accident and identified as: Frequent, Likely, Occasional, Seldom or RAC Chart
“Severity” is the outcome/degree if an incident, near miss, or accident E = Extremely High Risk

Job Hazards Controls RAC
 Burn Injury  Wearing of protective clothing, safety jackets, welding apron, gloves, etc.
 100% body protection/welding apron. Welding blanket shall be used to
prevent welding slags from falling on people working below and to avoid
fire incident. H
 Drop zone should be provided with barricade and warning signs to warn
the workers.

 Electrical shock  Check electrical equipment prior to its use. Ensure it is inspected, color
coded and properly documented for the month.
 Ensure of proper grounding. Grounding cable must be clamped near the
joint to be weld. Welding cables, electrical cords must be properly H
insulated.

 Fire Hazards  Fire extinguisher shall be located near the welding machine and on
welding operation. Good housekeeping must be observed.

to adjacent work area. H

 Tripping Hazard  All electrical cords, extension cords must be elevated and rolled-up after
use.
 All materials should be stored at designated area and remove scrap and
unnecessary material regularly.

 Head, eye, hand injury 100% hardhat with welding mask; Face shield with helmet; safety glasses
and working gloves shall be worn at all times.
