AFFIDAVIT OF DISCREPANCY

I, SALOMAE MUTYA LAMBO, Filipino Citizen, of legal age,


single, presently living at Blk 6 Lot 16 Umakap Ka Cmpd
Fatima I, City of Dasmariñas, Cavite, under oath, hereby
deposed and state the following:

1. That I am executing this AFFIDAVIT in support to


my application for MARRIAGE LICENSE before the
Civil Registry of City of Dasmariñas, Cavite;

2. That I was born 28 January 1997 to my parents


ELY RANIS LAMBO and JEAN SUMAGANG MUTYA;

3. That in compliance with the above-application, I


secured a copy of my CENOMAR and the same was
issued under-control number 08019-H1-144JDV-
00184-ME030. However, in my Certificate of LIVE
BIRTH clerical error was committed because the
middle name of my father was written as “RANES”;

4. Based on records, the real name of my father is ELY


RANIS LAMBO and not ELY RANES LAMBO as
appearing in my CENOMAR;

5. That I am attesting to the truthfulness that ELY


RANIS LAMBO and ELY RANES LAMBO as
appearing in my certificate of live birth and
appearing in my CENOMAR respectively is one of
the same person; That the said person is my
biological father;

6. That I am executing this affidavit not for the


purpose of fraud, perjury and to perpetuate
injustice.
