Affinity scheme (Optical, Dental & Over 18 children)

Dear OQIC Member,

Greetings from Oman Qatar Insurance Company SAOG!

Members who wish to enroll your dependents under the affinity scheme we request you to please follow
below steps to complete the process:

Enrollment Open Date: 01-Feb-2023


Enrollment Close Date: 15-Mar-2023

Dental Coverage
Limit RO.250/- per person per year.

The scope of cover of dental benefit includes the following:

Dentist’s Consultation
Diagnostic procedures
Related Medications
Extractions
Bridge Work / Crowns, Gum and root canal Treatments
All fillings including amalgam, composite and glass ionomer fillings

Oral Surgery Co-insurance: -10% at Emerald Hospitals.


Preventive dental care including scaling, cleaning & polishing covered twice a year Dental Prostheses
and orthodontics are excluded.

Optical Coverage
Limit RO.150/- per person per year.

The Scope of cover of optical benefit includes the following: Vision tests to diagnose the following errors
of refraction:

Hyperopia
Myopia
Astigmatism
Anisometropia
Presbyopia

Plain, Progressive and crizal lenses, (restricted to one pair per member) & contact lenses for the
correction of the above-mentioned errors of refraction. Laser/Lasik treatments are covered in case the
eyesight power correction is above +3 and -3 up to optical limit

Co-insurance: 10% at Emerald Hospitals

Exclusions: The following treatments and services are excluded:

1) Strabismus
2) Ptosis
3) Laser/Lasik treatments for correction of refractive errors and cosmetic purpose
4) Spectacle Frames

Dependent Children above 18 years until 23rd Birthday

Eligible members:

• Citizen Employee: Children above 18 years until 23rd birthday.


• Expatriate Employee: from 04th child or children above 18 years old.
