WE, PATROLMAN RONEL H CAHAGNA-AN and PATROLWOMAN MONALISA
CHUA of legal age, bona fide members of PHILIPPINE NATIONAL POLICE (PNP), presently assigned at Tacloban City Police Office, Police Station 3, New Kawayan, Tacloban City, after having been duly sworn to an oath in accordance with the law, hereby dispose and says that; 1. That, On June 6, 2022 at about 8:04 in the evening elements of SDEU PS3, TCPO led by PLT ANGELO LEO S SILVESTRECE, and under direct supervision of PMAJ JOSE B. GALLANES JR, Station Commander, in coordination with PDEA8 conducted a successful BUY-BUST OPERATION at Barangay 106 Sto. Niño Tacloban City. Which resulted to the apprehension of ALLAN AROMIN y' Arigola @JUN (Street Level Individual), 29 years old, male, with live-in Partner, DOB: August 17 1992, POB: Sta. Rosa Nueva Ecija, High School level, tricycle driver, and a resident of B3 L17, St Francis Village Phase 2, Brgy. 105 Suhi San Isidro, Tacloban City. Confiscated during the operation were: Five (5) pieces heat- sealed transparent plastic sachets containing white crystalline substance a suspected “SHABU” One (1) subject of sale and Four (4) subjects of possession, with marking HBFE, HBFE-1, HBFE-2, HBFE-3, HBFE-4, and One (1) Unit SKYGO color black motorcycle with side-car (without key), without plate number (unregistered), with date, time, place and signature of poseur-buyer, with estimated street value of Eight Thousand Five Hundred Pesos (Php 8,500.00). One (1) piece subscribed Five Hundred Peso Bill with N714105. Subsequently, the marking and inventory of subject of sale and recovered pieces of evidences was done at the place of operation with the presence of the suspect, witnessed by Councilor Renier Reas, Tacloban City, and Mr. Lito Bagunas (Radioman of DYMS) (Media Representative). A complaint for violation of Section 5 & 11 Article II of R. A. 9165 is being prepared against the suspects to be filed before the City Prosecution’s Office, Bulwagan ng Katarungan, Tacloban City thru an Inquest Proceeding. PB/SO/EC: PSSg Hernan Brent F. No. 21-06-08-SC (Rules on the use of Body-Worn Camera in the execution of Warrants).
3. That, I PAT RONEL H CAHAGNA-AN informed the accused that the execution of the apprehension is being recorded through the use of Body-worn Camera.
4. Then, PAT RONEL H. CAHAGNA-AN turn-over the aforesaid recording to PMSG
FLORENTINO E MILLA JR, (Data custodian) for download on June 7, 2022 at about _______, at Tacloban City Police Office, Tactical Operation Center prior submission to court;
5. That, PMSG FLORENTINO E MILLA JR, (data custodian) downloaded the said recordings from our body-worn cameras at about _______ same date at Tactical Operations Center in the presence of the accused
