peak incidence occurs between six and twelve months of life

Diagnostic Tests  CBC, serum lipase and amylase, electrolytes, Pneumatic otoscopy (94% sensitive and 90% specific). (1)
ALT, AST, bilirubin, calcium, and albumin. (6) Tympanometry (70% to 94% sensitive and 90% specific). (1)
 Ultrasonography (6) Tympanocentesis (diagnostic standard) for identifying the causative
 HIDA scan (6) bacteria. (1)
 MRCP (6)
 Abdominal CT (6)
Typical Laparoscopic cholecystectomy. (3)  Observation for 48 to 72 hours without antibiotics in lower-risk
Treatment If complicated, may be open cholecystectomy. (3) children. (1)
Poor surgical candidate – percutaneous  Antibiotic therapy:
cholecystostomy or low-fat, low-spice diet. (3) o A 10-day course of antibiotics for age <2 years olds or with
severe symptoms. (1)
o A 5 to 7-day course for age >2 years or older and with less
severe symptoms. (1)

o High-dose amoxicillin (80 to 90 mg per kg per day in two
divided doses) is a first-line therapy. (1)
o For penicillin or cephalosporin allergy risk, use Cefdinir or
azithromycin (Zithromax) as the first-line antibiotic. (1)
