D.C.R. Gratuity Nomination Form

This document is a nomination form for death-cum-retirement gratuity. It allows an employee to nominate members of their family to receive any gratuity payments in the event of the employee's death or if the gratuity remains unpaid at the time of the employee's death after retirement. When the Officer has a family

of my family and confer on them the right to receive, to the extent
specified below, any gratuity that may be sanctioned by Government
in the event of my death while in service and the right to receive on
my death, to the extent specified below, any gratuity which having
become admissible to me on my retirement may remain unpaid on my
