PERSON
We, PRUDENCIO S. CANDELARIO and ARTHURO VIERNES, both of
legal age, Filipinos, and both resident of Barangay Caliat, Quezon, Nueva Vizcaya, after
having been sworn in accordance with law do hereby depose and state that:
1. Firstly, we both personally know JONAH HAZLYN C. SORIANO whose
maiden name before her marriage, is JONAH HAZLYN PACAY CACAL,
because we used to live in the same locality in Barangay Caliat, Quezon, Nueva
Vizcaya, where JONAH HAZLYN PACAY CACAL was born, for a long time
and her family has been our close family friend.
2. We also know and we hereby declare that the names appearing in the
Extrajudicial Judicial Settlement of Estate with Deed of Absolute Sale dated May
10, 2024, as JONAH HAZLYN P. CACAL, in her National ID as JONAH
HAZLYN C. SORIANO and her given name per authenticated Certificate of Live
Birth as RAFUNSEL PACAY CACAL pertains to one and the same person.
3. Secondly, we also know the person ARNEL JAY CACAL, who was born on
February 2, 1971 at Caliat, Quezon, Nueva Vizcaya.
4. However, in his National Identification issued by the Philippine Government, it
was erroneously place therein as Date of Birth “February 3, 1999” instead of his
true and correct Date of Birth “February 2, 1971” as appearing in his PSA
Authenticated Certificate of Live Birth. The error may have been caused by the
inadvertence of the person who prepared his National ID.
5. Lastly, we also know the person JEANELYN CACAL DERIJE, a resident of
Barangay Caliat, Quezon, Nueva Vizcaya, who died sometime in the year 1991 at
Tondo Manila.
6. We knew for a fact that the said person has been dead, since we have witnessed
her wake at their house at Barangay Caliat, Quezon, Nueva Vizcaya.
7. We are not related in any way, either by consanguinity or affinity, to the said
abovementioned persons.
8. As such, we are executing this Joint Affidavit to attest to the truth of the foregoing
facts and for any legal purposes that this affidavit may serve.
