ABSTRACT

People nowadays want a health service in a skyrocketing form, which is rarely possible. To
get health services faster different factors act as barriers in the present context of Nepal.
Firstly, Nepal is a landlocked country with an uneven geographical condition in the
northern part of the country which is the main factor for degrading the quality and
accessibility of health services to the people. The other factors are uneasy road access, road
strikes by different groups of people, strikes or protests from the doctor or hospital
management team and many other small problems. So, keeping these problems as a top
priority, we as CSIT students united to develop an online health service that makes it easier
for doctors and patients to provide and gain health services online. To get health services
in an easy, comfortable, and convenient way we designed an application named
"SWASTHYA SATHI—Health services at your fingertips", which aims to provide health-
related services like booking appointments with hospitals and doctors for treatment, finding
the available schedule of different specialist doctors, knowing the health services of
different hospitals, instant video consultation with specialists doctor, managing patient
reports, prices of different health check-ups in different hospitals, blood bank services at
common applications, and many more services. It is a complete packaged application for
health-related services. This application helps to book appointments easily in less time,
reduce the crowd of patients in hospitals and reduce the extra transportation expenses of
patients too. Shortly, we aim to release our application on different platforms and want to
serve the patient in an increasing manner without any issues.

1 INTRODUCTION
1.1 Background
In a world that is continuously advancing through the evolution of technology and
innovation, it is only fitting that our healthcare ecosystem also evolves in parallel.
"SWASTHYA SATHI" stands as a milestone of this evolution, a project set to redefine the
shapes of healthcare delivery, making it more efficient, patient-centric, and accessible than
ever before.

Healthcare, an essential aspect of human well-being has often been characterized by


complex nature and long waiting times. Patients frequently navigate a complicated system,
struggling with appointments, medical records, and billing processes, while doctors and
hospitals also struggle with the details of managing schedules, admissions, and resource
allocations. "SWASTHYA SATHI" emerges as the pillar of hope in current healthcare
situation, promising a transformation that empowers all stakeholders; hospitals, doctors,
and patients with a digital bridge to better health services.

Imagine a healthcare services where every aspect of the process, from the moment a patient
seeks medical attention to the final bill settlement, is as simple as a few taps on a screen.
Picture a world where hospitals can effortlessly manage their resources, doctors have a
user-friendly platform to streamline their schedules and access vital patient information,
and patients are seamlessly connected to the care they need. "SWASTHYA SATHI" is not
just an application; it is a revolutionary change in how we imagine and experience
healthcare.

With a dedicated login portal, hospitals can efficiently manage their team of doctors,
admitting patients, maintaining optimal medicine stocks, and managing detailed doctor
schedules. "SWASTHYA SATHI" redefines hospital administration by placing control
back in the hands of healthcare institutions, enabling them to provide unparalleled service
to their communities.

Doctors, the core of healthcare delivery, are equally integral to this digital transformation.
Through personalized accounts, they gain the ability to organize and manage their
schedules, access patient information instantaneously, update medical records efficiently,
and even enable appointment cancellations with a few clicks.
However, the true heartbeat of "SWASTHYA SATHI" lies in the patient's experience. In
an era where convenience is most crucial factor, the patient module provides a systematic,
patient-centric ecosystem. Patients can effortlessly login or register, explore the list of
available doctors, ascertain bed availability in hospitals, schedule appointments and
consultation with their preferred specialist, and offer feedback through reviews. Moreover,
"SWASTHYA SATHI" democratizes medical records by allowing patients to access and
upload their medical histories, developing a more comprehensive and informed healthcare
experience. With digital payment options, settling bills becomes an effortless task.

1.2 Problem Statement
In the present context, the healthcare system is affected by inefficiencies and lack of digital
integration, resulting in unsatisfactory in patient experiences and challenges in hospital and
doctor management. Patients often face difficulties in accessing healthcare services,
including finding suitable doctors, booking appointments, managing medical records and
settling bills in which they need to wait in queue for hours. Hospitals and doctors struggle
with the complexities of resource management, patient admission and scheduling, leading
to reduced efficiency and an increased administrative burden.
Additionally, the paper based medical record keeping system is inaccurate and obstructs
the seamless exchange of critical patient information among healthcare providers, leading
to unsatisfactory care and delayed diagnoses.

1.3 Objectives
i. To provide hospitals with a user-friendly system to manage doctors, admit patients,
maintain medicine stocks, and schedule doctor appointments efficiently.
ii. To enable doctors to manage their schedules, access patient information, update
patient records and cancel appointments with ease.
iii. To empower patients to access healthcare services digitally including booking
appointments, reviewing doctors, purchasing medicine, accessing and uploading
medical history, and making digital payments.

1.4 Scope and Limitation

1.4.1 Scope
The project will be developed in such a way that it would benefit doctors, patients as well
as hospitals. As a web application that serve as a platform connecting hospitals, patients
and doctors, it will be easily accessible to those with a device supporting a browser and an
internet connection.

“Swasthya Sathi” as a web application will allow the hospitals to secured login to access
their dashboard. It also allows hospitals to add, manage and remove doctors from their
network. Hospitals will be able to admit patients, assign beds, and manage the patient
records. They will also be able to track and replenish the medicine stocks. Also, will be
able to create and manage doctor schedules.

Doctors as well can view and manage their appointment schedules. They will also have an
easy access to the patient’s medical history. They will also be able to cancel an
appointment.

Patients will be able to register and login to their accounts in order to use the services. They
will able to search and view available doctors based on their specialization, location of
hospital and availability. They will be able to book an appointment with their chosen
doctors and also check for bed availability of bed in any hospital. They will be able to view
and upload their medical history documents. They will be able to purchase medicine online
and pay bills digitally for services.

1.4.2 Limitation
The system has some limitations. They are:

 Since the application will be web based, internet connection will be necessary to
use the application.
 The success of the application will depend on the digital literacy of users.
 Healthcare professionals may resist adopting to new system.
 For the initial phase of implementation only local hospitals and doctors will only
be included in the system

2 METHODOLOGY
2.1 Literature Review
The healthcare services in Nepal is mostly provided by the physical presence of the patients
in the hospitals by the specialists. The trend of digital healthcare is increasing in Nepal as
well as in the world after the world wide pandemic Covid-19. The pandemic strained
Nepal’s small health system to the limit, and seeing how hard it was for people to access
even basic health services [1]. The World Health Organization recognizes that
digitalization of health care can facilitate universal health coverage and points out that
eHealth should be developed with accessibility in mind and designed to propel
inclusiveness [2]. The importance of accessible eHealth services has become particularly
evident during the COVID-19 pandemic, and it has been suggested that eHealth contributes
to reducing the spread of COVID-19 [3]. eHealth is defined as the use of information and
communication technologies to exchange information digitally between a health care
provider and a patient in order to achieve and maintain health [4].

Safe and secure digital technologies promise to transform the health sector. It provides
immense opportunities to solve some of our most pressing health problems. For example,
tele-medicine, tele-radiology and tele-dispensing can connect and provide specialized
services from health professionals to patients in remote or hard-to-reach areas; digital
technologies have the means to better collect, analyze and disseminate data at all levels, so
changes in population health can be identified and acted on; safe and secure digital
technology supported personal health records (PHR) empower the citizens to access health
services more conveniently and enable consent-based access to patient’s history to
healthcare providers to enable quality personalized care [5].

The technical solutions that provide communication between a patient and a health care
provider are often complex systems affected by legislation and regulations on issues, such
as privacy, security, identification, internet-related infrastructure, interoperability,
information sharing, accuracy, and accessibility. Despite standards and guidelines to
improve digital accessibility and legislation to regulate the use of these guidelines, eHealth
services are still perceived as inaccessible by users [6]. Therefore, it is important to
investigate how research on eHealth services has included digital accessibility in the
development and evaluation of eHealth services.

Existing system:

1. Hamro Doctor: Hamro Doctor is the first online healthcare service provider from
Nepal where patients can consult certified medical personnel and get additional
health related services along with other information related to health online. Hamro
doctor is also a virtual community where doctors and patients can engage in live
chats and discussions in order to make things more convenient. Their facilities help
patients understand their health better, make better decisions related to their health
and find the best doctors and the best cure. Patients can also keep a record of their
medical history along with their profiles, saving them from the hassle of carrying
medical files and reports here and there. Hamro doctor was founded with a vision
of making health care and medical consultation less complicated and widely
accessible. Along with online consultation they also provide services like blood
donation, online discussion and health feed [7].
2. Hospitalin: It is an Indian online medial health care service provider. Hospitalin is
a telemedicine app aimed at providing online consultation. It is designed to find
your nearby doctors in map search. Instant consultation through direct phone call
or online audio and video calling. Share prescriptions through instant messaging.
Doctors can create their own account and start consultancy online from home.
Patient can book mark their preferred doctors for future consultation. QR code is
enabled to share doctor profile so that another user can find doctor to consult they
need. Doctor will receive their consultation fee instantly just after completing
consultation to his/her personal bKash account. Online appointment is also there for
the physical chamber. Here doctor can manage patients on the go, patient can get
appointment online through this app [8].

2.2 Requirement Analysis


This web application will be designed with the series of processes starting with requirement
analysis, design, implementation, integration, testing. During requirement analysis, all the
functional and non-functional requirement are analyzed than application will be designed
and application will be developed according to the requirement. After the design process,
coding and development part will start then after integrating the different modules in the

application, there is testing of the application. If the testing is positive then application will
be implemented otherwise some maintenance is done and application come in operation.

2.3 Feasibility Analysis


The feasibility study concluded that the project will be able to be implement to success as
it is carefully planned.

i. Technical Feasibility Study


The system is technically feasible as the requirement for the development of the system is
easily accessible. The necessary hardware and software required for the development and
implementation of the system are available. The basic programming language which is
suitable for project are available and the libraries required for project will be capable of
achieving the result that we are aiming for. All the existing resources will be used for the
development and maintenance system.

ii. Operational Feasibility Study


The system will be easy to operate with the basic knowledge of computer and internet and
well-trained manpower will not necessary. User can also easily access the system as it will
be user friendly in many aspects with good User Interface (UI). This system includes all
the requirements used for sponsorship system and this system is completely operational and

can be successfully implemented and administration feel easy to use this system as it is
user-friendly.

iii. Economic Feasibility Study


The system is economically feasible and cost effective. As all the tools and resources
required are either open sources or free. After the completion of the system organization
didn’t need to deploy any new hardware and software as the required software and
hardware. The existing resource of the system can be used.

iv. Schedule
We expect the project to be complete within 15 weeks from the start of the project. The
work is divided as follows:

2.3 Methodology of the system


2.3.1 Incremental model
The incremental model is the popular software development approach that breaks a project
into smaller, manageable segments called increments. Each increment represents a part of
the overall application functionality. This project can benefit from the incremental model
due to its complexity and the need to deliver value incrementally.

At first the project starts with an initial planning phase where the project scope, objectives
and requirements are defined. The overall architecture and design of the system are
outlined. And the major functionalities and the modules of the system that can be developed
incrementally are identified. Than a priority of these functionalities is set.
Afterwards, the first increment which focuses on the core framework of the system is
developed. Basic user registration and authentication for patients and doctors are
implemented. As well as database structure of database is set.
Than, appointment management features will be developed. So that the patients will be able
to search for the doctors and book an appointment. Scheduling appointment and its
management for doctors are implemented.
Hospital management features, such as patient admissions will be developed. So, hospitals
can login and manage doctors, patients and schedules. And the medicine stock and
management module will be developed.
Patients will be able to access and upload their medical history which will enhance the
patients experience.
Secure payment gateway for digital payments will be integrated. Payment processing and
security mechanisms will be tested. Testing and quality assurance for earlier increments
will start to be tested.
Thorough testing and quality assurance of the system will be conducted for all the modules.
Any remaining bugs and issues will be addressed and preparation for the system’s
deployment is carried out.

3 EXPECTED OUTCOME

“SWASTHYA SATHI” will provide a user-friendly digital platform that empowers


patients to access healthcare services easily. Patients will be able to find doctors, book
appointments and purchase medicines conveniently from the comfort of their homes,
resulting to improved accessibility to healthcare services. Patients will benefit from a
uninterrupted healthcare journey. They can access and upload their medical history,
communicate with doctors and settle bills digitally, resulting in a more patient-friendly
experience. The appointment booking system will reduce patient wait times, allowing them
to see doctors quickly. This will contribute to enhanced healthcare quality and patient
outcomes which will contribute to faster diagnoses and treatments.

Hospitals and doctors will experience increased efficiency in managing their operations.
Their operations. The scheduling and patient management features will reduce
administrative burdens and optimize processes, allowing healthcare professionals to focus
more on patient care. By providing doctors and hospitals with easier access to patient’s
information, “SWASTHYA SATHI” will contribute to encourage healthcare quality and
patient outcomes. The platform will enable patients to leave reviews and feedback,
promoting transparency in healthcare services which will encourage healthcare providers
to maintain high standards of care

Ultimately, the expected outcome is a high level of user satisfaction among hospitals,
doctors and patients as they will experience the benefits of a modern digital healthcare
ecosystem.
