INSPECTION / MONITORING REPORT OF


DIAMER BASHA DAM PROJECT

M&S (WAPDA) team comprising of Director Monitoring (DBDP) M&S and Dy. Director
Monitoring (DBDP) M&S carried out inspection / monitoring of Diamer Basha Dam
Project (DBDP) from 18.04.2022 to 23.04.2022 and visited various components of
ongoing works i.e. Diversion Tunnel # 02, Diversion Canal, Right & Left Bank Dam
Abutments, Tangir Hydropower Project Works, Right Bank Periphery Road Packages
(01, 02, 03 & 04), Permanent Access Roads, Harpan Das Composite Model Village,
RKKH, WAPDA Colony at Thor including Thor Powerhouse and Cadet College.

The observations and findings on the ongoing/completed activities at site are


discussed in detail as under, however, it is added that all information relating to
progress has been provided by Project office.
1. DAM PART (CIVIL WORKS) & TANGIR HPP WORKS – CONTRACT MW-01

 M/s DBCG (Engineer) has issued notice to Contractor under Contract Clause
46.1 (Rate of Progress) and refused Contractor’s request for EoT-1. Contractor

under Contract Clause 67 (Settlement of Disputes).
 M/s DBCG’s design team is not stationed at site for quick resolution of
Contractor’s design related issues.
 NOC from District Administration is pending for the transportation, storage and
usage of Explosives for Blasting which can hamper the excavation activities.

4. WAPDA PROJECT COLONY AT THOR VALLEY

Observations:
 Training Works of Nullahs (No.3 & 4) are essentially required for the protection of
Thor Colony owing to the damages incurred during the flood witnessed in years
2015, 2020 & 2021.
 Water Supply and Sewerage system yet to be established.
 Colony access road with proper sign boards to be constructed.
 Colony internal roads need to be constructed.
 No bridge over Thor Nullah for access to Thor Power House is available.
 Security check post at colony entrance needs to be strengthened.
 Safety measure is required for Electricity supply in Thor colony wherein electric
poles are being used to support temporary water supply pipelines which is
hazardous.
 Construction of Colony boundary wall / fencing need to be constructed at the
earliest for security of residents.

5. COMPOSITE MODEL VILLAGE AT HARPAN DAS (CMV-II)

i. Contract Package CMV-II-01 - Construction of Roads and Road Structures


Observations:
 No road signage is provided.
 No slope stability measures have been taken along roads.

ii. Contract Package CMV-II-02 - Construction of Water Supply & Sewerage System

6. OBSERVATIONS & RECOMMENDATIONS

i. Delay in Project Completion - Contract MW-01


Completion of most of the Project activities is significantly delayed w.r.t baseline
schedule (R4) including critical activities i.e. River Diversion works & Cofferdams,
Left & Right Dam Abutment works, installation of Aggregate Processing Plant &
Concrete Batching Plant, Source Approval & Material Testing for RCC Mix Design
etc., which has resulted in delay of 315 days in tentative completion of Whole of
the Works under Contract MW-01. The matter needs to be reviewed under
Contract Clause GCC-46.1 [Rate of Progress] by the Project office as it will have
serious impacts on the subsequent Contracts such as HM-01, MW-02 etc.

ii. Approval of River Diversion Scheme - Contract MW-01


River Diversion Scheme is still not finalized despite being one of the most critical
activities for the completion of the Project. The matter needs to be treated as most
urgent by the Project management.

iii. Deployment of Additional Resources by MW-01 Contractor


It has been observed that MW-01 Contractor needs to deploy additional resources
in order to expedite the work and complete the Project activities in time. Following
table shows the progress rate desired vs. current rate of progress of the Contractor
for the excavation of Diversion Canal and similarly all other work fronts can also be
worked out by the Engineer and be reported to PD/CEO DBDC.

iv. Poor Support Work at Diversion Tunnel-2 Outlet


The quality of Rock Support work at DT-2 outlet portal was observed to be poor as
Shortcrete performed was not uniform and Rock bolting was improper.

v. Handing-Over of Land to MW-01 Contractor


Land for Tangir HPP and Sub Camp in Area-3 is still not officially handed over to
MW-01 Contractor which is hampering construction activities.

vi. MW-01 Contractor’s Access to Gini Pozzolan Deposits


Issue of MW-01 Contractor’s access to Gini area is still unresolved due to
confrontations with M/s CGGC (Dasu HPP Contractor), which has suspended the
investigation work of pozzolanic materials at Gini deposits by MW-01 Contractor.
The situation has serious impacts on the critical activities of RCC Mix Design and
the Plant Design.

vii. Relocation of Basri Check post and Other Local Facilities in Area-14
Area-14 is being utilized as Spoil area by MW-01 Contractor and existing
structures i.e. Rangers’ Basri Check post and other local facilities (electricity poles)
need to be completely removed from the area.

viii. WAPDA Thor Colony


Site of WAPDA Thor Colony is prone to landslides and flooding risk as evident
from actual events of flooding witnessed in the past few years (2015, 2020 and
2021) which severely affected the Thor Colony. M/s DBCG (Consultants) has also
showed serious reservations over their accommodation in Thor Colony and
declared it highly unsafe for habitation. Instead, M/s DBCG (Consultants) has
directed the MW-01 Contractor to submit an estimate of cost and time required for
the additional work for prefab-construction of offices and living facilities in Area-6
(11.50 km D/S of Dam) to be dealt as a variation under Clause-51 of the Contract.
Whereas, WAPDA staff is residing in the Colony since long and has no other
option than to live under vulnerable conditions as no safety measures against flood
have been taken yet. Therefore, it is strongly recommended to complete all
remaining works and finalize the training works of side nullahs in Thor Colony at
the earliest.

ix. Solid Waste Management at Project Area


No proper arrangements for the disposal of solid waste are observed in the Project
area and the matter needs to be taken up by the HSE departments of both
Consultant & Contractors.

x. Poor Lighting Facilities at Project Site


Lighting arrangements done by the MW-01 Contractor for working at the sites of
River Diversion Works during night shift observed to be improper, hence, the
lighting conditions need to be improved.

xi. Absence of HSE Staff at Site


During M&S team visit, no HSE personnel (either from Contractor or Consultant)
was observed at MW-01 sites which is in contradiction with the HSE standards and
can result in any untoward incident. Furthermore, the availability of PPEs for the
WAPDA staff be ensured during site visits.

xii. Operation of WAPDA Hospital at Thor Colony


Building of WAPDA Hospital at WAPDA Thor Colony has been completed and it
needs to be equipped and made functional on priority basis to cater any kind of
medical emergencies at site.

xiii. Construction of Access Road to WAPDA Office Chilas


At present, there exists no paved access to WAPDA office Chilas which has also
been noticed by the worthy Chairman WAPDA during his visit to the Project.

Therefore construction of paved road from DHQ Hospital to WAPDA office Chilas
is essentially required and necessary action should be taken by the Project office
in this regard.

xiv. Plantation in Project Area


No plants/trees were observed in WAPDA office at Chilas and WAPDA Thor
Colony, therefore, plantation needs to be done for the benefit of environment and
landscaping of the WAPDA premises in Project area.

xv. Shortage WAPDA Engineers at Project Site


It has been observed with grave concern that there is only one Senior Engineer
posted at Project site and several positions of Middle Management are lying
vacant.

xvi. Active Involvement of WAPDA Technical Staff at Site


WAPDA Engineers/Technical Staff deputed at site observed not to be actively
involved in the Project activities and following recommendations may be
considered to improve the work efficiency of the Engineers:
1. Clearly defining work specific assignments related to Construction
Supervision
2. Deputation on discrete components of the Project
3. Close coordination with the Consultant/Contractor at Site
4. Availability of road worthy (4x4) vehicle at site
5. Provision of Contract Documents, IFC drawings, Method statements of
ongoing activities to WAPDA Engineers at site
6. Provision of Desktop / Laptop and printer at WAPDA site cabin

xvii. Provision of Record to M&S Monitoring Team WAPDA during Site Visits
It has been seriously noticed during the site visit that Consultants (M/s DBCG) was
unable to provide the information required by M&S Monitoring Team owing to the
unavailability of data at site, neither was it provided lately. It is therefore requested
that the provision/availability of data be ensured at site for the ready reckoning of
M&S Monitoring Team in future.

xviii. Sharing of Official Information with Director (Monitoring) DBDP, M&S Office
It has been noticed with grave concern that the office of Director (Monitoring)
DBDP, M&S is not receiving emails anymore regarding official
correspondence among Project Stakeholders (i.e. Employer, Consultants and
Contractors) relevant to the Project activities since 21.04.2022 which is essentially
required for the performance of this office in the best interest of Authority’s work.
The same reservation was also communicated to the Project office vide letter
dated 11-05-2022, which has not been addressed yet.

xix. Tribute to Engr. Muhammad Shafiq Ahmad (Late), Director (Coordination)


DBDP, WAPDA
Worthy WAPDA Management is requested to pay tribute to “Engr. Muhammad
Shafiq Ahmad (Late)” who served as Director (Coordination)/Chief Engineer,
WAPDA at Project site, Chilas and died on Feb 10 th, 2022 in the line of duty. He
had been actively involved in the preparation of PC-1 (AL&R) and coordination with
locals and District Administration regarding compensation payments which made
its way for smooth running of the Project. His services for WAPDA and the Project
needs to be appreciated in the form of tribute such as naming any Building Block of
WAPDA Colony or road of WAPDA Office / Colony after him. He will be
remembered always especially for his services for resettlement issues and his
legacy will remain immortal.

All the recommendations are presented in the best interest of Project and may be
considered without cost and time impact under intimation to this office positively.
