the request to change the name of the voting center in Barangay Laiya-Iabao, San
Juan, Batangas, from Laiya Elementary School to Laiya lbabao Elementary
School:

center, ensuring accuracy and consistency in all electoral records and
communications.

The undersigned, serving as the Acting Election Officer for San Juan,
Batangas, hereby provide a written explanation regarding the recent
discovery of a different name associated with the voting center at Laiya
Ibabao Elementary School of Barangay Laiya-Ibabao, San ]uan, Batangas,
and the subsequent actions taken to correct this discrepancy.

On April 79,2023, the name of the institution previously known as

Laiya Elementary School was officially changed to Laiya Ibabao Elementary
School. This change was made to accurately reflect the correct designation
of the school. However, it has come to our attention that this change was not
immediately updated in the official records of the voting center.

The Office of the Election Officer only received the Confirmation of

Renaming of the Public School after a formal request was made. This request
was prompted by his observation that the name recorded in the electoral
documents did not match the updated name of the school as provided by
the school's administration. This discrepancy became evident during an
ocular inspection conducted at Laiya Ibabao Elementary School on June 04,
2024.

During this inspection, it was discovered that the name listed in the
Proiect of Precinct and other official electoral records was still erroneously
indicated as Laiya Elementary School. Recognizing the potential for
confusion among voters and electoral stakeholders, the undersigned took
immediate action to address this issue.

The undersigned requesting that the official records be corrected to

reflect the accurate name of the voting center as Laiya Ibabao Elementary
School. This correction is crucial to ensure clarity and accuracy in all electoral
documents and communications.

Thank you for your prompt attention to this matter and your
cooperation in updating the records accordingly to maintain the integrity of
our electoral process.

to ensure the preparedness and suitabiiity of the facilities for their
designated purposes, including but not limited to, serving as a voting center
pursuant to COMELEC Resolution No. 10884, promulgated on 25 January
2023.

evaluated during this visit.
