Certification of Loss of Animal

A farmer in Sablan, Benguet owned two goats - a seven month old female and five month old male - as an additional source of income. However, both goats died of pneumonia on August 8, 2023. The municipal agriculturist of Sablan issued a certification to confirm the death of the goats in support of the farmer's claim for agricultural insurance. Additionally, the municipal agriculturist certified that a hog farm called Marinel's Piggery located in Sablan is African Swine Fever free based on an inspection. This certification was issued to support the farm owner's loan application.

ASF FREE CERTIFICATION


This is to certify that hog farm Marinel’s Piggery located at Bayabas Sablan, Benguet is an

African Swine Fever (ASF) FREE FARM

This is to further certify that based on inspection by the government/LGU-OMAG, found no

signs of notifiable disease such as African Swine fever has been observed in the herd.

This certificate is issued upon the request of Ms. Marinel A. Basilio in support to her loan

Application.

Done this 4th day of AUGUST 2023 at Sablan, Benguet.
