AFFIDAVIT OF LOSS

I, SHERIFA A. TABARES, of legal age, Filipino, married, and a


resident of Poblacion, Margosatubig, Zamboanga del Sur after having been
duly sworn to in accordance with law, do hereby depose and state THAT:

1. After opening an account with 1st Valley Bank, I was issued a


Passbook under my name.

2. Recently, I had tried to look for the said Passbook issued by 1 st


Valley Bank but the same could not be found.

3. Despite diligent search and effort to locate the said Passbook, I


could not find the same such that I believe that it is now lost beyond
recovery.

4. As such, I am executing this affidavit to attest the foregoing facts, to


support the application for a new Passbook, and for whatever legal
purpose this may serve me best.

IN WITNESS WHEREOF, I have hereunto set my hand this 4


November 2022, in San Miguel, Zamboanga del Sur, Philippines.
