(Full Name/Rank/Fleet) Management Maritime Services INC., (OSM), a resident of __________________ contact number, ______________________ respectfully recommend Mr./Mrs. ____________________
who is my ________________, likewise of legal age, Filipino residing in ____________________
(Relationship) A graduate of ____________________________ with a degree in __________________________ for a possible employment that suits his/her qualifications with one of Order and Service Management Services Inc.’ s manned vessel.
I personally vouch for his/her good moral character, sobriety, and competence to perform his/her assigned shipboard duties subject his/her passing the On the Job Training given by OSM.
In the event, that my recommendee, fails to finish and complete his contract on-board for any reason, I will be liable to pay his/her repatriation costs as well as the transportation cost of his/her reliever, including all the costs of his//her trainings, visa, flag state costs incurred in processing his/her embarkation (International trade). The total company costs will be deducted from my salary and or will be paid directly to OSM before my recommendee will be cleared and issued a Sea Service.
Further, I am fully aware that this recommedtaion is not a guarantee that OSM will employ my recommendee and that OSM shall have the sole option and/or prerogative to accept or ignore this recommendation.
I fully understand, that my recommendee shall wait, undergo and follow the OSM Procedures in accepting my recommendee; and that my recommendee shall not be allowed to go on the same vessel with me.
In the event that I am no longer an active Seafarer of Order and Service Management Maritime Services Inc., for whatever reason, this recommendation is automatically withdrawn and terminated.
