SPECIFIC GUIDELINES
Definition of Healthy Communities Program
The P/CWHS is considered a Healthy Communities Program implementer if atleast
one of the following criteria are met.
a. The P/CWHS implements at least one (1) Healthy Communities Volume 2
Playbook, provided that all the essential activities of the Playbook are included in
the work and financial plan (see template WFPs through this link:
https://bit.ly/HealthyCommunitiesTemplateWFP ) or

b. The P/CWHS implements a health promotion program not included in the menu of
Healthy Communities Volume 2 Playbooks, provided that the program meets all of
the ff criteria:
i. The program addresses a priority area of the HPFS 2030, and/or a
component of a Health Promoting Environment as enumerated in the DOH-DILG
JAO 2021-0002: Promotion and Recognition of Healthy Communities; and
ii. The P/CWHS has oversight in the implementation of the program; and
Iii. The P/CWHS ensures that the program sufficiently accomplishes all the
requirements of the Health Promotion Self-Appraisal Checklist as per DOH DM
2022-0082

In consideration of equity:

● Only last mile elementary schools (LMES) shall be eligible to receive


financial assistance to operationalize HLI-related school-level action
plans.

● Non-LMES are certified by their School Division Office (SDO) to meet


at least one (1) criterion as per DepEd Memorandum No. 059, s 2019
to be considered an LMES.

Roles and Responsibilities of the Local School Board

national directives on HLI at the local level

adoption of HLI

● Provide overall direction and oversight for the Healthy Learning

Roles and Responsibilities of the Provincial/City Health Office

● Co-lead in the overall planning, coordination, implementation, monitoring &


evaluation of HLI
● Coordinate with and support local implementers to meet relevant standards
for HLI
● Submit Work and Financial Plan and utilize funds as necessary
● Undergo and provide capacity development and training activities
necessary for successful HLI implementation
● Initiate and host regular feedback monitoring sessions with DepEd
SDOs and selected implementing schools, as well as conduct of evaluation
activities
● Provide updates and recommendations to the Province/City Health
Promotion Committee and Local School Board
Roles and Responsibilities of the DepEd Schools Division Office
● In coordination with the P/CHO, co-lead the implementation, monitoring and evaluation of HLI in the
LGU
● Issue local DepEd policies to support HLI
● Undergo and/or implement capacity development and training activities necessary for successful HLI
implementation, including quarterly or monthly monitoring sessions
● Provide logistical and other support to the selected implementing schools to implement their action plans
for HLI
● Establish or strengthen Information and Service Delivery Networks to ensure appropriate medical
assistance are available to learners
● Review and collate school-level plans for HLI and other monitoring and evaluation tools
● Send delegates and participate in intersectoral consultative meetings and other activities of Province/City
Health Promotion Committees
● At the end of the year, assist in reviewing school outputs and recognize (or award) implementing schools
that meet or surpass standards for HLI
