JOINT AFFIDAVIT OF DISINTERESTED PERSONS

We, Adelaida N. Antonio and Ramon S.D. Espiritu, both of


legal age, Filipinos and residents of 144 Bayan Bayanan Prenza II,
Marilao, Bulacan, and Prenza I, Marilao, Bulacan after having been
duly sworn to in accordance with law do hereby depose and state
THAT:

1. We have known Maria Belen R. Benedicto, for at least Fifty


Five (55) years;

2. We, being neighbors for more than Fifty Five (55) years we
have known her nickname as Maribel Benedicto;

3. We know her for a fact as well to Maria Belen R. Benedicto


since the early days, but fondly called Maribel R.
Benedicto;

4. We know her to have been married to Raymundo


Benedicto, and after couple of years of marriage was
discovered that her name in the Certificate of Marriage is
Maribel R. Benedicto;

5. However, she intend to register and be a member of the


Social Security System and her Certificate of Birth together
with the Marriage Certificate are required to submit, and the
discrepancy is shown thereat;

6. The Certificate of Marriage reflects her other name Maribel


R. Benedicto, while her Certificate of Birth shows her Maria
Belen Rieza as her name. Her first name in the two (2)
documents has discrepancy. Considering that the correction
of entry in the Marriage Certificate is a long process, she
intends to use this Affidavit to attest to the truth that Maria
Belen R. Benedicto and Maribel R. Benedicto is one and
the same person;
7. We attest that Maria Belen R. Benedicto and Maribel R.
Benedicto is one and the same person;

8. This Joint Affidavit is being executed to attest to the truth of


the foregoing facts and to establish the identity of Maria
Belen R. Benedicto being the same person known as
Maribel R. Benedicto in her Certificate of Marriage.

9. This affidavit is executed for purposes of attesting the truth


of the foregoing declaration that Maria Belen R. Benedicto
being the same person known as Maribel R. Benedicto in
her Certificate of Marriage.

AFFIANTS FURTHER SAYETH NAUGHT.
