TITLE: PEUTZ JEGHERS SYNDROME PRESENTING WITH ACUTE INTESTINAL
OBSTRUCTION: A RARE CASE REPORT.
Aims:
To report a rare case of Peutz Jeghers syndrome (PJS) in a 16-year-old boy presenting with acute
intestinal obstruction.
Case report:
A 16 years old boy presented with colicky abdominal pain in the peri-umbilical region with
bilious vomiting and constipation of one day duration. Physical examination revealed multiple
black colored pigmentations over buccal mucosa and both the lips since childhood. Abdominal
examination revealed tenderness in the umbilical and hypogastric regions associated with
palpable distended bowel loops in hypogastric region with diminished bowel sounds. Radiograph
showed dilated bowel loops with air fluid levels suggestive of acute intestinal obstruction.
Ultrasound abdomen showed target sign with b owel in bowel with an echogenic portion
revealing mesentery. Lab investigations revealed anemia with Hb of 7.6gm% with polymorpho
leukocytosis (N-84%) with routine biochemical parameters were un remarkable. Patient was
adequately hydrated with IV fluids and two units of blood transfusion preoperatively. Patient
underwent Emergency exploratory laparotomy. Ileo-ileal intussusception was noted about 50 cms
from ileo caecal junction, which was reduced. A large polyp was seen in the lumen as cause of
intussusception. Small bowel segmental resection with end to end anastomosis was performed.
Specimen sent for Histopathological Examination. Histopathological examination revealed
multiple sessile polyps with one large 1.5×1cm with dysplastic glands with nuclear atypia and
increased mitotic activity and smooth muscle bundles intersecting glands.
Conclusion:
Peutz –Jeghers syndrome is a rare syndrome. All the infants with pigmented macules in perioral
regions should be investigated for PJS. These patients should be kept for lifelong surveillance
keeping in view the fact that they are more prone for malignancies.
