1.    OBJECTIVE
      To define proper safety measure that has to be taken while working at height
      to protect the workers and concerned persons from free fall and falling object
      hazard.
2.    SCOPE
      This procedure is applicable to all project sites.
3.    RESPONSIBILITY
3.1   Engineer (ENGR) are responsible for monitoring proper working platform,
      toe-board, guardrail, overhead protection and all legal requirements.
3.2   The Project Manager (PM) / Person-in-Charge (PIC) and shall ensure that
      proper safety precaution has been taken during working at height.
3.3   Sub-contractor is responsible for ensuring that their respective workers
      comply with all PPE requirements and use while they are working at height.
4.    PROCEDURE
4.1   Safety Measure
      4.1.1   ENGR, Supervisor and Safety Supervisor shall take the following
              measure to avoid incident while working at height:
              a)   Barricade all open side, into or through which a person may fall
              b)   Ensure proper working platform i.e. strong & good materials is
                   used for working platform, secure and adequate, not over
                   loaded, not uneven loading, etc
              c)   Use toe-board to prevent falling objects
              d)   Use continuous safety nets
              e)   Erect continuous overhead protection shelter
              f)   Regular housekeeping i.e. no accumulation of debris on the
                   working platform
              g)   Carry out operation systematically i.e. floor by floor to avoid
                   falling-object hazard
              h)   Supply right tools for right job
4.2   Proper use of PPE (point of anchorage of life line shall not be lower than
      the level of his working position, safety belt, safety helmet etc., refer to
      Personal Protective Equipment procedure).
4.3   Monitoring

       4.3.1    The WSHO / WSHC and ENGR shall monitor the proper use of PPE at
                site regularly.
4.4   Non-compliance
       4.4.1    During inspection, the Engineer (ENGR) or WSHO / WSHC shall
                advise the relevant parties to remove all unsafe acts and conditions
                where if possible. Sub-contractor      shall ensure that follow up
                actions have been carried out accordingly.
       4.4.2    In the event of any non-compliance ENGR / Foreman (FM) or
                WSHO / WSHC shall inform the PM / PIC. The PM / PIC shall take
                necessary steps i.e. impose fine / warning letter to the responsible
                parties to prevent reoccurrence.
       4.4.3    ENGR / FM or WSHO / WSHC shall monitor the performance after
                rectification work has been done.
4.5   Training
       4.5.1    The relevant WAH personnel has to undergo the MOM’s Approved
                Training Provider (ATP) WAH courses as followed:

Supervisors         WAH Course for     - To manage and direct the manner of work at

                                              safety lapses, and advise the authorised
                                              manager to revoke any PTW where

                                          -   The assessor shall endorse the application
                                              for the PTW and forward to the endorsed
                                              application to the authorised manager of the
                                              workplace
                                          -   To conduct independent checks to ensure

 4.5.2   WSHO / WSHC / ENGR / Supervisor / Safety Supervisor shall conduct in
         house training for the worker through tool box meeting or safety talks,
         safety promotion like display banner etc. During training ENGR / FM or
         WSHO / WSHC shall use simple words i.e. (Dos and Don'ts for Working at
         Heights) to make workers understand.

4.6   Permit-To-Work (PTW) system
      4.6.1   PTW flow shall be as followed:

        5.1     Life line constructed/erected shall be accordance to the relevant PE
                design. Personnel working on the building edge shall anchorage on the
                life line with lanyard of the safety harness. 100% tie off should be applied
                at all times.
        5.2     All personnel must receive adequate training or knowledge prior to
                carrying out WAH activities as per item 5.5.1. PTW must be applied and
                approved before commencement of WAH activities.
        5.3     All personnel shall also be briefed with the Fall Prevention Plan (FPP).
        5.4     All permanent or temporary opening should be covered with good and
                sound materials.
        5.5     Supervisor to ensure that all personnel to WAH are equipped with the
                necessary PPE and such PPE are of good and proper conditions.

 5.6   The Full Body Harness (FBH) should not be modify or cause any
       damage, such as using marker to draw/write, taping of any type over
       shock absorber/ painting on FBH, stickers, etc. The FBH should also
       be use within the supplier validity dates and not expired FBH.
 5.7   For accessing of vertical ladder (>3m in height), temporary vertical
       lifeline of at least 16mm thickness rope at side of ladder for
       anchorage of fall arrestor to be installed should other control
       measures are not in place. Ladder cage can be installed
       alternatively).
 5.8   MEWP operator has to undergo MOM’s Approved Training Provider
       (ATP) and has passed the course requirement before operating the
       MEW.
 5.9   For use of A-Frame ladder, buddy system should be deployed
       where the other working personnel holding and supporting onto the
       ladder. If anchorage point is absent for anchorage of safety
       harness, the lanyard should be anchor to permanent structure of
       sound and good material (I.e. I- beam). Temporary horizontal lifeline
       of at least 16mm thickness rope for anchorage of lanyard to be
       installed should other control measures are not in place.
