This document is an account closure form for a savings or current account with NSDL Payments Bank Limited. The customer provides their customer identification number, account type and number, personal details like date of birth or date of incorporation, and PAN number. If applicable, the customer also provides details of any linked accounts that should be closed and consents to funds being transferred from the linked account to their NSDL account. The customer provides alternate bank account details to transfer remaining funds. Documents like PAN card, cancelled cheque or statement of alternate bank account must be attached. Account closure charges or dues will be deducted and taxes applied to closure proceeds as per bank policies.

charges. Closure proceeds of account cannot be transferred to any third-party account.
