POSITION DETAILS
§  POSTION TITLE
§  Indicate the title of the position if you are requesting for a regular position to be filled-up. A copy of Job
description is required.
§  Make sure you attach a Scope of Work if you are requesting for a contract employee.
§  The Job Description and Scope of Work will help HR to have an initial idea what the request is about.
§  DATE REQUISITIONED: Indicate the date when you made the request
§  DATE REQUIRED: Indicate the date when you expect hiring to be completed
§  LOCATION: Indicate the location where the employee will be assigned
§  SECTION/DEPARMENT: Indicate the Section and the Department where the request comes from.
§  JOB GRADE: Indicate the job grade (if you know the job grade).
§  NO. OF PERSONS REQUISITIONED: Indicate the number of persons you are requesting.

§  TYPE OF REQUISITION:
§  New Position: First formal placement of newly created job or re-evaluation due to significant changes.
§  Replacement Due To:
