How to deal with some ailments in
breastfeeding women

It’s important for the pharmcist to
be aware of the treatment of some
conditions that affect lactating
mothers. Here are some examples
and their ideal drugs.
Pain & Fever Acetaminophen
(Headache,
Muscle Pain):
Dosage
Typically 500mg every 4-6
hours as needed.
Patient Counseling
Take after nursing to minimize infant
exposure. Ensure not to exceed the
recommended dose.
Cetirizine
Allergic Rhinitis
Dosage
Cetirizine 10 mg once daily
Patient Counseling
Monitor for symptom relief; avoid first-
generation antihistamines as they may
cause sedation.
Dextromethorphan
Cough and Cold:
Dosage
Usually 10-20mg every 4-8
hours.
Patient Counseling
Avoid products containing alcohol.
Drink plenty of fluids to stay hydrated.
Psyllium husk
Mild
Constipation
Dosage
Start with 1 teaspoon mixed with water or
juice and gradually increase as needed.
Patient Counseling
Drink plenty of water when taking it to
prevent further constipation.
Calcium carbonate
Heartburn (Acid
Reflux):
Dosage
Chew 2-4 tablets as needed for relief, but
not exceeding 15 tablets in 24 hours.
Patient Counseling
Avoid high doses due to the risk of high
calcium intake.
Loratadine
Allergies
(Sneezing, Itchy
Eyes): Dosage
Typically, 10mg once daily.
Patient Counseling
Take with a glass of water. It's generally
non-drowsy but monitor for any unusual
effects on the baby.
Thrush (Oral Nystatin
Candidiasis):
Dosage
Nystatin oral suspension 100,000
units/mL, 4 times a day for both mother
and baby
Patient Counseling
Try to keep the suspension in the mouth
as long as possible before spit or swallow.
Vitamin B6
Nausea and
(Pyridoxine)
Vomiting:
Dosage
25mg PO up to 3 times per day.
Patient Counseling
This is often recommended in pregnancy-
related nausea too.
Cephalexin
Mastitis:
Dosage
500 mg every 6 hours for 10-14
days
Patient Counseling
Continue breastfeeding; seek physician’s
attention if no improvement within 48-72
hours.
Hydrocortisone
Skin Rash (Minor cream
Irritations):
Dosage
Apply a thin layer to the affected
area 2-3 times a day.
Patient Counseling
Avoid using it on broken or infected skin,
and wash hands thoroughly after
application.
Postpartum Sertraline
Depression
Dosage
Sertraline 50-200 mg once daily
Patient Counseling
Advise patient to Seek support from
family and friends; attend counseling
sessions.
Amoxicillin
Urinary Tract
Infection (UTI)
Dosage
Amoxicillin 500 mg three times
daily for 3-7 days
Patient Counseling
Complete the full course of antibiotics;
increase fluid intake.
hydrocortisone
Hemorrhoids cream
Dosage
Applied to the affected area up to
4 times daily;
Patient Counseling
Maintain good hygiene; use topical
treatments after bowel movements.
