1.0 Purpose

The purpose of the Safe Work Instruction is to detail measures that reduce the safety and health
risks associated with the use of crane and lifting operation in construction site.

2.0 Scope

The scope of this instruction applies for the whole Gagasan Maya’s project site.

3.0 Definitions

 Crane
 Crane machinery such as crawler crane, mobile crane, tower crane, etc.

 Competent
 Capability of identifying existing and predictable hazards in the surroundings or working
condition which are unsanitary, hazardous, or dangerous to employees and has
authorization to take prompt corrective measures to eliminate them.

4.0 Responsibilities

4.1 Project Manager / Construction Manager


 The Project Manager / Construction Manager shall ensure full compliance of this
Safe Work Instruction and its requirement along with other legal and statutory
requirement.

4.2 Site Engineer


 The Site Engineer shall ensure the compliance of the safety and health requirement
before any commencement of work.
 The Site Engineer shall ensure all permit to work is properly filled and approved
before any commencement of work.
 The Site Engineer shall ensure that the assessment of the hazards and its control
measure has been implemented before any commencement of work.

4.3 Site Superintendent / Site Supervisor


 The Site Superintendent / Site Supervisor shall conduct Pre-Task Talk before any

6.0 Procedure
6.1 General Requirement
 A pre-task briefing based on the job safety and health analysis shall be provided for
all person assigned to lifting operations.
 Work shall not be conducted within the specified clearances without the approval
from the electrical utility provider.
 No persons shall be allowed to be under a suspended load.
 Cranes shall be maintained regularly in accordance to manufacturer’s
recommendations and the full maintenance history records shall be duly kept and
produced when required.
 All cranes shall have the following information/documents displayed prominently
on them:-
 Valid PMA Certificate issued by DOSH.
 Valid PMA number as issued by DOSH.
 Respective crane operator’s competency certificate issued by DOSH.
 SWL and Load Charts.
 All cranes shall have the following documents readily available in respective
cabin:-
 Records of independent 3rd party inspection (If applicable).
 Records of re-examination done (for crawler cranes).
 Manufacturer’s Operation and Maintenance manual.
 All crane and lifting appliance hooks shall display their SWL.

6.2 Lifting Gears Requirement


 All lifting gears shall be tested to ensure its integrity.
 All lifting gear shall display its unique identifying number.
 All lifting gear shall be marked with its SWL.
 Lifting gear that has not got a test certificate or its not marked with a unique number
or SWL shall be removed from site.
 Lifting gear that is manufactured to a national or international standard shall have
a certificate of test of fitness for use from the manufacturer.
 Site made or specifically fabricated lifting gear such as distribution beams, material
lifting cages and crane lift able waste skips shall meet the following requirements
before use.
 Option 1
 Have a design which is approved by a Professional Engineer.
 Have an examination and certificate of fitness for use issued by a
Professional Engineer.
 Option 2
 Be load tested to 5 times the SWL (Example, if SWL=1 Ton, then the
Test Load=5 Tons)
 Have the load test witnessed by a qualified mechanical engineer.

 Have a written record of test signed by the qualified mechanical


engineer that witnessed the test.
 Lifting gears that has been inspected or examined and found to be defective shall
be colour coded with red paint, not be used and removed from operational work
areas.
 Lifting gear that has been examined and found to be in good condition shall be color
coded in accordance with the details given below.

 The use of hand spliced wire ropes for lifting is prohibited.
 The practice of tying knots in wire ropes or chains used for lifting is prohibited.
 Lifting gear shall be inspected before each use by the rigger.
 It is advisable to establish and maintain a log that contains details of all lifting gears.

6.3 Lifting Work Requirement


 Lifting work areas shall be well illuminated for night time operations.
 An exclusion zone shall be created for lifting areas.
 Lifting work areas shall be free from overhead utilities.
 Mobile crane booms shall be fully lowered and locked and outriggers shall be fully
retracted prior to moving the cranes.
 No load shall be left suspended on a crane or lifting appliance unless the operator
is at the controls.
 Loads shall only be suspended for the minimum amount of time necessary to
complete the lifting operation; at all other times they shall be lowered to rest on a
solid surface.
 Working adjacent to highways or buildings, the crane boom shall be lowered down
or retracted at the end of the day, immediately prior to holidays or at weekends.
 Cranes booms that are unable to be retracted or lowered down due to space
constraints should be left in a parked position, facing away from the highway or
buildings.
 Crane and lifting appliances shall be switched off, have the doors locked and keys
removed when not in use.
 Suspended loads shall be controlled at all times by using a minimum of two (2)
taglines at opposite corners of the load.

 Loads shall not be lifted over any public area unless approval has been obtained
and control measures, such as traffic and pedestrian control, to temporarily exclude
the public from the area are in place.

6.4 Permit to Lift


 The Permit to lift will be used to check and verify the safety of lifting operations
based on the complexity of the lifting operations to be undertaken on worksite the
complex lifting is required to apply the Permit to Lift.
 Classification of the complexity of Lifting Operations is detailed below: -
 Simple Lifts – Lifts using a single crane with a Maximum Safe Working Load
capacity of less than 20 tons and a load weight of up to 5 tons excluding any
lift over or adjacent to the track area, public roads, water, power lines, on soft
ground or from temporary works.
 Intermediate Lifts – Lifts using a single crane with a Maximum Safe Working
Load capacity of less than 50 tons and a load weight over 5 tons up to a
maximum of 20 tons and any lift adjacent to the track area, public roads water,
power lines, on soft ground or from temporary works.
 Complex Lifts – Lifts using a single crane with Maximum Safe Working Load
capacity of more than 50 tons and a load weight over 20 tons, any lift using
more than 1 crane or any lift involving persons as part of the load.

6.5 Crane Erection and Dismantling


 Where cranes are to be erected and dismantled on the worksite a method statement
and HIRARC for each operation shall be provided. No onsite crane erection is
permitted without an approved method statement, HIRARC and DOSH approval.
Refer flow chart 1 for Crawler Crane.

6.6 Testing, Examination and Inspection of Crane


 Cranes will be inspected and tested by DOSH inspector at periods not exceeding 15
months. A thorough examination of each crane will be made by the DOSH inspector
at periods not exceeding 15 months and after substantial re-erection, alteration or
repair.
 Crane will be tested and thoroughly examined after exposure to weather conditions
likely to have affected its stability.
 Automatic Safe Load Indicators will be tested and thoroughly examined before the
crane is taken into use, after it has been dismantled or after anything has been done
that is likely to affect the proper operation of the indicator, e.g. a change in jib
length. They will be inspected weekly as part of the normal weekly inspection of
the crane.
 A copy of the current 3rd party test and examination report for every crane in use
on the worksite shall be kept and displayed with the crane on site.
 On a monthly basis all crane operators or other suitably trained persons will carry
out a detailed safety inspection of the crane and record the findings on the

appropriate checklist. Such records will be kept in the crane and be produced by the
crane operator upon request.
 No part of a crane, which has been subjected to lifting stresses, shall be altered,
welded or changed in any way without reference to the crane manufacturer’s
procedures.
 All crane operators or lifting supervisor shall conduct a daily safety check of their
cranes.

6.7 General Operation of Cranes


 Prior to work on the worksite all lifting operations will be authorized by the
Authorized Person. No authorization, no work. The Safety personnel will record
the authorization on the Permit to Lift.
 Where intermediate lifting operations are to be conducted the lifting operation
supervisor will ensure that such operations are properly planned. Planning
arrangements will be recorded on the Lifting Plan.
 Where complex lifting operations are to be undertaken a method statement and
HIRARC will be provide for approval. No such operations will be conducted
without an approved method statement.
 All cranes on the worksite will have a copy of the manufacturers operating manual
and load rating charts available for use and kept with the crane operator or in the
crane.
 Close attention will be paid to the stability of the surface upon which a crane is
positioned. Steel plates will be provided and used at all times (Soft Ground) under
each mobile crane outrigger. Outrigger loads or track pressures for the specific
conditions of machine operation will be obtained from the crane operating manual
before setting up the crane for work. Checks will be made to ensure that the ground
is capable of bearing the loads imposed by the crane outriggers or tracks.
 Hooks on cranes will be of an approved safe design and fitted with safety latches.
 The weight of any load to be lifted will be accurately determined prior to lifting.
 Loads will only be lifted vertically and will not be not pulled or dragged.
 Close attention will be paid to proximity hazards. Extra precautions need to be taken
when cranes are operated adjacent to public roads, above water pipelines, below
power lines, on soft ground or from temporary works.
 Where a crane has a traveling or slewing motion, a gap of at least 0.6m will be
maintained between any moving part of the crane and any fixed or stationary object.
Should this be impractical, access to the danger area will be prevented.
 Crane manufacturers may specify the maximum wind speeds in which their cranes
can safely operate and therefore, lifting operations will cease when these speeds are
reached. In certain circumstances, dependent upon the nature of the load and
prevailing conditions, it may be necessary for lifting operations to be curtailed at
wind speeds lower than those indicated by the crane manufacturer. Wind forces can
be greater at height by as much as 35% and all lifts above ground level must account
for such forces (e.g., side loads, down drafts, etc.), as applied to the load and the
boom.

 Where two or more cranes are required to work together to raise a single load, the
operation will be the subject of a Method Statement and HIRARC. Such operations
will be directly supervised by lifting supervisor. The Safe Working Load of each
crane used for multiple crane lifts will be down rated by at least 25%.
 The complexities of dual lift operations need to be clearly appreciated and all
necessary steps taken to ensure that neither crane takes more than its calculated
share of the load, that neither crane becomes out of level, and that the hoist ropes
remain plumb.
 Pre-task briefing shall be conducted by the process owner or supervisor in charge
before start work and attended by a related person on the activities.
 Communication method (flags, hand signal, walkie-talkie etc.) available at site.

6.8 Mobile Cranes


 Crane providers will be required to provide load rating charts and these will be
consulted when the load and radius has been determined to identify the maximum
safe working load. It is essential when selecting a mobile crane to ensure that the
safe working load is considered in the worst condition in which it is planned to
operate.
 Before a mobile crane is positioned for a lift, attention will be paid to the condition
of the ground upon which the crane will stand, as this will be subjected to high
point-loadings from outriggers or tires. Dependent upon the nature of the ground
and/or the size of the crane to be used, a special hard standing may have to be
prepared.
 Outriggers will be properly set and locked if a locking device is provided. When
controlled remotely from the cab, the operator will make a physical check to ensure
that each pad has an adequate bearing before a load is lifted. Steel plates will be
used under each outrigger pad to distribute the load. It is essential that outriggers
are supported at the jacking points and not under the outrigger beams.
 The practice of extending only those outriggers on the side of a crane where a lift
is taking place is highly dangerous and will not be permitted. Backward stability is
equally as important as forward stability. All outriggers will be fully extended at all
times whilst lifting operations are ongoing.
 Before a lift takes place, the crane will be level. If the carriage is slightly off level,
the error is magnified at the jib head, particularly when slewing, and could result in
failure when lifting an otherwise safe load.
 Trucks fitted with cranes are frequently misused and feature very highly in lifting
operation incidents. They will be closely supervised and only operated according
to the manufacturer’s instructions. They are only designed to lift loads on or off the
truck and not to be used as general cranes. Care will be taken to ensure that the
operator of a truck crane does not trap himself between the load and the truck.
 Prior to start work, Safety personnel and Crane Operator shall conduct joint
inspection and keep record. Only safe cranes shall be allowed to operate at site.
Crane operator shall conduct daily visual inspection. Safety personnel and crane
operator shall conduct weekly joint inspection and keep record.

6.9 Crawler Crane


 Crawler cranes are erected and disassembled. To ensure the safety of these
machines, it is imperative that they are assembled and disassembled by qualified
and trained personnel according to the recommendations of the manufacturers´
manuals.
 Crane operator shall conduct daily inspection. Safety personnel and crane operator
shall conduct monthly joint inspection and keep record.

6.10 Lifting Gear


 Lifting gear means any chain sling, rope sling, or similar gear, and any beam, ring,
link, hook, plate clamp, shackle, swivel or eye bolt, used for lifting operations on
the worksite.
 All lifting gear will be subjected to a test by a competent person (Rigger) prior to
its first use to prove that it is capable of withstanding the load that it is designed for.
 The lifting supervisor will ensure that all items of lifting gear in use on the worksite
are properly inspected and certified to a particular safe working load. All lifting
gear will be marked with color code with a unique means of identification.
 All lifting gear will be inspected at periods not exceeding 3 months. The results of
regular inspections of lifting gear will be recorded on the Lifting Gear Inspection
Checklist
 Riggers will check the condition of lifting gear prior to each and every lift.
 Wire ropes and fiber ropes and slings will be marked with their safe working load
unless this information can be accurately obtained from a table posted prominently
on site. All other chains, ropes and items of lifting equipment will be clearly marked
with their safe working load.
 A wire rope used in raising, lowering or suspension of a load, will not be used if it
is kinked, significantly rusted, the core is visible or the rope has visible broken
wires, exceeding 5% of the total or in any length equal to 10 times the diameter of
the rope.
 Garbage skips will not be used for lifting materials other than loose lightweight
garbage. They will be inspected on a weekly basis by riggers to ensure that the
lifting points are in good condition and that the skip body is not worn or split. Whilst
garbage is being lifted in a garbage skip it will be surrounded by a cargo net that is
attached to the crane hook. The cargo net will be of sufficient capacity to hold the
garbage skip and its contents in the event of a lifting point or skip body failure.
 Any lifting gear that is found to be defective will be removed from service and
sprayed with red paint to indicate that it is not to be used. All defective equipment
will be removed from the worksite.
 A system will be established on the worksite for storage of all lifting equipment not
regularly in use. Such equipment will not be left in the work areas.

6.11 Safety Signs and Marking


 All warning signs shall be posted in English and Bahasa Malaysia or in any other
suitable languages.
 Boom length, size and safe working load capacities at the appropriate radius and
recommended operating conditions shall be identified on an instructions plaque
which shall be permanently affixed to the cab of the crane in a location readily
visible to the operator while seated at the control station.

reduction less than 7%, breakage of strands less than 10% no
kinks, corrosion etc.
Periksa penyangkuk keselamatan dan pin pengadang
